[Endoscopic sphincterotomy].

The last 105 endoscopic sphincterotomies performed between 1983 and 1987 are evaluated and compared with the recent data of the literature. The sphincterotomy has been successful in 98 cases (93%). The indications were: 95 cases of common bile duct stones and 10 inflammatory stenoses of the duodenal papilla. The stones have been removed in all cases of sphincterotomy for choledocholithiasis (100% of the successful sphincterotomies, 93% of all attempts). The morbidity rate was 5%. There has been no need for emergency intervention. Mortality was zero. In order to obtain high success rates with e

[Quo vadis endoscopic sphincterotomy?].

The results of a study from 25 centers (= Series I: 1974-1980) covering 9041 endoscopic sphincterotomies (EST) were compared with those of a second study from 20 centers (= Series II: 1981-1986) covering 10177 cases. A change was seen in the indications during the past 5 years: While choledocholithiasis after cholecystectomy remained the main indication, EST is performed with increasing frequency in patients with common bile duct stones having their gallbladder in situ as a definitive method with low complications (only 0.61% emergency cholecystectomies): Circumscript papillary stenosis became

Bleeding after endoscopic sphincterotomy.

A total of 194 patients who underwent 235 endoscopic sphincterotomies for common bile duct (CBD) stones were reviewed with respect to bleeding after the procedure. Evidence of bleeding was found in 21, but was occult in 7. Of the 14 patients with overt bleeding, 6 required operation and 2 died. There was also one death prior to surgery. Early consultation with surgical colleagues, observance of the usual surgical criteria for upper gastro-intestinal haemorrhage and the use of non-absorbable suture material to achieve haemostasis may reduce the morbidity and mortality of this complication of en

Endoscopic sphincterotomy in outpatients.

We retrospectively reviewed 137 cases of outpatient endoscopic sphincterotomy (ES) performed over a 4-year period in a single center and compared them with an equal number of inpatient ES. The indications for ES in outpatients as compared with inpatients were, respectively: choledocholithiasis, 60% and 70%; papillary stenosis, 35% and 15% (p less than 0.001); stent insertion, 3.6% and 14% (p less than 0.01); and ampullary tumor, 1.4% and 0.7%. Complications were noted within 2 to 4 hours of ES in 6.6% of outpatients, a rate similar to that of inpatients--7.3%. Outpatients with complications we

Endoscopic sphincterotomy for acute pancreatitis.

There is now considerable evidence linking the severity of gallstone-associated acute pancreatitis with obstruction to both the bile duct and the pancreatic duct by "persisting" stones either because of continuous or repeated impaction, or repeated passage. Two prospective trials have shown endoscopic sphincterotomy to be superior to other forms of treatment, but only in severe cases, and general experience now extends to over 1000 cases so treated. Endoscopic sphincterotomy is also very effective in acute pancreatitis associated with acute cholangitis. Prophylactic endoscopic sphincterotomy m

[Gallstone ileus after endoscopic sphincterotomy].

The authors report on a case of gallstone ileus developed after endoscopic sphincterotomy (EST). The patient had to be operated and recovered uneventfully after enterotomy. Emphasis is placed on the importance of patient's observation after EST so this rare complication could be revealed in time.

Biliary sequelae of endoscopic sphincterotomy.

Twenty five patients were reviewed a mean of 36 months after successful endoscopic sphincterotomy for the removal of bile duct stones. All the patients had improved symptomatically but 20% had episodes of mild abdominal pain and a similar number had elevated serum gamma glutamyltranspeptidase activities (up to 3 times normal). In 12 patients (50%) biliary gas was demonstrated indicating reflux of duodenal contents. Clinical cholangitis did not occur. Aspiration liver biopsy revealed mild portal tract fibrosis and inflammation in patients with biliary reflux. Biliary reflux was significantly as
