Levodopa (LD) is the most effective drug for symptomatic control of Parkinson's disease, but has been suspected to be toxic to substantia nigra (SN) dopaminergic neurons. Vascular parkinsonism (VaP) is a term used to describe a parkinsonism of vascular cause. However, only ischemic or hemorrhagic lesions in the substantia nigra or nigrostriatal pathway, leading to a reduction of dopaminergic stimulus are pure VaP. Here, we report a case of true VaP due to nigrostriatal pathway stroke with accumulation of neuromelanin in Substantia nigra. This is the first description of neuromelanin accumulation from nigrostriatal damage. Also, it can help to understand the physiopathological changes in VaP.

Laboratory studies and studies of animal models of parkinsonism have produced contradictory evidence, but there is no evidence that LD is toxic to normal substantia nigra in animals. We have studied human subjects longitudinally to address that issue. A cumulative LD dose up to 24 kg was not toxic in one autopsied essential tremor (ET) case. We report a patient with right hemiparkinsonism following haemorrhage in the left substantia nigra. The hemiparkinsonism responded to treatment with trihexyphenidyl hydrochloride and deteriorated after temporary discontinuation of the drug. Single photon emission computed tomography using technetium 99m d, l-hexamethylpropyleneamine oxide showed reduced uptake in the left putamen, globus pallidus and thalamus.

A 16-year-old boy presented with febrile encephalopathy with meningeal signs, abnormal eye movements and extrapyramidal features. Clinical impression was meningo-encephalitis due to infective aetiology. He was diagnosed as a case of Japanese encephalitis. MRI brain revealed bilateral, symmetrical hyperintensities of substantia nigra. This neuro-imaging finding is very rarely reported in Japanese encephalitis.

The role of substantia nigra (SN) in the cat cognitive activity of different complexity degree, was investigated by original technique. Neurosurgery or neurochemical SN switching off leads to reliable disturbances of condition, reflexes, generalization and abstraction. Rehabilitation was possible after pharmacological stimulation of dopaminergic, partly GABA-ergic, and cholinergic systems. Stimulation of serotonin system was ineffective.

Infectious mononucleosis due to Epstein-Barr virus (EBV) is a usually benign systemic viral illness common in children. Many studies described nervous system manifestations of infectious mononucleosis with a wide spectrum of neurologic deficits. Neurologic complications of EBV are seen in both acute and reactivate infection. Herein, we describe a patient diagnosed by acute EBV encephalitis with substantia nigra involvement and excellent clinical recovery.
