BACKGROUND: To date there has been no direct comparison of two frequently used endovascular thrombectomy (EVT) methods (forced arterial suction thrombectomy (FAST) and stent retriever thrombectomy) in M2 occlusions. We review our experiences with EVT performed using FAST and stent retriever thrombectomy in such cases.
METHODS: The subjects comprised 41 patients with an M2 occlusion who underwent EVT (25 with FAST, 16 with stent retriever thrombectomy). The patients' data were retrospectively analyzed to evaluate the technical characteristics and angiographic outcome of the two EVT techniques.…

Distal embolization of thrombus can lead to impairment of microvascular perfusion, and measures of abnormal microvascular perfusion have been associated with increased mortality and worsened clinical outcomes. Large multicenter randomized controlled trials and multiple meta-analyses have failed to demonstrate an improvement in clinical outcomes with the routine use of manual aspiration thrombectomy, with some studies suggesting an increased incidence of stroke, likely owing to thrombus dislodgement during retrieval leading to cerebral vessel embolization. In patients with high thrombus burden …

Clot-induced blockage in arteries or veins can cause severe medical conditions1. Mechanical thrombectomy is a minimally invasive technique used to treat ischaemic stroke, myocardial infarction, pulmonary embolism and peripheral vascular disease2-4 by removing clots through aspiration5, stent retriever6 or cutting mechanisms7. However, current mechanical thrombectomy methods fail to remove clots in 10-30% of patients8-10, especially in the case of large, fibrin-rich clots11. These methods can also rupture and fragment clots12, causing distal emboli and poor outcomes13. To overcome these challen…

Twentyeight patients with ileofemoral venous thrombosis were treated surgically. Five of the patients had moderate degree of venous congestion, 18 patients had phlegmasia alba dolens and five patients had phlegmasia coerulea dolens. The mean age was 54 years, range 15-80 years, and 15 were men and 13 were women. In all cases the thrombosis was verified by phlebography. Thrombectomy was performed with a Fogarty venous thrombectomy catheter. Peroperative phlebography was used in most cases to guarantee complete extraction of thrombotic material. No operative pulmonary embolism or mortality was e…

Acute and subacute ischemia of the limbs is still one of the most frequent reasons for amputation. There are different therapy options for reperfusion therapy including balloon thrombectomy, local lysis and rotational thrombectomy. Balloon thrombectomy, the standard surgical treatment, has proven an effective tool for the treatment of acute vascular obstructions. Nevertheless technical problems and complications can occur. Localized lysis on the other hand could be time - consuming and cost intensive, requiring intensive care monitoring and reangiograms. To achieve a successful result, at leas…

BACKGROUND: Endovascular treatment (EVT) is a well-established approach for acute ischemic stroke. Whether bridging intravenous thrombolysis (IVT) before EVT confers any benefits remains uncertain. The objective of the study was to compare the efficacy and safety of direct EVT with or without bridging IVT in patients with acute basilar artery occlusion (BAO).
METHODS: This multicenter cohort study enrolled 647 patients with acute BAO who underwent either bridging IVT before EVT or direct EVT from the BASILAR registry. The primary outcome was an independent functional outcome measured by the m

CLINICAL PROBLEM: Ischemic stroke is the most common cause of disability in adults and an important disease for society; therefore, the prevention, diagnosis, and treatment play an important role. Endovascular mechanical recanalization to reopen large occlusions of the intracerebral vessels has established itself as the gold standard in therapy, which is why knowledge of the most important complications and their management is essential for interventionalists.
PRACTICAL RECOMMENDATIONS: Intracranial hemorrhages, which include intracerebral and subarachnoid hemorrhages, are dreaded complicatio

Mechanical thrombectomy (MT) has become the standard procedure in the treatment of patients with acute ischemic stroke (AIS) due to occlusion of a large proximal cerebral artery of the anterior circulation. Nevertheless, according to the current guidelines large patient collectives are still excluded from this highly effective treatment method. Therefore, this article gives an overview of possible extensions of the indications for treatment with MT. For example, patients in the extended time window with distal occlusions, with large infarct cores and also for very old (90+ years) or young (0-1…

Deep venous thrombosis is a common source of morbidity and mortality in the United States. Complications include pulmonary embolism and chro … sis, and cost. As a result, alternative measures for managing DVT have evolved including mechanical thrombectomy. Mechanical thrombectomy for DVT has the potential to shorten the time for lysis, reduce the risk of thrombolytic agents, and potentially impact cost savings.

Percutaneous thrombectomy (PT) is an established technique for the removal of acute thrombus in occluded arteries, veins and vascular grafts. Percutaneous thrombectomy can be used as an adjunctive treatment to other methods of thrombus removal such as thrombolysis or as sole therapy. The two main methods are percutaneous aspiration thrombectomy in which thrombus is removed by suction with the aid of wide-bore catheters, and mechanical thrombectomy using a variety of automated devices to fragment or remove thrombus. Aspiration thrombectomy is often used as an adjunct to thrombolysis in acute ar…

Thrombectomy has a place in the treatment of acute venous thrombosis. However, this is only the case in certain conditions. Its use is indisputable in true blue venous thrombosis which is a rarely occurring pathology. It may be proposed and discussed for recent thrombosis, of a known and correctable cause, blocking one or several important venous channels in a patient likely to develop an invalidating postthrombotic syndrome.
