Of all operations in general surgery, those on the thyroid have a special esteem, because the introduction of safe thyroid surgery by masters like Joll and Dunhill in this country and Crile and Lahey in the United States saved young lives, and returned the unpleasant features associated with untreated thyrotoxicosis to normal. The emphasis of these Princes of Surgery to anatomic detail and careful surgery with good haemostasis remain the hallmark of thyroid surgery today.

Recent randomized series did not support routine prophylactic drainage after thyroidectomy. We undertook a prospective study in order to evaluate the effectiveness and the morbidity of a non drainage strategy after thyroidectomy. Thyroxine and calcitonin secreted by the thyroid gland are reduced in 26 cases patients with thyroid cancer after total or subtotal thyroidectomy. Since calcitonin participates in bone mineral metabolisim, bone mineral density (BMD) was examined in these patients treated in the past 10 years. It was found that serum calcitonin level was decreased, and the degree of reduction positively correlated with BMD (r = 0.78, P < 0.005). Osteoporosis developed about 10 years of age earlier. 94 patients with postoperative recurrent hyperthyroidism were evaluated for duration of remission, goitre size, and response to radio-iodine (131I). 6 patients required 131I therapy within twelve months of operation--5 had large remnants because of inadequate surgery. 57% of patients relapsed within 5 years, but 16% relapsed after 20 years and 8% after more than 30 years. Estimated goitre weights ranged from 4 g to 65 g, and goitre size was unrelated to the duration of remission.
