OBJECTIVE: To explore the serological feature and molecular mechanism for a case with A307 subgroup of the ABO blood group system.

Blood group antigens are red blood cell (RBC) surface markers comprising specific carbohydrate moieties attached to the glycolipids and glycoproteins within the membrane. In addition to the major ABO blood group antigens, at least 35 minor blood group antigens have been defined to date. These antigens have immunogenic potential and may cause a transfusion reaction. ABO discrepancy refers to incongruence between the results of red cell and serum groupings. One such case is described here; the discrepant results of whose routine ABO grouping led to the diagnosis of common variable immunodeficiency. There was no reaction in the reverse grouping of a young patient presenting with recurrent bacterial infections, pointing towards an absence of antibodies in the serum. Diagnosis was made on the basis of markedly decreased serum immunoglobulin levels and by serum protein electrophoresis showing scanty gamma regions.

BACKGROUND: There is a lack of evidence for guiding the best strategy for ABO selection of platelet (PLT) transfusions. As a baseline for future studies, the BEST Collaborative performed an international survey of current practices in this area.
STUDY DESIGN AND METHODS: An international survey was sent via BEST members to transfusion services and hospitals requesting the demographics of the transfused patient population, ABO matching policies, anti-A and anti-B measurements in PLT concentrates (PCs), and practices regarding ABO-incompatible PC transfusions to adult and pediatric patients.

BACKGROUND: In the pediatric population, hemolysis after out-of-group platelet (PLT) transfusion is a potentially fatal event that is thought to be underrecognized. Group A patients transfused with group O single-donor PLTs (SDPs) with "high-titer" anti-A are at greatest risk for hemolysis.

The first examples of hyperacute rejection of renal hemografts were seen almost 25 years ago when kidneys were transplanted to ABO incompatible recipients whose plasma contained antigraft isoagglutinins.
