Effects of adenoidectomy or adenotonsillectomy on the cardiovascular system in children: a meta-analysis.

OBJECTIVE: Adenoid or adenotonsillar hypertrophy (AATH) causes upper airway obstruction, leading to cardiovascular complications. This meta-analysis was conducted to evaluate the efficacy of adenoidectomy or adenotonsillectomy (AATE) on the cardiovascular system. METHODS: Using the Cochrane Central Register of Controlled Trials, PubMed, and EMBASE databases, we identified studies involving a comparison of preoperative and postoperative cardiovascular function in children with AATH. The Cochrane Collaboration's Review Manager 5.3 software was used for meta-analysis. RESULTS: A total of 13 stu…

Radiological and Audiological Assessment in Patients with Adenoid Hypertrophy Undergoing Adenoidectomy.

Large amounts of d-2-hydroxyglutaric acid (DGA) accumulate in d-2-hydroxyglutaric aciduria (D-2-OHGA), an inherited neurometabolic disorder characterized by severe neurological dysfunction and cerebral atrophy. Despite the significant brain abnormalities, the neurotoxic mechanisms of brain injury in this disease are virtually unknown. In this work, the in vitro effect of DGA on various parameters of oxidative stress was investigated; namely chemiluminescence, thiobarbituric acid-reactive substances (TBA-RS), total radical-trapping antioxidant potential (TRAP), total antioxidant reactivity (TAR

Correlation of mucociliary clearance and symptomatology before and after adenoidectomy in children.

OBJECTIVE: The goal of this study is to better understand the role of adenoid hypertrophy and its impact on mucociliary clearance as it relates to the symptoms of chronic sinusitis in children. More specifically, the goal is to better understand which symptoms of chronic rhinosinusitis might be most likely to improve in children undergoing an adenoidectomy. METHODS: We conducted this study on 35 healthy children (>3 and <18 years of age), either male or female, who underwent adenotonsillectomy or adenoidectomy (with or without tympanostomy tube insertion). Eighteen patients completed pre- and…

Assessment of the consensus about tonsillectomy and/or adenoidectomy among pediatricians and otolaryngologists.

BACKGROUND: Several recent publications have shown that the current indications for tonsillectomy and/or adenoidectomy (T&A) in children vary considerably between and even within countries. The objective of our study is to present statistically valid information to surgeons and pediatricians, primary care physicians, and family physicians as key referral channels to the operation, whether there is consensus between pediatricians and otolaryngologists regarding the appropriateness of T&A. METHODS: Six academic pediatricians and six otolaryngologists participated in the study. After otolaryngol…

Central incisor width as a predictor of appropriate curette size in adenoidectomy.

As inadvertent eustachian tube injury during adenoidectomy can have serious short- and long-term implications, a simple anatomic correlate that could predict the optimal curette choice for adenoidectomy, especially in severely hypertrophic cases, is beneficial. This study evaluates the correlation of the distance between the lateral borders of the upper central incisors and the distance between the tori tubarius in the nasopharynx. One hundred one consecutive patients undergoing adenoidectomy at a pediatric tertiary care hospital were enrolled in this study. The patients ranged in age from 7 m…

[Obstructive sleep apnea syndrome in children; an indication for tonsillectomy and adenoidectomy].

A 3-year-old girl with obstructive sleep apnea syndrome associated with tonsillar and adenoid hypertrophy is discussed. Nocturnal sleep registration, 4 months after adeno-tonsillectomy revealed complete arrest of apnea episodes and there was improvement in sleep pattern. In the early detection of obstructive sleep apnea in children a careful history and nocturnal sleep registration are essential. In children, tonsillar and adenoid hypertrophy are the main causes of obstructive sleep apnea.

Role of tonsillectomy and adenoidectomy in the management of children with middle ear effusion.

Evidence for the clinical effectiveness of adenoidectomy or tonsillectomy and adenoidectomy in the management of children with persistent middle ear effusions is incomplete and what evidence there is the literature is not convincing. In the first year after tonsillectomy and adenoidectomy there is less otitis media. The beneficial effects of tonsillectomy and adenoidectomy on the incidence of otitis media declines in the second year after the operation. The prevailing opinion is that adenoidectomy is not indicated in the vast majority of children in whom a middle ear effusion has persisted for…

Retropharyngeal emphysema following adenoidectomy: a case report and review of literature.

Retropharyngeal infection and emphysema are rare clinical entities, with only eight reported cases in the literature. One of the uncommon risk factors is a recent history of adenotonsillectomy, which may be associated with an increased risk of retropharyngeal infection. Thereby, we report a case of a 2-year-old female who presented with a high-grade fever and neck stiffness following adenotonsillectomy. Neck X-ray and computed tomography scan of the neck confirmed the diagnosis of retropharyngeal infection with emphysema. The patient was effectively managed with an intravenous course of dual b

Improvement in allergic and nonallergic rhinitis: A secondary benefit of adenoidectomy in children.

Chronic rhinitis (CR) is a common disorder in children. Allergic rhinitis (AR) is a risk factor for CR, and children with AR tend to suffer more from hypertrophic adenoids than do patients with nonallergic rhinitis (NAR). Few studies have addressed the issue of alleviating symptoms of pediatric CR or AR following adenoidectomy alone. We conducted a retrospective chart review to determine whether CR in children improves after adenoidectomy and whether children with AR will benefit more than those with NAR. Charts of 47 children who had undergone adenoidectomy for nasal obstruction and chronic m…
