[Panic disorder and agoraphobia: what is effective?].

The present survey of controlled studies on the treatment of panic disorder and agoraphobia (PDA) reveals that treatment with tricyclic antidepressants (e.g., imipramine and clomipramine), benzodiazepines (e.g., alprazolam), serotonin reuptake inhibitors (e.g., fluvoxamine) and the monoamine oxidase inhibitor phenelzine has been proven effective. Among psychological therapies, cognitive therapy and exposure therapy in agoraphobia have been shown to be effective. There is an insufficient number of comparisons between pharmacological and psychological treatments. From the existing studies it can…

Cognitive mediators of situational fear in agoraphobia.

The aim of this study was to examine cognitive mediators of situational fear in agoraphobia. Patients suffering from panic disorder with moderate or severe agoraphobia were randomly assigned to receive either cognitive therapy or guided mastery therapy in a 6-week inpatient group program. The results were consistent with a self-efficacy model of agoraphobia. Across test occasions, and when the other cognitive variables were controlled for, self-efficacy made a significant contribution to the prediction of situational fear or changes in situational fear in 5 out of 5 cases. Catastrophic beliefs…

Combined behavioral therapy and pharmacotherapy of agoraphobia.

There are few controlled studies of combined behavioral-pharmacological treatments of agoraphobia with panic. This review of the empirical evidence from studies using exposure in vivo as the behavioral modality and imipramine as the pharmacological agent suggests that these treatments have mutually potentiating effects in this disorder.

Patterns of comorbidity in panic disorder and agoraphobia.

Diagnoses of comorbid disorders were determined in a sample of 54 patients with panic disorder as defined in DSM-III-R. The sample was divided into the following three groups: (1) uncomplicated panic disorder (PDU); (2) panic disorder with mild agoraphobia (PDM); and (3) panic disorder with moderate to severe agoraphobia (PDA). In comparison with patients with PDU, patients with PDA had higher comorbidity rates in general, received multiple comorbid diagnoses more frequently, had a higher prevalence of major depression, dysthymia, social phobia, generalized anxiety disorder, and obsessive-comp…

[An ambulatory model for treatment of panic and agoraphobia].

The article describes an 11-session outpatient treatment programme in groups for patients with panic disorder and agoraphobia. The main components are cognitive-behavioural therapy and use of tricyclic antidepressives. Preliminary results after the first year of this programme indicate that most patients were much improved after completing the programme, and most of them had maintained their gains at follow-up.

Panic attacks with and without agoraphobia: a comparison.

22 patients with panic disorder (PD) were compared with 42 cases suffering from agoraphobia with panic attacks for a number of variables. The two groups did not differ for age, sex ratio, age of onset, social class, severity of nonsituational anxiety and personality profiles. On the other hand agoraphobics showed lower education and worse social adaptation. A higher prevalence of traumatic life events was also observed for agoraphobics compared with subjects suffering from PD.

Treatment of panic disorder and agoraphobia with clonazepam.

Clonazepam, a high-potency benzodiazepine marketed for the treatment of minor motor epilepsy, was used to treat 50 patients with panic disorder (N = 22) or agoraphobia with panic attacks (N = 28). Of the 50 patients, 41 had previously been poorly responsive to standard pharmacologic therapies. At a mean dose of only 1.9 (+/- 1.0) mg/day, 39 patients (78%) responded. No serious adverse effects were encountered. This study, although retrospective and uncontrolled, suggests that clonazepam, like alprazolam, may be effective in blocking panic attacks. A possible common mechanism for the two drugs …

Autonomic characteristics of agoraphobia with panic attacks.

We compared electrodermal and heart rate measures of autonomic activation between patients meeting DSM-III criteria for agoraphobia with panic attacks and controls in terms of tonic level, reactivity to various types of stimuli, recovery, habituation, and spontaneous variability. The most striking differences between groups in the laboratory were higher tonic levels of skin conductance and heart rate among patients. Patients' heart rates were also tonically elevated in a test situation outside the laboratory. Certain measures of habituation and spontaneous variability also differed between gro…

The spouse as co-therapist in the treatment of agoraphobia.

Involving the spouse as a co-therapist made no significant difference to the outcome of treatment in a controlled study of 19 married patients suffering from agoraphobia who were engaged in short-term home-based behavioural treatment. Improvement continued to 6-month follow-up. The marriages of those with some marital difficulties at the start tended to improve, whether or not the spouse was involved. Poor initial marital adjustment was not related to the degree of improvement in agoraphobic symptoms.
