Opioid Literacy Among Individuals Living in Rural Alabama: The Role of Social Determinants of Health.

Using the social determinants of health (SDOH) framework, the current study aimed to examine opioid literacy and the role of SDOH on opioid literacy. This study used a cross-sectional survey design to collect self-reported data from people living in four rural Alabama counties affected by the opioid crisis. Participants reported moderate levels of opioid knowledge. There were no significant predictors of general knowledge. For opioid overdose knowledge, the strongest individual predictors were educational level (Bachelor's degree) and self-rated financial strain, which contributed to higher sc…

A Geospatial Analysis of Social and Structural Determinants of Health and High HIV Prevalence in Alabama, USA.

OBJECTIVE: This study utilizes geospatial analytic techniques to examine HIV hotspots in Alabama leveraging Medicaid utilization data. METHODS: This cross-sectional study leveraged Medicaid utilization data from Alabama's 67 counties, averaging 9,861 Medicaid recipients aged > 18 years old per county. We used Alabama Medicaid administrative claims data from January 1, 2016, to December 31, 2020, to identify individuals with HIV. Using Microsoft SQL Server, we obtained the average annual count of HIV Medicaid claims in each of the 67 Alabama counties (numerator) and the number of adult Medicai…

The effects of West Nile virus on the reproductive success and overwinter survival of eastern bluebirds in Alabama.

We tested for negative effects of West Nile virus (WNV) on a breeding population of eastern bluebirds in Alabama by comparing fecundity and reproductive success in years before and after the arrival of WNV and by comparing fecundity, reproductive success, and overwinter survival of seropositive and seronegative individuals within the same population in the same years. We found that female bluebirds were more likely to be seropositive than male bluebirds. Age and individual condition did not affect likelihood of being seropositive. Being seropositive for WNV was not associated with any negative…

Prevalence of and risk factors associated with shedding of Cryptosporidium felis in domestic cats of Mississippi and Alabama.

The prevalence of and risk factors associated with the shedding of cryptosporidial oocysts were determined for domestic cats in northeastern Mississippi and northwestern Alabama. Cryptosporidial oocysts were found in a single fecal samples from 30 of 250 cats using a centrifugal concentration technique followed by an immunofluorescent assay. The odds of a cat shedding oocysts were increased when another cat shedding oocysts was present in the household. Logistic regression analysis showed only concurrent shedding of Giardia cysts to be significantly associated with the presence of Cryptosporid…

Incidence and risk factors for sexually transmitted infections among women in an Alabama HIV clinic.

The objectives of this study were to assess the prevalence and incidence of curable sexually transmitted infections (STI), i.e., gonorrhea, chlamydial infection, or trichomoniasis, in a cohort of HIV-infected women. The study population was derived from women seeking primary care at an outpatient university HIV clinic who were participants in a women's natural history study. Enrollees (n = 225) were predominantly African-American, heterosexually infected, with mean age 35 years. Mean entry CD4+ T cell count was 405 cells/mm3. Over 6% were STI positive at initial screening. Subsequently, the co

Prevalence of Leprosy-Causing Bacteria in Mexican Long-Nosed Armadillos (Dasypus mexicanus) in Alabama, USA.

Previous studies have examined prevalence of leprosy-causing bacteria, Mycobacterium leprae and Mycobacterium lepromatosis, in rural Mexican long-nosed armadillo (Dasypus mexicanus) populations of the southeastern USA. However, few prevalence studies have been conducted in Alabama nor in urbanized, human-dominated areas where potential zoonotic transmission may be a public health concern. This study evaluated the prevalence of leprosy-causing bacteria in armadillos from rural, suburban, and urban zones in Lee and Mobile County, Alabama, USA. This study detected the first evidence of M. leprae-…

Therapeutic Error Calls Among Older Adults Reported to a Regional Poison Control Center in Alabama.

OBJECTIVES: Although Americans aged 65 years and older account for only 13.0% of the population, they consume one-third of all prescription medications each year. Increased life expectancy, age-related deterioration in health, and polypharmacy lead to a significant risk of potential medication errors. National Poison Data System studies have evaluated the older adult population and their interaction with poison centers; however, descriptive studies using regional poison center data to evaluate older adult-related medication errors, specifically in Alabama, have not been conducted. Our study as…

An Innovative Approach for Increasing Access to Over-the-Counter Hearing Aids in Rural Communities of Alabama.

Adults living in rural communities of Alabama have limited or no access to hearing healthcare for a variety of reasons. Hearing loss can result in decreased social contact, potentially leading to loneliness, and it has been associated with depression and cognitive decline. To improve access to hearing healthcare, the FDA released its Final Rule in 2022 stating that adults with perceived mild-to-moderate hearing loss can purchase over-the-counter hearing aids (OTC HAs) on their own without clearance from a physician or through the care of an audiologist. Current research suggests, however, that…

Three cases of congenital rubella syndrome in the postelimination era--Maryland, Alabama, and Illinois, 2012.

Infection with rubella virus during pregnancy, especially during the first trimester, can result in congenital rubella syndrome (CRS). Serious manifestations of CRS include deafness, cataracts, cardiac defects, mental retardation, and death. In the last major rubella epidemic in the United States, during 1964-1965, an estimated 12.5 million rubella virus infections resulted in 11,250 therapeutic or spontaneous abortions, 2,100 neonatal deaths, and 20,000 infants born with CRS. In 2004, after implementation of a universal vaccination program, elimination of endemic rubella virus transmission wa

Idiopathic thrombocytopenic purpura: a 10-year natural history study at the childrens hospital of alabama.

Childhood idiopathic thrombocytopenic purpura (ITP) is a common disorder. However, single-institution, long-term, natural history data are limited. The objective of this paper is to review presenting features, response to therapy, and natural history of ITP treated at a single pediatric academic medical center. A retrospective chart review was made for all children (ages birth-18 years) diagnosed with ITP (ICD 287.3) and treated at the Childrens Hospital of Alabama/University of Alabama at Birmingham between 1993 and 2003. Four hundred nine patients were identified (49% male, 51% female; mean …

Mental Health Emergency Department Visits and Hospitalizations Among Alabama Publicly Insured Youth During COVID-19.

OBJECTIVE: Increases in the proportion of pediatric emergency department (ED) visits due to mental illness (MI) or intentional self-harm (ISH) in early months of the COVID-19 pandemic have been documented. Less is known about hospitalizations, or trends during later months of the pandemic. This study assessed changes in MI/ISH-related ED visits and hospitalizations among enrollees in Alabama's Children's Health Insurance Program, ALL Kids, during the pandemic. METHODS: Administrative claims data from ALL Kids, were used for enrollees aged 6-17 from 2017 to 2022. The study-period was divided i…

Venous thromboembolism in pediatric patients: epidemiologic data from a pediatric tertiary care center in Alabama.

Venous thrombosis is an infrequent but serious cause of hospitalization in children. The epidemiology and natural history remains incompletely defined, especially in geographically distinct regions of the United States. We thus evaluated thrombosis in a single children's hospital over a 3-year period. Of 41,906 hospitalizations, 92 children were identified for review. The incidence of thrombosis was 21.9 per 10,000 admissions (0.22%). Venous thrombosis was of equal incidence in African-American and white patients. Locations of thrombosis included deep venous (51%), pulmonary (21%), renal vein
