The term "centrocentral anastomosis" is used to describe the end-to-end connection across interposed nerve grafts between paired fascicular groups of the proximal stump of a severed nerve. In 22 patients harboring a painful terminal neuroma following amputation of a lower limb (20 neuromas on the sciatic nerve and two on the peroneal nerve), a centrocentral anastomosis was performed on the end of the sectioned nerve to treat pain that had not improved with conventional conservative treatment. Follow-up review at 1 year revealed that the typical neuroma pain had disappeared in all cases, althou

BACKGROUND: Many factors affect the rehabilitation of lower limb amputation. One of these factors is the length of the stump.
OBJECTIVE: The aim of this study was to assess the relationship of stump length with muscle strength, proprioception, and balance in patients with traumatic unilateral transfemoral amputation.
STUDY DESIGN: Sixteen patients with traumatic unilateral transfemoral amputation were included in this cross-sectional study.
METHODS: Stump length was determined by measuring the distance from the trochanter major to the tip of the stump. Hip extensor and flexor muscle strengt

Preservation of the knee joint has enormous advantages in terms of mobility and rehabilitation of an amputee. Any cause of breakdown requiring revision to an above-knee amputation is a major setback because it reduces the patient's rehabilitative potential. We report a case of intra-arterial thrombolysis use to save a below-knee amputation stump with acute ischemia. A 56-year-old man who sought the emergency department with 1-day history of acute pain on his right below-knee stump. The angiography confirmed popliteal artery occlusion. Pharmacomechanical thrombectomy, with Aspirex (rotational c

BACKGROUND: This is the first update of a Cochrane review published in Issue 5, 2010 on transcutaneous electrical nerve stimulation (TENS) for phantom pain and stump pain following amputation in adults. Pain may present in a body part that has been amputated (phantom pain) or at the site of amputation (stump pain), or both. Phantom pain and stump pain are complex and multidimensional and the underlying pathophysiology remains unclear. The condition remains a severe burden for those who are affected by it. The mainstay treatments are predominately pharmacological, with increasing acknowledgemen

PURPOSE: To predict healing of the stump by assessing the microscopic vascular changes at the amputation site.
METHODS: A cohort study was conducted on 39 patients, 18 of them had below-knee amputation (group A) and 21 had ray amputation of a single toe (group B). Biopsies were taken from the anterior and posterior tibial arteries and the venae comitantes of group A patients. For group B patients, biopsies of the digital artery and dorsal vein of the toe were taken.
RESULTS: In group A, 15 patients required no further amputation (group A1) and 3 underwent a further above-knee amputation (gro

BACKGROUND: A multicentre randomised controlled trial to determine the effect of a rigid plaster dressing applied at the time of trans-tibial amputation on the number of days to casting for a prosthesis, and the incidence of post-operative stump infection.
METHODS: Patients requiring trans-tibial amputation were randomised to one of 2 groups: In Group 1 (intervention) a rigid above-knee plaster dressing was applied at operation and patients were managed according to a standard protocol. Group 2 (control) had the individual surgeons' usual non-rigid dressing regime. Rehabilitation data were ex

The technique of combined scapular/parascapular reconstruction of the below-knee stump wound allows three-dimensional contouring of fasciocutaneous tissue into a conical shape. The flap can supply durable cover to the circumference of the stump with good functional results. This technique should be considered for reconstruction of extensive circumferential defects of the below-knee stump.

Two hundred seventy-five lower extremity amputations were performed over a 4-year interval for end-stage peripheral vascular disease. Fourteen patients (8.5%) of a total of 165 patients undergoing above-knee amputation (AKA) either suffered acute gangrenous ischemia of the AKA stump postoperatively, or were thought to be at high risk for same, and therefore underwent prophylactic inflow revascularization prior to or concomitant with AKA. The overall operative mortality rate was 28.5% in these 14 patients and was related either to inability to revascularize (two of three patients) or to the att

OBJECTIVE: To evaluate the efficacy of abobotulinumtoxinA in stump preparation for prosthetic fitting in patients with limb amputations.
MATERIAL AND METHODS: This prospective open-label study included 88 male patients over 18 years old with lower limb amputations due to mine-blast trauma and phantom limb pain syndrome (PLP) with pain intensity of at least 5 points on the visual analogue scale (VAS), absence of significant effect from pharmacological therapy, stump tenderness, and preserved active movements in the transected muscles. AbobotulinumtoxinA at doses of 500-800 U was used to reduce

BACKGROUND: Persistent residual limb pain (RLP) is a common and disabling complication following limb amputation. Although nerve injury and neuroma formation are well-established contributors, the potential association between stump infection and long-term pain outcomes has not been fully elucidated.
AIM: To evaluate the association between post-amputation stump infection and persistent residual limb pain, with particular emphasis on infection severity, microbiological profile, and antimicrobial resistance.
METHODS: This prospective observational cohort study included 430 adult patients who

The design and fitting of prosthetic sockets can significantly affect the acceptance of an artificial limb by persons with lower limb amputations. Clinical fitting is typically an iterative process, which requires patients' feedback and professional assessment. When feedback is unreliable due to the patient's physical or psychological conditions, quantitative measures can support decision-making. Specifically, monitoring the skin temperature of the residual limb can provide valuable information regarding unwanted mechanical stresses and reduced vascularization, which can lead to inflammation,

CASE: We describe the use of local antibiotics to salvage an infected below-knee amputation in a diabetic patient and prevent the need to convert to the above-knee level. After débridement, an antibiotic tibial "dowel" was fashioned and inserted into the intramedullary canal before closure. The wound was fully healed after four months, and the patient remained satisfied at 15 months.
CONCLUSION: This case report offers an adjunct to the conventional management of diabetic stump infections. The addition of an antibiotic spacer could offer an alternative to a higher level of amputation and impr

INTRODUCTION: The stump wound complications after above-knee amputation lead to other problems, such as prolonged rehabilitation, delayed prosthetic restoration, the increase in total treatment cost and high mortality rates.
OBJECTIVE: To evaluate the safety and outcomes of negative-pressure wound therapy (NPWT) using Vacuum-Assisted Closure (VAC®) therapy in patients with stump complication after above-knee amputation (AKA).
METHODS: From January 2011 to July 2014, AKA was performed in 137 patients at the University Cardiovascular Clinic. Nineteen (12.4%) of these patients (mean age 69.3 ±

A 63-year-old man dropped a metal chunk onto his left foot during his work and suffered a crush injury of the left forefoot. He underwent Chopart's amputation followed by stump coverage with sole skin at the orthopedic department on the same day. He was referred to our department for reconstruction because of poor vascularization and subsequent necrosis of tissue at the stump. After the necrotic tissue was debrided, exposure of the talus bone was noted. An artificial dermis was then applied to the stump wound, followed by local negative pressure wound therapy. After 3 weeks, the generation of

The reconstruction of defects around the knee often poses a challenge due to the limited availability of local soft tissues. Indeed, this same problem is encountered when attempting to revise a below-knee amputation stump. Moreover, due to a paucity of recipient vessels in those who have undergone previous amputation secondary to trauma, free-flap reconstruction is often challenging and not always successful. We report a case of a reverse anterolateral thigh (ALT) flap used to revise a long below-knee amputation stump. Previous reports in the literature attest to the versatility of the reverse

BACKGROUND: The single-leg squat test (SLST) is supported by evidence for reliability and validity across various visual rating methods, but the optimal number of ordinal categories for assessing movement quality remains unclear. The study aims to determine the most informative and reliable number of ordinal scoring categories for visually rating the SLST.
METHODS: A total of 58 subjects with lower extremity injuries participated. A single therapist rated the SLST with a 1-week interval to establish intra-rater reliability. Two therapists independently rated the SLST at the same time point to
