Maternal morbidity and mortality in the United States continues to be high. Understanding parturient complications and causes of death is critical to determine corrective actions. Analysis of closed malpractice claims evaluates patient care, identifies preventable morbidity and mortality, and offers recommendations for improvement. A review of obstetric anesthesia malpractice claims filed against nurse anesthetists (N = 21), extracted from the American Association of Nurse Anesthetists Foundation Closed Claims database, was completed. The malpractice claims included 18 maternal claims and 3 ne…

Anesthesia-associated nerve injury is a common cause of patient morbidity and litigation. To identify factors associated with perioperative nerve injuries and rationalize preventive strategies, 44 cases from the American Association of Nurse Anesthetists (AANA) Foundation Closed Malpractice Claims Database pertaining to nerve injuries in which nurse anesthetists provided care were analyzed. Emerging patterns and themes related to the development of injury were identified. The database is a collection of medical liability claims filed against CRNAs insured by the St Paul Fire and Marine Insuran…

The journey from nurse anesthesiology resident (NAR) to certified registered nurse anesthetist (CRNA) is a profound transformation, one that extends beyond skill acquisition to the evolution of professional identity. Using the metaphor of a caterpillar's metamorphosis into a butterfly, this article explores how academic rigor, clinical immersion, and mentorship shape resilience, adaptability, and confidence in future CRNAs. Grounded in growth mindset theory, it highlights how embracing challenges, seeking feedback, and reframing setbacks as opportunities for learning build both competence and

BACKGROUND: Although extubation is a high-risk phase associated with risk of severe complications for patients undergoing general anaesthesia, there is a lack of research about this phenomenon from the perspective of anaesthesiologists' experiences of the process of extubation in the anaesthesia setting.
AIM: To describe Swedish anaesthesiologists' experiences of the extubation process in the anaesthesia setting.
METHODS: A qualitative descriptive design study with individual semi-structured interviews was conducted in three hospitals in Sweden with a total of 17 anaesthesiologists. A qualit

Rubinstein-Taybi syndrome (RTS) is a rare genetic disorder that is associated with dysmorphism, moderate to severe intellectual disability and abnormalities involving multiple organ systems. For anesthetists, these patients pose unique challenges in the administration of anesthesia and airway management. We describe the anesthetic conduct of a pediatric patient with RTS coming for dental surgery as a day surgery case and will discuss the anesthetic considerations for such patients.

Peripheral nerve block has many advantages in surgical anesthesia with or without general anesthesia; postoperative analgesia, faster postoperative rehabilitation, and chronic pain management. However, serious adverse complications after peripheral nerve block can happen. Therefore, anesthetists should obtain full informed consent for possible complications, and require scrupulous attention to this procedure. This review focuses on complications of brachial plexus block because it is the most popular peripheral nerve block.

Closed chest injury with rupture of the trachea and main bronchi is a rare and extremely life-threatening pathology. In suspected tracheal injury, as well as in severe closed chest injury with the gas syndrome, diagnostic tracheobronchoscopy with possible intubation is indicated in order to isolate the airway lumen from the paratracheal space. Tracheal intubation through a fibrobronchoscope should be considered to be the first aid. Tracheostomy is not the operation of choice and it is indicated only when there is no available technology or experience in intubating the airways under endoscopic

The use of illegal drugs in Taiwan is on the rise. Drug addicts often have complex physical, psychological, and social problems. In addition, they often avoid disclosing their illicit drug use by deceit, concealment, or under-reporting. Building and maintaining relationships of trust with drug-addict patients has become a critical issue in achieving better care quality. In this case report, we report on an anesthesia care process for a heroin addict who was admitted for open reduction and internal fixation surgery for the femur and patella fractures after a car accident. Traditionally, the management of type B aortic dissection has been the domain of the vascular surgeons. Timing and type of intervention still generate debate. We sought to review our early experience with the treatment of this condition based on a hybrid approach following an aortic multi-disciplinary team meeting involving close cooperation between cardiac surgeons, vascular surgeons, interventional radiologists, vascular anesthetists, and cardiac anesthetists. Four patients (age 41-56 years; 3 males; 1 female) with type B aortic dissection underwent aortic arch surgery through a hybrid appro…

Accurate management of primary postpartum hemorrhage requires perfect knowledge of a recent protocol accessible at all times. Staff members and equipment need to be adjusted to the emergency situation. The first step is to establish a rapid diagnosis. This could be done with collecting bags, placed immediately after the birth. Once the diagnosis has been established, it has to be communicated to the whole team including obstetricians, midwives and anesthetists. A search for the cause of the hemorrhage is premordial. Abdominal uterine palpation confirms the diagnosis of uterine atony, the most …

Paroxysmal supraventricular tachycardia (PSVT) is a serious cardiac disturbance that can arise spontaneously or be initiated by noncardiac mechanisms. An occurrence of this dysrhythmia in a 55-year-old male undergoing monitored anesthesia care for excision of a subxiphoid subcutaneous mass is reported. The unusual cause of this patient's ectopy is discussed. The nature and treatment of PSVT are reviewed, as are the clinical considerations for anesthetists who encounter a comparable clinical problem.

Some anesthetists in Switzerland and elsewhere use a finger cot to protect the cuff of the endotracheal tube during nasotracheal intubation. In the presented report the finger cut was lost during the procedure and the patient presented 3 months later with a lateral neck mass. The finger cot was found within that mass at exploration. Apart from the other potential risks of this manoeuvre, this severe complication should incite caution against the practice described above.

Certified Registered Nurse Anesthetists (CRNAs) care for patients with opioid use disorder frequently. Goals are to support recovery, prevent relapse, and effectively and safely treat perioperative pain. During emergencies, care may be urgent to prevent patient harm, potentially interfering with helpful interventions. This article discusses care principles that CRNAs should follow to assure that the anesthetic care goals are achieved during emergent care of patients with opioid use disorder.
