ANGIOEDEMA AFTER ANGIOTENSIN-CONVERTING ENZYME INHIBITORS.

Angiotensin-converting enzyme inhibitors (ACEI) are one of the most frequently used drugs in the therapy of cardiovascular diseases. Beneficial therapeutic effects may be accompanied by different adverse reactions including angioedema. Recently published data suggest that ACEI-induced angioedema can be life-threatening without emergent intervention, for example due to the respiratory arrest. Monitoring this adverse Jreaction may be especially important for elderly patients who probably receive many different medicines and supplements and thus might be particularly at risk of adverse drug react…

Acute allergic angioedema of upper lip.

Mishaps can occur during dental procedures, some owing to inattention to detail and others are totally unpredictable. They usually include anaphylaxis or allergic reactions to materials used for restorative purposes or drugs such as local anesthetics. A patient reported to our department with moderate dental fluorosis, and the treatment was planned with indirect composite veneering. During the procedure while cementation acute allergic reaction occurred, the specific cause could not be identified after allergic testing. During the procedure while cementationacute allergic angioedema of upper l…

Hereditary angioedema with normal C1 inhibitor.

Until recently it was assumed that hereditary angioedema was a disease that results exclusively from a genetic deficiency of the C1 inhibitor. In 2000, families with hereditary angioedema, normal C1 inhibitor activity, and protein in plasma were described. Since then, numerous patients and families with that condition have been reported. Most of the patients were women. In many of the affected women, oral contraceptives, hormone replacement therapy containing estrogens, and pregnancies triggered the clinical symptoms. In some families mutations in the coagulation factor XII (Hageman factor) ge…

Managed care implications of hereditary angioedema.

Hereditary angioedema (HAE) is a genetic syndrome caused by a functional deficit in complement C1 inhibitor that results in recurrent episodes of nonpruritic swelling of the hands, feet, arms, legs, trunk, face, genitalia, bowels, and larynx beginning in childhood or adolescence and continuing throughout the patient's lifetime. Delays in the diagnosis of HAE average several years owing to the rarity of the disorder and the mimicking of clinical symptoms of those of a variety of other diseases. The lives of people with HAE have been transformed in recent years by the approval of several effecti…

Angioedema: patients experience and treatment relief.

Angioedema is defined as a swelling of the skin, mucosa and submucosa of the respiratory tract. It may also impair the intestinal epithelium and other mucous membranes. It can be potentially life-threatening if the upper respiratory tract is involved. In these cases emergency treatment is often required in particular if the pharynx and larynx are swollen. Beside the well-known etiologies of allergic angioedema, many forms of nonallergic angioedema are known and in the majority of these forms increased plasma and tissue concentrations of bradykinin play a critical role.

Hereditary angioedema: classification, pathogenesis, and diagnosis.

Hereditary angioedema (HAE) is a rare autosomal dominant genetic disorder associated with a deficiency in C1 inhibitor. More than 200 mutations in this gene, located on chromosome 11, have been identified. Although HAE is often inherited, 20-25% of cases are from new spontaneous mutations and they have no family history of swelling. Decreased C1 inhibitor activity leads to inappropriate activation of multiple pathways, including the complement and contact systems and the fibrinolysis and coagulation systems. Reduced C1 inhibitor activity results in increased activation of plasma kallikrein-kin…

Acquired angioedema in non-Hodgkin's lymphoma.

This paper describes a middle-aged patient who developed repeated episodes of swelling of the orofacial tissues after dental treatment. On investigation, C1 inhibitor, C1q, C2, and C4 levels were all markedly reduced, and a diagnosis of acquired C1 inhibitor deficiency was made. The patient had been diagnosed with non-Hodgkin's lymphoma (NHL) 2 years previously and had undergone a successful course of chemotherapy. The development of her episodes of angioedema prompted thorough reinvestigation and a recurrence of NHL was identified. Therefore, acquired C1 inhibitor deficiency heralded a recurr…

Solar angioedema: an uncommonly recognized condition?

Solar urticaria is a well defined although uncommon photosensitivity disorder, and is said to be the underlying cause of chronic urticaria in approximately 0.5% cases. In contrast, solar angioedema is seldom reported. We describe two patients with postulated solar angioedema, associated with clinical and/or phototest features of solar urticaria. Recognition of solar provocation of angioedema has important consequences for patient management.

[Phenylbutazone-induced sialadenitis fever simulating angioedema].

BACKGROUND: Drug-induced sialadenitis is uncommon and unrecognized. Drugs such as nitrofurantoïn, nifedipine and methimazole have been reported to induce sialadenitis. However, phenylbutazone and oxyphenbutazone are the most frequently implicated agents. We describe a case of phenylbutazone-induced parotitis and submaxillitis with cutaneous and hepatic involvement. CASE REPORT: A 51 year-old woman who had received phenylbutazone for the past 6 days was hospitalized for diagnosis of Quincke's oedema. Clinical examination in fact revealed bilateral parotitis and submaxillitis. The patient had c

Drug-induced, life-threatening angioedema revisited.

Few drug reactions are more life threatening than the sudden development of edema involving the mucosal and submucosal layers of the upper aerodigestive tract. Drug-induced angioedema is a recognized entity of angiotensin-converting enzyme (ACE) inhibitors, and despite reports in medical journals and drug insert warnings, captopril and enalapril continue to be widely prescribed. As these drugs are efficacious and usually well-tolerated in the treatment of mild forms of hypertension, their popularity is rising. From June 1, 1984 to August 1, 1991, 36 patients with angioedema secondary to ACE in…

Urticaria and angioedema. Common clinical problems.

The cause of urticaria and angioedema often is difficult to ascertain. In most cases the conditions are transient, but a chronic idiopathic form does occur and may be intractable. Acute urticaria and angioedema usually result from an IgE-mediated mechanism; success in treatment depends on recognition of the underlying factor. Chronic urticaria may ultimately necessitate use of corticosteroids. Hereditary angioedema is easily differentiated from idiopathic angioedema by the family history and absence of pruritus.

[Treatment of mast cell-mediated angioedema].

Mast cell-mediated angioedema represents the most common form of angioedema and may occur either in isolation or in association with urticaria. Clinically, the spectrum ranges from anaphylactic or anaphylactoid reactions, such as those occurring in allergic conditions, to chronic recurrent disease courses, for example, in the context of chronic spontaneous urticaria (CSU). Beyond classical IgE-mediated reactions observed in allergic diseases, it is now recognized that these conditions, as well as pseudoallergic reactions and chronic disorders such as CSU, are driven by more complex immunologic…

The International/Canadian hereditary angioedema guideline.

This update to the 2019 Canadian Hereditary Angioedema (HAE) Guideline broadens its focus to include the management of patients with HAE worldwide, building on its established international framework. It has been developed through a collaboration of Canadian and international HAE experts and patient groups, coordinated by the Canadian Hereditary Angioedema Network. The objective is to provide evidence-based recommendations, using the Grading of Recommendations Assessment, Development and Evaluation system, for the management of patients with HAE. These include recommendations for the treatment…

Mast cell mediators in hereditary angioedema.

BACKGROUND: Bradykinin-mediated hereditary angioedema due to C1 inhibitor deficiency (HAE-C1INH) is often hard to differentiate from mast cell (MC) mediated diseases. MCs can be activated by numerous stimuli, leading to the release of various mediators. Recently, crosstalk between MCs and the complement system has been reported. Clinical observations report a higher prevalence of MC-mediated diseases in the HAE-C1INH population than in the general population. OBJECTIVE: To investigate MC activation in HAE-C1INH. METHODS: Serum samples from patients during angioedema attack (HAE-A) and during s…

[Angioedema associated with hyaluronic acid injection].

INTRODUCTION: Angioedema is a form of localized swelling involving the deeper layers of the skin and mucous membranes, caused by fluid extravasation and lasting for several days. While aging of the skin is a natural process, various compounds such as hyaluronic acid, active amino acids, and peptides are used for their anti-aging properties. However, in some individuals, these substances can overstimulate the immune system, triggering hypersensitivity reactions. CASE REPORT: A 38-year-old woman with a medical history of controlled hypertension and allergy to quinolones. She underwent hyaluroni…

Angioedema without urticaria: Diagnosis and management.

Angioedema is nonpitting swelling that involves the deeper subcutaneous and submucosal layers of tissue. Angioedema can be classified as histaminergic, bradykinin mediated, or idiopathic in etiology. Bradykinin-mediated angioedema presents without urticaria, whereas histaminergic angioedema is usually associated with urticaria (i.e., chronic spontaneous urticaria and angioedema) but manifests with isolated angioedema in ∼20% of patients and clinically overlaps with idiopathic angioedema. Bradykinin-mediated angioedema most commonly occurs in hereditary angioedema (HAE) with or without C1-ester…

Acquired Angioedema Associated with Lymphoproliferative Disorders.

INTRODUCTION: Acquired angioedema due to C1 esterase inhibitor deficiency (C1INH-AAE) is most associated with lymphoproliferative disorders (LPDs), particularly low-grade B-cell subtypes. The condition remains under-recognized with long diagnostic delays due to various challenges including a lack of awareness of the condition. CASE PRESENTATION: We discuss 4 cases of C1INH-AAE associated with low-grade B-cell LPDs, including various diagnostic and management challenges. As our cases illustrate, constitutional symptoms or overt manifestations of LPD at diagnosis are often absent. Hence, a comp…
