Ankle and foot injuries in pediatric and adult athletes.

Foot and ankle injuries are among the most common in athletes.Differential diagnosis, imaging decisions, and treatment plan should be influenced by the age of the athlete. A thorough history and physical examination, with an understanding of the anatomy of the foot and ankle anatomy and the mechanism of injury, will give the best opportunity to make the correct diagnosis. For most athletes, the prognosis for returning to play after a foot or ankle injury is very good. The recipe for keeping our athletes healthy includes early intervention, undergoing proper rehabilitation, applying braces or o

Epidemiology of foot and ankle injuries in the workplace.

Injuries to the ankle, foot, and toes of injured workers are common. Although the workers' compensation system attempts to collect information about these injuries and report them in a meaningful way, the process is somewhat inconsistent, inaccurate, and provides information relevant to government agencies but not to physician organizations seeking to improve the prevention and treatment of work-related injuries. Several improvements in data collection could greatly increase the system's efficiency and usefulness. The first is to establish a national system of uniform data collection from each

Magnetic resonance assessment of inversion ankle injuries in children.

A cohort of ten children with X-ray negative inversion ankle injuries were studied using Magnetic Resonance Imaging. The images obtained were scored on six pathological criteria. Despite all ten having normal plain radiographs, 50 per cent were found to have direct evidence of growth plate injury. Seventy per cent of the cohort had periosteal elevation including all those with evidence of growth plate injury. This study supports previous work which suggested that children with 'X-ray' negative acute inversion ankle injuries who exhibited the clinical signs of injury to their lower fibular grow

Guidelines for selective radiological assessment of inversion ankle injuries.

A prospective study was performed to establish definitive guidelines for selective use of radiography in the assessment of inversion ankle injuries. Five hundred patients were included, representing 3.2% of the workload of the department during the study period. There were 379 soft tissue injuries, 56 malleolar fractures, 40 avulsion fractures, 21 fractures at the base of the fifth metatarsal, and four calcaneal fractures. Multiple logistic regression identified distal fibular tenderness, age, and ability to bear weight as the most important clinical variables in predicting important fractures

Lateral ankle ligament injuries. Inversion and anterior stress radiography.

Known anatomic lesions of the ankle ligaments were correlated with stress radiographic measurements on both anteroposterior and lateral projections, and the effect of foot position during stress radiography was determined. In autopsy specimens ankle ligaments were surgically divided to produce various single and combined lesions. The resulting instability was then assessed by stress radiography in two planes. Talar tilt and anterior displacement of the talus were related to the lesions present. Talar tilt did not normally exceed 4 degrees, and significant tilt occurred only when more than one

[A teenager with a traumatic ankle injury].

A 13-year-old girl presented to the traumatology outpatient clinic with a painful right ankle after an inversion trauma. She was unable to bear weight on the right foot. Additional radiographic imaging and CT-scan revealed a Tillaux fracture of the right ankle. As the fracture fragment was displaced 8 mm, open reduction and screw fixation was performed to restore the anatomy of the ankle joint.

Prognosis of ankle syndesmosis injury.

PURPOSE: Ankle syndesmosis injury has been associated with persistent pain and prolonged recovery; however, no predictors of prolonged recovery have been identified. The aims of this study were to establish prognosis for ankle syndesmosis injury compared with a lateral ankle sprain and to explore factors associated with prolonged recovery. METHODS: Participants (n = 63) age 21 ± 3.2 yr, with acute ankle ligament injuries (diagnosed through magnetic resonance imaging), were recruited from 10 sport clubs and sports medicine and physiotherapy clinics in two Australian cities. Follow-up was until

Injury to the lateral ligaments of the ankle.

Ankle sprains are common and often are treated inadequately. Unstable lateral ankle ligaments cause abnormal ankle motion and can lead to degenerative changes. Arthrography is the superior radiographic study in acutely injured ankles. Stress tests are useful if positive; however, because false-negative tests are common, general anesthesia should be considered. Chronic injuries may be evaluated well with stress testing. Conservative treatment (that is, mobilization) with aggressive rehabilitation is probably the best treatment for all lateral ligament injuries, unless certain mechanical stabili

Extremity CT and ultrasound in the assessment of ankle injuries: occult fractures and ligament injuries.

OBJECTIVE: To assess the rates of fractures and ligament injuries in patients with an acute ankle injury and a normal radiographic examination, and to consider the most appropriate examination protocol. METHODS: Patients with an acute ankle injury who presented to the John Radcliffe Hospital Emergency Department with a normal radiographic examination were eligible for the study. They were invited to receive a cone beam CT and ultrasound examination at a local radiology department within 5 days of their ankle injury. RESULTS: Of the 100 patients recruited to the study, 19 patients were found

Posterior Ankle Structure Injury During Total Ankle Replacement.

Total ankle replacement studies have focused on reporting complications that are directly observed clinically or radiographically, including wound problems, technical errors, implant loosening, subsidence, infection, bone fractures, and heterotopic ossification. However, patients can still experience unresolved pain even when these problems have been ruled out. We initiated a study to more clearly define the relative risk of injury to the anatomic structures in the posterior ankle during total ankle replacement using a third-generation implant system. Ten fresh-frozen adult cadaveric below-the

The effectiveness of the parachutist ankle brace in reducing ankle injuries in an airborne ranger battalion.

The purpose of this study was to determine if the parachutist ankle brace (PAB) decreases the number and severity of ankle injuries in an airborne Ranger battalion. A retrospective study was performed covering a 38-month period. A computer database was used to track all jump injuries with a diagnosis of ankle pain, sprain, or fracture. The frequency was calculated for ankle injuries per 1,000 jumps and the average length of medically restricted duty per ankle injury. A total of 13,782 static line parachute jumps were conducted during the study period. Without the PAB, 35 ankle injuries were se

Isolated syndesmosis ankle injury.

Isolated syndesmosis injuries often go unrecognized and are diagnosed as lateral ankle sprains; however, they are more disabling than lateral ankle sprains. The reported incidence of isolated syndesmosis injuries in acute ankle sprains ranges between 1% and 16%. When ankle disability lasts for more than 2 months after an ankle sprain, the incidence increases to 23.6%. Diagnostic workup may include stress radiographs, magnetic resonance imaging, or diagnostic arthroscopy. A simple stress test radiograph may reveal an unstable grade III syndesmosis sprain that may go unrecognized on plain antero

[Injury of the ankle joint ligaments].

The diagnosis of lateral collateral ankle ligament trauma is based on patient history, clinical examination and clinical stress tests. If the clinical stress test is positive, stress radiography can be performed. There is, however, no consensus about the usefulness of stress radiography in acute ankle sprain, and in particular about the cut-off talar tilt angle beyond which a two-ligament rupture would be certain, ranging from 5 degrees to 30 degrees. Today, magnetic resonance imaging (MRI) is not used in this area, although it does allow controlled positioning of the foot and defined section

Neurocognitive Deficits Related to Ligamentous Ankle Injuries: A Systematic Review.

BACKGROUND: Lateral ankle sprains (LAS) are among the most common sports injuries, with up to 40% of individuals developing chronic ankle instability (CAI). While altering neurocognitive demands can affect lower limb biomechanics, the underlying mechanisms in CAI patients remain unclear. This systematic review aimed to summarise evidence on the neurocognitive deficits linked with ligamentous ankle injuries and CAI. METHODS: Following PRISMA guidelines (PROSPERO: CRD42023406395), a comprehensive search of five databases (up to September 24, 2024) identified studies examining neurocognitive per

Arthroscopic management of ankle ligament injuries: a new gold standard?

Ankle ligament injuries are among the most common lesions encountered in emergency medicine and in chronic contexts. The present study aimed to assess the role of arthroscopy, addressing 5 questions. (1) What are the indications for surgery in ankle ligament injury? - In the acute phase, conservative treatment is the rule, but surgery may be considered for lateral lesions, in first injury in athletes with high functional demand, and in case of associated proximal lesions in the anterior talofibular ligament and calcaneofibular ligament. Surgery is also indicated for unstable lesions in the dis
