PURPOSE: We investigated the relationship between the level of obstruction of the upper urinary tract and the risk and onset of urinary tract infection in infants with severe obstructive hydronephrosis to determine the need for antibiotic prophylaxis.
MATERIALS AND METHODS: A total of 105 patients were prenatally diagnosed with severe hydronephrosis (Society for Fetal Urology grade III or IV) due to upper urinary tract obstruction between 1994 and 2004. Of these patients 75 had ureteropelvic junction obstruction and 30 had lower ureteral obstruction. We retrospectively evaluated the clinical …

Urethroplasty is a highly effective surgical treatment for urethral stricture disease. While overall complication rates are low, surgical site infections and urinary tract infections (UTIs) represent the most common complications. Due to the prolonged need for catheterization following reconstruction, many urologists place patients on extended antibiotic prophylaxis postoperatively. However, antibiotic stewardship is important given the risks of antibiotic overuse including opportunistic infections and the emergence of multidrug-resistant organisms. There are currently no established guideline…

One of the main goals of treatment of chronic obstructive pulmonary disease (COPD) is the prevention of exacerbations. Bronchodilators and anti-inflammatories are the first line therapy for treatment of COPD; however, these drugs are not effective in suppressing all infective exacerbations. In fact, the use of inhaled corticosteroids in patients with COPD and chronic bronchial infection may even increase the bacterial load in the airways and increase the risk of pneumonia. In this context, the use of long-term or intermittent antibiotic treatment has shown to prevent COPD exacerbations and hos

The prophylactic use of antibiotics can only be justified when clinical benefits on relevant patient outcomes, such as morbidity or mortality, cost-effectiveness, and absence of immediate emergence of antibiotic resistance have been unequivocally demonstrated. In some intensive care unit (ICU) patients, antibiotic prophylaxis is used as part of selective digestive tract decontamination (SDD) and selective oropharyngeal decontamination (SOD). Recent trials in ICUs with low levels of antibiotic resistance strongly suggest that both regimens reduce the incidence of ICU-acquired infections and imp…

BACKGROUND: Antibiotic prophylaxis in nail surgery is not clearly established, and there is scant scientific evidence regarding the need for its use in preventing surgical site infection, hematogenous total joint infection, and infective endocarditis.
OBJECTIVES: To propose an algorithm based on the evidence for the management of antibiotic prophylaxis in onychocryptosis surgery.
METHODS: A literature review was performed in Medline, Pubmed, Cochrane database and Scopus and recent prospective studies were examined. The most-current authoritative guidelines together with new classification sy…

The utility of antibiotic prophylaxis for colorectal surgery has been assessed and largely confirmed worldwide. There is homogeneous consensus that the antibiotic chosen for prophylaxis must cover both aerobic and anaerobic bacteria, that the toxicity should be minimal and it should be cost effective. Recent studies have questioned the role of preoperative bowel cleansing, which is considered by many surgeons as a widely established practice prior to elective colorectal procedures. New clinical trials are needed to resolve these key questions such as the efficacy of bowel preparation and how t…

BACKGROUND: Postoperative antibiotic prophylaxis after expander/implant-based breast reconstruction is an important consideration, but existing literature is largely focused on subpectoral reconstruction. With the prevailing paradigm shift towards prepectoral reconstruction, there is concern for greater infection risk given less vascularized tissue coverage of the expander/implant when compared to partial or total submuscular approaches, along with adjunct use of acellular dermal matrix. We investigate the optimal duration of postoperative antibiotic prophylaxis specifically for prepectoral ex…

Intrawound vancomycin powder is the most widely used intraoperative prophylactic intervention against surgical site infections (SSIs). However, applying raw drug powder to irregular or small surgical cavities can result in uneven wound coverage, drug precipitation, and airborne drug particles. To address these issues, we developed a surfactant-based aqueous foam specifically for spine surgery, where deep incisions and instrumentation heighten SSI risks. A rat model of spinal implant-associated Staphylococcus aureus infection was developed to assess the antimicrobial efficacy of intrawound vanc

Spontaneous bacterial peritonitis (SBP) is associated with risk of acute on chronic liver failure (ACLF). Current guidelines recommend primary and secondary antibiotic prophylaxis for patients with cirrhosis and ascites who are at risk of a first episode and to prevent recurrence, respectively. Factors associated with prophylaxis failure leading to SBP, ACLF, and increased mortality are not well established. Gram-positive and multidrug-resistant organisms have become more frequently associated with SBP, particularly in the setting of ACLF. Efforts to understand how long-term antibiotic therapy…

There are no guidelines for antibiotic prophylaxis for ureteral stent removal after kidney transplantation. We reviewed the charts of 277 adult kidney transplant recipients with ureteral stents transplanted at our center between September 2014 and December 2015 and investigated whether antibiotic prophylaxis for stent removal was associated with reduced incidence of urinary tract infections (UTI). We defined UTI as a urine culture ≥104 CFU/mL of bacterial isolates irrespective of symptoms. Primary outcome was the incidence of UTI within four weeks of stent removal. Among the 277 recipients, 1…

BACKGROUND: Orthognathic surgery (OS) is a term that refers to many elective surgical techniques to correct facial deformity; the associated malocclusion and functional disorders related to the stomatognathic system. Whilst such surgery is classed as "clean-contaminated", the usefulness of and the most appropriate regimen for antibiotic prophylaxis in these patients are still debated.
OBJECTIVES: To assess the effects of antibiotic prophylaxis for preventing surgical site infection (SSI) in people undergoing orthognathic surgery.
SEARCH METHODS: In June 2014, we searched the Cochrane Wounds …

OBJECTIVE: To determine the potential economic impact from the practice of antibiotic prophylaxis for dental procedures.
STUDY DESIGN: We estimated the prevalence of patients in the United States with 15 medical conditions and devices. We multiplied the prevalence for each patient population by the percentage of specialists recommending prophylaxis, then by the estimated number of dental office visits per year, and then by an average pharmacy cost to arrive at a total estimated range of annual cost for this practice.
RESULTS: The 15 medical conditions and devices included in the present stud…
