Strabismus was present in 40% of patients with cataracts/dislocated lenses at the time of initial diagnosis and continued to present an obstacle to complete rehabilitation with 86% of the congenital patients and 61% of the acquired patients found to have strabismus at their latest post therapy visits. The type of horizontal deviation found at the initial examinations was distributed equally between esotropia and exotropia in the unilateral congenital cataracts patients. The first 4 symptoms of binocular confusion concern primarily daytime vision and the fifth symptom concerns nighttime vision. The correction of unilateral and bilateral aphakia with contact lenses has given good results. The contact lens of choice today is either the hard corneal lens or the soft hydrophilic lens. High-water-content lenses have excellent oxygen permeability which permits continuous wear. Experience with the 'Sauflon 85' lens is described.

Following extracapsular senile cataract extraction, visual thresholds were measured in 15 aphakic eyes corrected with glasses and compared with an appropriately matched group of 23 eyes with poly (methyl methacrylate) posterior chamber lenses. All subjects were six months to three years postoperative and had 20/30 or better visual acuity. The paramacular dark adapted threshold, the foveal luminance threshold, and the glare effect on both thresholds were not significantly different in the two groups despite the optical differences between aphakic and pseudophakic correction.
