Appendiceal mucinous neoplasms: Optimizing treatment strategies based on clinical, histological, and molecular features.

Appendiceal mucinous neoplasms (AMNs) are rare tumors originating from mucin-producing epithelial cells of the appendix. They can exhibit both benign and malignant behavior. They are often incidentally discovered during appendectomy. Clinical presentation ranges from asymptomatic to mimicking acute appendicitis. Histologically, noninvasive AMNs are classified as low-grade AMNs (LAMNs) or high-grade AMNs (HAMNs), whereas invasive tumors are categorized as mucinous adenocarcinomas. Although LAMNs and HAMNs are generally nonmalignant, rupture can lead to pseudomyxoma peritonei (PMP). Surgical res

Follow-up of 35 appendiceal orifice neoplasms resected by endoscopic full-thickness resection.

BACKGROUND AND AIMS: Endoscopic full-thickness resection (EFTR) is an established, safe technique for the resection of appendiceal orifice (AO) neoplasms. Post-EFTR appendicitis is a recognized adverse event. There are no systematic reviews, and there is a paucity of literature with assessed outcomes, particularly with respect to delayed appendicitis, mucocele, or fistula formation. We aimed to evaluate the efficacy of EFTR for AO lesions. METHODS: Consecutive AO lesions referred for consideration of EFTR were prospectively studied. Multiple data points were recorded including technical succe

Extent of Disease on Visceral Peritoneal Surfaces of Mucinous Appendiceal Neoplasms Controls Survival.

OBJECTIVE: To determine causes of treatment failure of low-grade appendiceal mucinous neoplasms (LAMN). BACKGROUND: For 3 decades, LAMN have been treated by cytoreductive surgery (CRS) and perioperative intraperitoneal chemotherapy. This combined treatment has resulted in a large change in the survival of these patients. METHODS: A retrospective review of a prospectively maintained database was performed. A restricted cohort of patients with only LAMN histology and complete CRS were included in the statistical analysis. RESULTS: Four hundred and fifty patients were available with a median f

Outcomes of Low-Grade Appendiceal Mucinous Neoplasms with Remote Acellular Mucinous Peritoneal Deposits.

BACKGROUND: Occasionally, low-grade appendiceal mucinous neoplasms (LAMN) present with mucinous peritoneal deposits (MPD) localized to periappendiceal tissue or diffused throughout the peritoneum. OBJECTIVE: This study was aimed at evaluating the relevance of mucin cellularity for predicting outcomes of LAMN with remote MPD. METHODS: The records of patients with LAMN and remote MPD who underwent initial assessment at a comprehensive cancer center from 1990 to 2015 were reviewed, and diagnostic procedures, treatments, and outcomes were analyzed. RESULTS: Of 48 patients included in the analys

Enterocutaneous fistula resulting from the drainage of a contained appendiceal perforation secondary to a partially obstructing low-grade appendiceal mucinous neoplasm.

We report a case of an enterocutaneous fistula resulting from drainage of a contained appendiceal perforation secondary to an obstructing appendiceal mucinous neoplasm. A 66-year-old otherwise healthy female patient proceeded to laparoscopic appendicectomy with resection of the fistula tract. Histopathology demonstrated a mid-appendiceal mucinous neoplasm, with a benign enterocutaneous fistula tract communicating with the distal, non-malignant portion of the appendix. We provide an overview of the literature and highlight the need to consider appendiceal mucinous neoplasm as a differential in

Mucosal prolapse syndrome-like inflammatory polyp protruding from the appendiceal orifice masquerading as an appendiceal neoplasm: A rare case report.

We describe a case of a mucosal prolapse syndrome (MPS)-like inflammatory polyp protruding from the appendiceal orifice, mimicking an appendiceal neoplasm. A 48-year-old man presented with lower abdominal pain and elevated white blood cell count and C-reactive protein level. Computed tomography showed a swollen appendix with multiple small saccular lesions and periappendiceal fat stranding and small saccular lesions in the ascending colon and cecum. Ultrasonography showed a swollen appendix 25 mm in diameter with multiple saccular structures and a periappendiceal high-echoic area, suggesting a

A scoping review of postoperative surveillance strategies for localized, low-grade appendiceal mucinous neoplasms.

INTRODUCTION: There is a paucity of data to support surveillance protocols for localized, low-grade appendiceal mucinous neoplasms (LAMNs) after resection. METHODS: A search strategy was developed to identify postoperative surveillance strategies for LAMNs, in the context of rates of recurrence and disease-free survival, and applied to four literature databases. Resultant citations were subject to screening in duplicate, in three stages: title, abstract, and full-text. Data was extracted from the final set of included articles and subject to descriptive statistics. RESULTS: A total of 16 art

Mucinous appendiceal neoplasms and pseudomyxoma peritonei: Morphological and clinical findings, differential diagnosis, and prognosis.

BACKGROUND: Low-grade appendiceal mucinous neoplasms (LAMN) of the appendix have bland cytological features and well-differentiated morphology. Despite this, they may cause a pseudomyxoma peritonei (PMP) disease characterized by mucinous deposits in the peritoneal cavity and may exhibit malignant behavior. AIMS AND OBJECTIVES: In this study, we evaluated the clinical course and histopathological findings of LAMN. The rate of progression to PMP, factors affecting its development, and the clinical course of cases with PMP were investigated. MATERIALS AND METHODS: Twelve thousand and forty-seve

Laparoscopic cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for perforated low-grade appendiceal mucinous neoplasms.

INTRODUCTION: Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) is an established treatment for pseudomyxoma peritonei (PMP) from perforated low-grade appendiceal mucinous neoplasms (LAMN II). In a selected group of LAMN II patients without established PMP, CRS/HIPEC can be performed laparoscopically (L-CRS/HIPEC); however the short-term benefits and safety of this approach have yet to be determined. This study aims to determine the short-term outcomes from a series of L-CRS/HIPEC LAMN II patients compared to those who have undergone a similar open operation (O-C

Purtscher-Like Retinopathy Secondary to an Appendiceal Neuroendocrine Neoplasm Complicated by a Periappendiceal Abscess.

Purtscher-like retinopathy (PLR) is a rare retinal microvasculopathy that is induced by non-traumatic systemic illnesses. We describe the case of a woman in her 70s who was diagnosed with PLR, ultimately linked to an appendiceal neuroendocrine neoplasm (ANEN) complicated by a periappendiceal abscess. The patient presented with sudden, unilateral, painless visual impairment. Initial examination revealed Purtscher flecken (PF) and cotton-wool spots (CWS) in the right eye, with corresponding hyperreflectivity and swelling of the inner retinal layers on optical coherence tomography (OCT). Systemic
