The clinical examination for motor apraxia.

The signs and symptoms of motor apraxia are described both for ideomotor and ideational apraxia. Typical errors are described. For ideomotor apraxia a list of tasks is given. The many conceptual and psychometric problems that still await an answer are briefly discussed.

Apraxia and the supplementary motor area.

Anatomical, electrophysiological, and cerebral blood flow studies suggest that the supplementary motor area (SMA) may be important for programming certain skilled motor acts of the limbs. However, to our knowledge, abnormalities of complex distal motor behavior following SMA lesions have not been reported. We have studied two patients with left mesial hemisphere infarctions that included the SMA. These patients had bilateral ideomotor apraxia for transitive limb movements without buccofacial apraxia. The observations suggest that the types of skilled motor acts programmed by the left SMA are l

Congenital ocular motor apraxia.

Nine patients with congenital ocular motor apraxia (COMA) are presented and the natural history of this condition is considered. Two presented in early infancy, before the onset of the head thrust, and the means of establishing the diagnosis at this age are discussed. All exhibited motor delay in infancy which lessened, but did not completely resolve, with time. Conceptual delay, particularly with speech, affected all in early childhood. Three had agenesis of the corpus callosum and 2 cerebellar abnormalities. The autopsy of one infant showed cerebellar cortical dysplasia. The pathogenesis of

Axial apraxia in Parkinson's disease.

Attention is drawn to certain disorders of posture and movement such as kneeling, turning around in the recumbent position, arising and walking which form a separate group of motor disabilities in Parkinson patients. Levodopa therapy is far less effective for these axial motor abnormalities than for hypokinesia, tremor, rigidity and manual dexterity. Inappropriate function of the axial musculature leads to debilitating situations. It is argued that the disordered axial movements are not the result of an akinetic mechanism but share the criteria applied to apraxic phenomena and the term axial a

The two types of motor apraxia.

Ideomotor and ideational apraxia are higher motor disturbances at two different levels. In ideomotor apraxia the execution of simple and complex, meaningful and meaningless movements with the orofacial musculature and/or with the limbs, is impaired in a very distinct way, as has been described in detail. The parapraxic distortions of movements occur, when the movement is required out of context. The recognition of movements and gestures is intact. In ideational apraxia, in contrast, there is a disturbance in the conceptual organization of complex actions requiring the use of various objects. T

Apraxia and the neurophysiology of motor control.

Liepmann's description of apraxia still serves as a point of reference for neurological thinking about higher motor function. The extent to which this model is accessible to interpretation in the light of current neurophysiological knowledge has been examined. Planning and programming processes have been distinguished, and the separate roles of striopallidal and cerebellar loops in the generation of those processes have been considered in relation to the contribution of parietal and frontal association areas. It is emphasized that a new taxonomy of action is imperative in order that neurophysi

Gait apraxia in communicating hydrocephalus.

Apraxia of gait in patients with communicating hydrocephalus appears in the context of a generalised motor disorder that includes defective righting reflexes, generalised increased tone to passive movements, grasp reflexes, difficulty with serial movements of the hands and defective smooth pursuit eye movements. The inability to walk does not appear to be due to a motor disorder but to release of proprioceptive supporting reactions. This mechanism is triggered by proprioceptive stimuli.

Developmental apraxia: diagnostic considerations.

This study compares the performance of developmentally apraxic children with that of children who have functional articulation disorders on a battery of diagnostic tests. Based on preliminary data, tests which assess auditory attention, auditory discrimination, auditory memory, oral-tactile feedback, and isolated and sequenced volitional oral movements appear to be useful in a differential diagnosis of articulatory defective children and, thus, warrant further study. Implications of these findings are discussed.
