We have restudied the genetic and clinical characteristics of a large Arab kindred previously reported in 1970 by Lebenthal et al. [Pediatrics 46:891-899]. At total of 40 affected individuals was identified; all, except one, were products of 22 different consanguineous matings between the parents. The syndrome, which is present at birth, is expressed mainly by flexion contractures at the knees and elbows, with muscle hypotrophy/weakness around the involved joints. The case records of all neonates admitted to the neonatal unit of Al Wasl Hospital (Dubai) in a period of 60 months (May 1987-April 1992) were analysed. One-hundred-and-six neonates had confirmed sepsis. Between September 1981 and March 1987, 92 episodes of bacterial meningitis in 90 children were treated in three major hospitals in Kuwait. The diagnosis was bacteriologically confirmed in 80 (87%). Haemophilus influenzae was the most common aetiological organism and accounted for 42 episodes (45.6%) followed by Streptococcus pneumoniae in 21 (22.8%) and Neisseria meningitidis in only three (3.3%). This report describes an Arab family in which the parents are first cousins and with 5 siblings having multiple pterygium syndrome. The last pregnancy which produced an affected sib was prenatally diagnosed by ultrasonography. Intrafamilial variability of clinical features is discussed. The report stresses the importance of the differentiation between various genetic entities with multiple pterygium.

Gum arabic (GA) is a water-soluble polysaccharide (molecular weight approximately 850 000) containing rhamnose, arabinose, glucuronic acid and galactose. The metabolism of GA has been studied in the rat. Adult male Wistar rats were given GA incorporated into either an Oxoid breeders (OB) diet or an elemental (Elem) diet. Intestinal contents were examined for precipitable GA using acidified ethanol. GA was found from stomach to small intestine but not in the caecum, colon or rectum. Hypertensive disorders of pregnancy (HDP), including chronic hypertension, gestational hypertension, pre-eclampsia, eclampsia, and HELLP syndrome, are major contributors to adverse maternal and neonatal outcomes. Given the warm climate, excessive water-related recreational activities, and large Muslim population practicing ablution, it is surprising that primary amoebic meningoencephalitis is not reported in the United Arab Emirates.
