BACKGROUND: Collection of cerebrospinal fluid (CSF) in the subdural compartment is a major cause of postoperative morbidity, especially for … an additional barrier to prevent CSF leak. It also prevents morbidity associated with adhesions and arachnoiditis. Proper closure of arachnoid makes durotomy during repeat surgery much easier and avoids injury to the underlying pia. A brief review of related literature shows the benefits of closing the arachnoid before dural closure and the different techniques …

Spinal adhesive arachnoiditis (SAA) with cyst formation secondary to infectious meningitis is a rare clinical entity. These cysts can compress the spinal cord and cause neurologic decline. We present a case of a patient who underwent resection for an intradural schwannoma which was complicated post-operatively by bacterial meningitis and development of several recurrent thoracic arachnoid cysts. After two separate thoracic decompressions with lysis of intradural adhesions, a permanent ventriculoperitoneal shunt (VPS) was eventually placed with complete recovery of his symptoms. Our review of t…

Spinal adhesive arachnoiditis is a rare pathology involving pia mater of the spinal cord and nerve roots. It can potentially lead to disability-many patients end up wheelchair-bound due to subsequent paraparesis. It is an infrequent but possible cause of lower extremities weakness in patients with a history of spinal surgery, epidural anaesthesia, myelography or spinal tumors. Three patients, one male and two females, admitted to our unit due to paraparesis presented at least one of the above mentioned risk factors. Each of them had a severe course of illness-progressive paresis of lower extre…

Context: A patient followed in the outpatient spinal cord injury support clinic at a VA Medical Center with a prior remote history of a guns … unexpected, albeit transient improvement in myelopathic symptoms who are known or suspected to have arachnoiditis should be evaluated for surgically remediable lesions. Remediation of these lesions can potentially improve long term outcome.

OBJECTIVE: Chronic adhesive arachnoiditis (CAA) is rare and has potentially devastating clinical consequences. The objective of this article is to review the clinical features of CAA and describe its appearance on imaging, to increase radiologists' awareness of this challenging diagnosis.
MATERIALS AND METHODS: Twenty-nine cases of advanced CAA seen at our institution over 18 years (1995-2013) were retrospectively reviewed. Chart review was performed, with attention given to data on clinical presentation, suspected cause, and interventions performed. All patients underwent MRI, and seven pati…

OBJECTIVE: Histoplasmosis of the central nervous system (CNS) is seen in 10% to 20% of patients with disseminated histoplasmosis and/or in association with immunocompromised patients. Meningitis, arachnoiditis, and hydrocephalus are the most common clinical manifestations of CNS histoplasmosis. Patients with CNS histoplasmosis present similarly to other infectious etiologies, and confirmatory diagnosis is important in the management of these patients. However, diagnosis of CNS histoplasmosis can be difficult, and sometimes performing a parenchymal biopsy is necessary to confirm the diagnosis.

The term arachnoiditis describes the inflammation of the meninges and subarachnoid spaces. Lumbar arachnoiditis is characterized by obliterated nerve root sleeves and the adherence of nerve roots to each other in the proximity of the cauda equina, and may be secondary to infectious diseases or tumors, iatrogenic (subsequent to spinal surgery) or idiopathic. It is not very clearly defined epidemiologically or clinically, and various theories regarding its pathophysiology have been proposed; furthermore, its treatment is difficult because there is a lack of evidence-based diagnostic and therapeu…

We conducted a retrospective study of 32 patients treated for syringomyelia associated with non-traumatic arachnoid scarring. We selected the cases with documented history of arachnoiditis with pre and post-operative clinical evaluation of the neurological status and anatomical study on MRI with a minimal follow-up of one year. Extensive arachnoid scarring (Group I) was noted in 18 cases, after spinal meningitis in 15 cases (tuberculosis in 9 cases, listeria in 3 cases, pyogenic meningitis in 3 cases), subarachnoid hemorrhage in 3 cases. Focal arachnoid scarring (Group II) occurred in 10 cases…

Neurologic and psychiatric examination of 32 patients with infectious cerebral arachnoiditis was performed. Psychopathic-like syndrome and intellectual deficiency were diagnosed in those patients and the correlation between high intracranial pressure and severity of mental disorders was established. Characteristics of forensic psychiatric examination of this group of patients are presented.

The authors have been treating with a synthetic D penicillamine 12 patients affected with pains in the lumbar region or lumbosciaticas imputed to duroarachnoiditis in connection with surgical procedure or/and radiculosacography. The diagnosis is based on clinical arguments (3 cases), clinical and radiological arguments (radiculosacography : 3, phlebography : 3) and on surgical arguments (3 cases). The findings are good 5 times and incomplete twice. There are 4 failures and 3 interruption of treatment out of 6 intolerances. The effect of penicillamine upon collagen could perhaps be the explanat…

While recovering from tuberculous meningitis, a 5-year-old boy developed a profound visual deficit because of optochiasmatic arachnoiditis. Following microneurosurgical decompression of the optic nerves and chiasm, prompt visual recovery occurred.

A patient studied preoperatively by a neuro-otological test battery showed signs of an extensive cerebellopontine angle tumour. During the operation dense fibrotic tissue was found extending from the choroid plexus to the right acoustic nerve, which was freed and thus decompressed. Postoperatively, low frequency hearing improved markedly in the right ear. In contrast to the well-known documented cases of successful decompression of the optic nerve it has not been possible up to now to achieve a successful outcome in similar procedures on the acoustic nerve. In the described case the fibrotic l

Arachnoid webs are rare intradural lesions which disrupt cerebrospinal fluid flow and may cause spinal cord compression. This is a case report of a previously healthy 19-year-old woman who presented with acute back pain and rapidly progressive tetraparesis. MRI revealed cervical spinal cord ischaemia and an arachnoid web at the thoracic level. Surgical removal led to partial neurological recovery. Early recognition is essential, as timely surgery may prevent irreversible injury.
