Chronic type A dissection in a pulmonary autograft.

A 37-year-old patient presented with severe aortic valve insufficiency due to massive dilatation of the neo-aortic root (77 mm diameter) 14 years after a Ross procedure. Intraoperatively, the dilatation appeared to be caused by a localized chronic dissection of the pulmonary autograft. Surgery consisted of a modified Bentall procedure with a mechanical composite valve, with an uncomplicated postoperative course.

Autograft containment in posterolateral spine fusion.

BACKGROUND CONTEXT: Pseudoarthrosis rates in lumbar intertransverse fusion remain high. Compression and displacement of the developing fusio … rthrodesis at L3-L4. The control group (n=7) received autograft alone, and the study group received autografts placed in open meshed hemicylinders fashioned from LactoSorb sheets (LactoSorb; Biomet Orthopedics Inc., Warsaw, IN). Spines were harvested at 6 weeks and imaged. Radiographs and computed tomography (CT) images were used to calculate the rate, area, …

The mitral pulmonary autograft: assessment at midterm.

BACKGROUND: There is a dire need, especially in emergent societies, for a mitral substitute that does not require anticoagulation, and is not affected by early degeneration. METHODS: Between 1997 and 2003, 80 patients had successful mitral valve replacement with a pulmonary autograft. Fifty-five patients were female, and the mean age was 39.3 years. Seventy-eight patients had rheumatic mitral disease and 2 congenital. The autograft was placed inside a rigid Dacron tubing for support, and the right ventricular outflow was reconstructed with a xenograft or a homograft. Recently we have used mic

Is there a place for pediatric valvotomy in the autograft era?

OBJECTIVE: Valvotomy and the autograft procedure are the most common surgical treatment options for children with valvular aortic stenosis. We evaluated the results of these surgical procedures in our institution. METHODS: Retrospective analysis was done of all patients presenting with aortic stenosis and operated upon before the age of 18. In 11 patients a valvotomy was performed and in 36 an autograft procedure. RESULTS: There was no hospital mortality. Mean follow-up in the valvotomy group was 4.8 years (SD 3.3), in the autograft group 4.5 years (SD 3.3). During follow-up one patient died

Current status of limbal conjunctival autograft.

The past decade has witnessed several major advances in the field of ocular surface reconstruction. It is now established that the limbal an … provided a surgical tool to use in tandem with limbal transplantation. Questions such as why limbal autografts fail, the long-term results of LAT, and whether limbal inclusion is mandatory during conjunctival transplantation after pterygium excision have not been answered fully. Studies on long-term outcomes with limbal allograft transplantation and those re …

Pulmonary autograft reoperation: incidence and management.

BACKGROUND: Pulmonary autograft replacement of the aortic valve is accepted in the young, those with an active life style, and those who are not candidates for anticoagulation. However, concern remains about autograft or homograft valve failure. METHODS: One hundred ninety-five operative survivors of the Ross operation (August 1986 through December 1995) were reviewed for operative pathology and factors associated with reoperation or valve dysfunction. RESULTS: Actuarial freedom from reoperation (autograft or homograft) is 89% +/- 3% at 5 years, 92% +/- 3% for the autograft alone. Early auto

Conjunctival autografting in the surgical management of pterygium.

In a retrospective survey of 117 operations for primary or recurrent pterygium, conjunctival autografting was compared with both excision without conjunctival closure ('bare sclera excision') and excision with complete conjunctival closure. The probability of corneal recurrence at 36 months after surgery was determined by survival curve analysis. In previously unoperated cases conjunctival autografting (n = 15) resulted in a 14% probability of recurrence, compared with 70% for bare sclera excision (n = 50) and 69% for excision with complete conjunctival closure (n = 20). In previously operated

[Pulmonary valve autograft for aortic valvulopathy].

Twelve patients were operated on between February 1992 and June 1993 because aortic valve disease with pulmonary autograft replacement of the aortic valve and reconstruction of the right ventricular outflow tract with a valved tube of autogenous pericardium and bovine prosthetic pericardium valve made at the Instituto Nacional de Cardiología Ignacio Chávez. Aortic and pulmonary annular diameters were taken preoperative in all patients by transthoracic echocardiography. During the surgical procedure, transthoracic echocardiography was done in order to assess valvular function of the pulmonary a

The pulmonary autograft--a permanent aortic valve.

Between 1969 and 1991, 339 patients had an aortic valve replacement with their own living pulmonary valve at the National Heart Hospital, Guy's Hospital and the Harley Street Clinic, London. The longest follow-up is 24 years and cumulative follow-up is 3774 patient-years. No form of anticoagulation was used and there were no emboli. There were 25 hospital deaths (7.4%) but only 1 death since 1976. Late deaths occurred in 38 patients mainly from technical mal-insertion. Bacterial endocarditis occurred in 11 patients. Thirty-eight patients were re-operated upon and account for 15 of the late dea

Aortic root replacement with pulmonary autograft.

Transfer of the pulmonary valve to the aortic position has been used successfully by Ross for more than 20 years and in our own institution since August 1986. To expand the use of this concept to patients with root disease and/or narrow annulus, we employed the main pulmonary artery with its valve as a conduit to replace the proximal ascending aorta, reimplanting the coronary ostia into the pulmonary trunk. Seventeen patients, aged 2-62 years, underwent the procedure with three deaths. Hemorrhage precipitated each death, and there was one reexploration for control of bleeding. Postoperative ec

Meniscal Substitution With a Semitendinosus Tendon Autograft.

The menisci play a critical role in load transmission and shock absorption and serve as important secondary stabilizers of the knee. Unfortunately, meniscal repair is not always feasible, and resection may be required. Meniscal allograft transplantation is an established method of meniscal substitution; however, its use remains limited by graft sizing challenges, high costs, and restricted availability. The semitendinosus tendon has the capacity to remodel and revascularize within the intra-articular environment. The purpose of this technical note is to describe the surgical technique for meni

Harvesting of Peroneus Longus Tendon Autograft.

Autografts are always used in anterior cruciate ligament (ACL) reconstruction. Traditional sources, such as hamstring tendons, bone-patellar tendon-bone, and quadriceps tendon, are popular choices but may result in muscle imbalances and dynamic differences in knee kinematics. The peroneus longus tendon (PLT) has emerged as an alternative autograft for ACL reconstruction, offering comparable functional outcomes and avoiding interference with the flexor-extensor complexes of the knee. However, concerns about donor site paresthesia and ankle dysfunction have been raised in PLT harvesting. This ar…

Autograft reoperations after the Ross procedure.

OBJECTIVES: After a Ross procedure, autograft failure can occur. At reoperation, repair of the autograft preserves the advantages of the Ross procedure. The aim of this retrospective study was to assess mid-term results after reoperation of a failed autograft. METHODS: Between 1997 and 2022, 30 consecutive patients (83% male; age 41 ± 11 years) underwent autograft reintervention between 60 days and 24 years (median 10 years) after a Ross procedure. The initial technique varied, full-root replacement (n = 25) being the most frequent. The indication for reoperation was isolated autograft regurg

Applications of maxillary tuberosity block autograft.

OBJECTIVE: Autogenous bone grafts are considered the gold standard due to their compatibility and osteogenic potential to induce new bone formation through osteogenesis, osteoinduction, and osteoconduction. The aim of this paper was to describe clinical applications of the maxillary tuberosity block autograft in small and moderate localized defects of the alveolar process around implants and teeth. CLINICAL CONSIDERATIONS: Maxillary tuberosity is often used as a particulate graft for augmentation of deficient alveolar ridge or maxillary sinus prior to or simultaneously with implant insertion,

Aortic Valve Neocuspidization in a Pulmonary Autograft.

Management of aortic valve (AV) disease in the pediatric population is challenged by a lack of attractive valve replacement options. The Ross procedure has been prominent for these patients; however, long-term reports have revealed concerns, namely neoaortic root dilation and regurgitation. More recently, AV neocuspidization has emerged as an alternative to the Ross operation or other valve replacement procedures in pediatric patients with congenital AV disease. We report a successful case of AV neocuspidization of a previously implanted pulmonary autograft.
