TAC-Tic syndromes: a case series on the complex overlap between trigeminal autonomic cephalalgias and trigeminal neuralgia.

INTRODUCTION: Trigeminal autonomic cephalalgias (TACs) and trigeminal neuralgia (TN) are classified as distinct disorders, although overlap syndromes are increasingly recognized. CASE REPORTS: We retrospectively describe five women with TAC-tic overlap: two with SUNCT-tic, two with cluster-tic, and one with paroxysmal hemicrania-tic syndrome. Each had distinguishable neuralgic and autonomic pain phenotypes, appearing either simultaneously or sequentially. In both SUNCT-tic cases, repeat imaging disclosed neurovascular compression after an initially negative study; microvascular decompression

Harlequin syndrome following paravertebral block adjacent to a stretched pleura: a case report.

BACKGROUND: Harlequin syndrome is characterized by unilateral facial flushing and sweating induced by exercise, heat, and emotion, which is mainly caused by autonomic nervous system impairment. CASE PRESENTATION: A patient underwent thoracoscopic segmentectomy for a tumor located in the apicoposterior segment of the left upper lobe. Within two hours after surgery, the patient developed ipsilateral facial pallor accompanied by contralateral flushing, without any neurological deficits or hemodynamic instability. The symptoms persisted until late at night, and facial erythema resolved spontaneous

Electrophysiological study on limb myokymia in three women.

Physiological studies on three women with limb myokymia were carried out. The patients had diabetic neuropathy, neuromyotonia, and autonomic polyneuropathy, respectively. The EMG discharge pattern, coincident with myokymia, in a patient with myokymia and neuromyotonia differed from those with myokymia without neuromyotonia. In only the first patient did the electrical stimulation of nerves evoke "late repetitive response" (LRR), which resembled the wave forms of the myokymic discharge. Epidural and peripheral nerve blocks abolished myokymia in the first and second cases, but peripheral nerve b

[Case of pure motor Guillain-Barré syndrome with motor conduction block and anti-GM1/GalNAc-GD1a antibody].

A 38-year-old man presented with distal-dominant limb weakness two weeks after an upper respiratory infection. He had no sensory and autonomic signs and no cranial nerve involvement during the course of the disease. Tendon reflexes were preserved except for an absent Achilles' tendon reflex. His disability at nadir was grade 2 on the Hughes functional scale. Cerebrospinal fluid analysis showed albuminocytologic dissociation and he was diagnosed with pure motor Guillain-Barré syndrome (GBS). Thin-layer chromatography immunostaining and an enzyme-linked immunosorbent assay revealed an immunoglob

Recent advances in facilitating the translation of bioelectronic medicine therapies.

Bioelectronic medicine is a growing field which involves directly interfacing with the vagus, sacral, enteric, and other autonomic nerves to treat conditions. Therapies based on bioelectronic medicine could address previously intractable diseases and provide an alternative to pharmaceuticals. However, translating a bioelectronic medicine therapy to the clinic requires overcoming several challenges, including titrating stimulation parameters to an individual's physiology, selectively stimulating target nerves without inducing off-target activation or block, and improving accessibility to clinic

Exercise-induced ventricular tachycardia associated with asymptomatic Brugada syndrome in a patient with urinary bladder stone.

It is well-known that autonomic nerve modulation has an important role in the occurrence of ventricular tachyarrhythmias in Brugada syndrome. A 59-year-old man underwent cardiac evaluation before surgery for urinary bladder stone. He had no cardiac complaints and the only coronary risk factor was heavy smoking. The electrocardiogram showed a saddleback type ST-segment elevation in leads V1-V2, and left axis deviation. During exercise stress test, ventricular tachycardia with a left bundle branch block pattern appeared, and the saddleback type ST-segment elevation in V2 changed into a coved-typ

Nerve entrapment and reflex sympathetic dystrophy after fractures of the distal radius.

In a series of 542 conservatively-treated fractures of the distal radius 30 patients (5,5%) developed compression neuropathy: of the median nerve (n = 23), of the ulnar nerve (n = 5), and of the radial nerve (n = 2). Three patients required decompression of the carpal tunnel, and one patient's ulnar symptoms were relieved after shortening osteotomy of the ulna. The remaining neuropathies resolved spontaneously over a period of three months. Five patients developed typical reflex sympathetic dystrophy with diffuse pain, loss of hand function, and significant autonomic dysfunction. None of them

[Axillary plexus block with long-acting local anaesthetics (a comparative study of etidocaine and bupivacaine) (author's transl)].

Brachial and axillary plexus blockade was carried out on volunteers using a mixture of etidocaine 1% with adrenaline, and bupivacine 0,5% with adrenaline. Both drugs are long-acting local anaesthetics. No difference has been established with regard to duration of analgesia and motor nerve block. Blockade of sympathetic nerve fibres lasts significantly longer with bupivacaine than with etidocaine. With the latter motor blockade seems to be more strongly influenced and significantly outlasts sensory sympathetic nerve block. The results and findings of other authors are discussed, and whether the

[Alternative approach to autonomic instability of very severe tetanus: stellate ganglion block].

Tetanus is an acute and deadly disease caused by Clostridium tetani. A 60-year-old male came to hospital after he injured his thumb with a knife. Ten days later, he returned to hospital with abdominal spasms. He was vaccinated against tetanus and referred to intensive care unit. As he had sudden difficulty in respiration, he was entubated. Midazolam, magnesium and esmolol infusion were started. Next day, muscle spasms progressed all over his body. Midazolam infusion was replaced with propofol and vecuronium. At the third day, morphine infusion was added. At the 16th day, dexmedetomidine infusi

Modeling the response of small myelinated axons in a compound nerve to kilohertz frequency signals.

OBJECTIVE: There is growing interest in electrical neuromodulation of peripheral nerves, particularly autonomic nerves, to treat various diseases. Electrical signals in the kilohertz frequency (KHF) range can produce different responses, including conduction block. For example, EnteroMedics' vBloc® therapy for obesity delivers 5 kHz stimulation to block the abdominal vagus nerves, but the mechanisms of action are unclear. APPROACH: We developed a two-part computational model, coupling a 3D finite element model of a cuff electrode around the human abdominal vagus nerve with biophysically-reali

Role of NaV1.7 in postganglionic sympathetic nerve function in human and guinea-pig arteries.

NaV1.7 plays a crucial role in inducing and conducting action potentials in pain-transducing sensory nociceptor fibres, suggesting that NaV1.7 blockers could be effective non-opioid analgesics. While SCN9A is expressed in both sensory and autonomic neurons, its functional role in the autonomic system remains less established. Our single neuron rt-PCR analysis revealed that 82% of sympathetic neurons isolated from guinea-pig stellate ganglia expressed NaV1.7 mRNA, with NaV1.3 being the only other tetrodotoxin-sensitive channel expressed in approximately 50% of neurons. We investigated the role

Continuous peripheral nerve blocks for burn management: a retrospective study of outcomes and complications in 281 burn patients.

BACKGROUND: There is scarce literature regarding the use of continuous peripheral nerve blocks in acute burn patients, who may be at higher risk for catheter-related complications, including infection. We sought to describe our center's experience and infection rate with continuous perineural catheters in the setting of pain management for patients suffering from burns. METHODS: A retrospective observational study was performed including all patients admitted to an American Burn Association-verified regional burn center between January 2018 and July 2023 who received a continuous peripheral n

A Systematic Review and Meta-Analysis of The Efficacy of Endoscopic Ultrasound Guided Celiac Plexus Blocks for Chronic Pancreatitis Pain.

Chronic pancreatitis is a globally prevalent progressive disease, with pain affecting up to 90% of patients, significantly impairing quality of life and leading to high rates of disability, hospitalizations, and opioid dependence. Pain management is crucial in treating chronic pancreatitis, with endoscopic ultrasound-guided celiac plexus block (EUS-CPB) recognized as an interventional option. This systematic review and meta-analysis, following PRISMA guidelines, synthesized data from 12 studies (5 randomized control trials and 7 observational) on the efficacy of EUS-CPB in managing chronic pan

[Selected interventional methods for the treatment of chronic pain: Part 1: peripheral nerve block and sympathetic block].

Approximately 5-8 million people in Germany suffer from chronic pain. Some patients can obtain relief from specific interventional techniques. In detail these are blocks of the sympathetic chain and peripheral nerve blocks, regional anesthetic techniques close to the spinal cord and neuromodulation. Part 1 of this article presents peripheral nerve blocks using the example of intercostal blocks and blocks of the sympathetic chain. Peripheral nerve blocks are important for postoperative pain treatment. Only a few methods are used for chronic pain and this applies primarily to the intercostal blo

Transcutaneous electrical stimulation of the stellate ganglion: A case report on its application in treating carotid sinus syndrome.

RATIONALE: Syncope is a common condition in emergency departments, posing a diagnostic challenge due to its multifactorial nature. Among the potential causes, carotid sinus hypersensitivity leading to carotid sinus syndrome (CSS) is a significant consideration that can severely impact patient quality of life. Despite its importance, establishing effective treatment methods for CSS has been difficult. PATIENT CONCERNS: A 43-year-old male presented with recurrent episodes of syncope, which significantly affected his daily life and well-being. DIAGNOSES: After a thorough evaluation, the patient
