We report a case of tuberculous vertebral osteomyelitis of the first and second cervical vertebrae with extensive adjacent soft-tissue involvement and extension into the mediastinum and bilateral flanks. The clinical presentation of tuberculous vertebral osteomyelitis depends on the vertebrae involved. The characteristic syndrome, Pott's disease, reflects the consequence of infection of the lower thoracic and lumbar spine, the most common site of tuberculous vertebral osteomyelitis. Cervical involvement is unusual: tuberculosis affects the cervical vertebrae in approximately 0.03% of all cases

OBJECTIVE: Neck imbalance negatively affects body aesthetics of adolescent idiopathic scoliosis (AIS) patients. The evaluation of neck imbalance is currently limited to radiographic parameters, but lacks visual indicators. Therefore, the purpose of this study was to establish indexes of neck imbalance based on body image and to investigate whether these indexes can truly reflect neck imbalance in AIS patients.
METHODS: We performed a cross-sectional study at a single institution between June 2017 and September 2020 and there were 115 subjects involved in this research. All patients were diagn

The cause of death in judicial hanging is controversial and often attributed to 'hangman's fracture' of the second cervical vertebra. Research has shown that such fractures are the exception in judicial hangings and the cause of death can be attributed to a range of head and neck injuries, particularly compression or rupture of the vertebral and carotid arteries leading to cerebral ischaemia. The rapidity of loss of consciousness and death is highly dependent upon knot positioning and the length of drop which has varied through the history of hanging as a capital punishment in the UK. The skel

Fractures and dislocations of the upper cervical spine (atlas and axis) differ markedly from those of the lower cervical spine (C3 - C7) because of the unique anatomy and function of these two vertebrae. Case reports of 4 rugby players who sustained serious injuries of the upper cervical spine are presented. The role of the high tackle in causing these injuries is described and the association of head and upper cervical spinal trauma is emphasized. The radiological management of the player with suspected injury is outlined.

BACKGROUND: Cervical myelopathy due to bilateral laminar cleft of the axis is extremely rare, and few cases have been previously reported. However, its treatment is relatively simple. Surgical removal of the anomalous bony structure compromising the spinal canal can sufficiently achieve a good outcome.
CASE DESCRIPTION: A 56-year-old man presented with a 2-year history of stiffness of the left upper and lower extremities. Cervical spine computed tomography showed a bony cleft at both laminae of the posterior arch of the axis, with obvious cord compression on magnetic resonance imaging. Althou

The vertebral artery is usually described as the first branch of the subclavian artery, originating medial to the scalenus anterior muscle. During its cervical course, the vertebral artery presents a prevertebral segment and then enters the foramen transversarium of the sixth cervical vertebra. We describe a case of an unusual origin and course of the right vertebral artery in a cadaver specimen wherein the right vertebral artery originates from the right common carotid artery at the inferior border of the thyroid gland. In its cervical course the vertebral artery ascends outside and anteriorl

OBJECTIVE: Preoperative cervical malalignment, including an increased cervical sagittal vertical axis and proximal junctional angle (PJA), has been associated with proximal junctional kyphosis (PJK) following thoracolumbar deformity correction. However, the influence of cervical disc degeneration on PJK risk remains unclear. This study evaluated whether preoperative MRI-based cervical degeneration predicts PJK following thoracolumbar deformity correction in patients with adult spinal deformity (ASD).
METHODS: One hundred twenty-six patients with ASD and preoperative cervical MRI who underwent

Recent advances and the technical options that have lately become available in the surgical treatment of spinal injuries mean that clear definition of the indications is essential. Precise observation and classification of each injury to the cervical spine are of paramount importance for this purpose, to allow the comparison of proposed methods of treatment and of the results obtained with them. There are a number of classification systems for injuries to the cervical spine, none of which, however, is generally accepted. These classification systems are based on different criteria, such as nat

Craniospinal and cervicospinal malformations were found in 15 (53.5%) of 28 cases of maxillonasal dysostosis (Binder syndrome) in which a complete radiographic analysis could be done. These anomalies have not been reported previously. Analysis of the anomalies suggested a common induction factor, or, at least, a simultaneity in the development of the maxillonasal and vertebral dysplasias that characterize Binder syndrome. The following procedures are recommended: (1) examination of the cranial and cervical spine in all subjects with symptoms of Binder syndrome, and (2) regular examination of p

BACKGROUND: Atlas fractures represent 25% of craniocervical injuries, most common in elders. Unstable first cervical vertebra fractures with lateral mass displacement require surgical fixation. Options include atlantoaxial fusion (AAF) or occiput-cervical fusion (OCF), each have their respective merits and detractors. Here, we compare the clinical and neurological outcomes of patients with unstable atlas fractures treated with AAF and OCF.
METHODS: A retrospective review of patients with unstable atlas fractures that underwent either AAF or OCF between 2005 and 2023 was performed. Demographic

Hangman's fracture in ventrally flexed traction between 10(6) and 75 degrees is analyzed on the basis of 220 fracture trials on plaster of real bone preparations of the atlas and axis of two adults. Hangman's fractures, which are usually regarded as a form of injury from dorsal flexion, occurred in 25% of the fracture trials under this kind of strain. Frequency depended on flexion angle, as well as on inclination of the dens axis. Thus Hangman's fractures occurred, as a whole, more frequently in the kyphotic dens type than in the lordotic type and were found as a rule at 75 degrees flexion. Fu

A technique for the stabilization of atlantoaxial subluxation in the dog was developed. The technique involved use of the nuchal ligament as a means of securing the spinous process of the axis to the dorsal arch of the atlas. Two cases were typical atlantoaxial subluxation in toy-breed dogs, and 2 cases were traumatic subluxation secondary to fracture of the axis in dogs of large breeds. On the basis of the results in these cases, it was concluded that the technique has advantages over techniques in which wire or nonmetallic suture material is utilized for stabilization.

Persistent torticollis in younger patients, particularly after trivial trauma or an upper respiratory tract infection, suggests a diagnosis of atlantoaxial rotary fixation. The diagnosis can be confirmed by cineroentgenography. Anterior displacement of the atlas, indicating a deficient transverse ligament, should be ruled out by flexion-extension lateral roentgenograms. If conservative management fails to achieve reduction or is followed by a recurrence of the deformity, arthrodesis is indicated.

A post mortem material of 11 consecutive cases of severe atlanto-axial dislocation (a.a.d.) with cord compression is reported. The total number of deaths from rheumatoid arthritis (RA) during the period of 5 years was 104, and all were autopsied. Neurological symptoms correlated poorly to fatal a.a.d. Hemiplegia was found in three cases, one of which, however, was caused by thrombosis cerebri. Spastic signs were transiently recorded in two patients and dysphagia in a further two. Five patients had a history of recent vomiting. A.a.d. was the sole or main cause of death in 8 cases and contribut
