Assessing cerebrovascular contribution to late dementia of the Alzheimer's type: the role of combined hemodynamic and structural MR analysis.

The critical question as to the respective role of Alzheimer's disease (AD) and cerebrovascular disease in dementia of the Alzheimer's type (DAT), the most common form of dementia, is still debated. But there has been considerable progress in understanding cerebral hemodynamics and the relationship between structural brain damage and cognitive decline, in routine neuro imaging techniques. These advances now allow the proposition of a novel MR classification of DAT including indicators of both cerebrovascular function and regional brain atrophy. MR indicators of windkessel function include the

Assessment of fetal blood volume for computer-assisted management of in utero transfusion.

We performed 41 intravascular ultrasound-guided fetal transfusions in a total of 20 pregnancies with erythroblastosis fetalis or alloimmune thrombocytopenia. On the basis of this experience, we developed a computer-assisted procedure for determining the volume to be transfused, which provides an adequate final concentration. Fetal weight was estimated using ultrasound measurements. Fetoplacental blood volume was estimated from the regression line: fetoplacental volume (ml) = 1.046 + fetal weight (g) X 0.14. The volume to be transfused was calculated using the simple dilution formula: Vtransfus

Human cerebral blood volume (CBV) measured by dynamic susceptibility contrast MRI and 99mTc-RBC SPECT.

BACKGROUND: Patients with elevated intracranial pressure risk compromising their cerebral blood flow, resulting in ischemia. Lowering of the raised intracranial pressure, is therefore, mandatory. Reduction of the cerebral blood volume (CBV) might be target. In finding ways to do so, one has to be able to measure CBV. Measurement of CBV is, however, difficult. Radio(99mTc-)labeled erythrocytes (99mTcRBC) single photon emission computer-aided tomography (SPECT) is one established method used for CBV measurement. Recently, dynamic susceptibility contrast (DSC) magnetic resonance imaging (MRI), ha

Intracranial dural arteriovenous fistulas with retrograde cortical venous drainage: assessment with cerebral blood volume by dynamic susceptibility contrast magnetic resonance imaging.

BACKGROUND AND PURPOSE: Retrograde cortical venous drainage (RCVD) is the most major risk factor for aggressive behavior of intracranial dural arteriovenous fistulas (DAVF). The purpose of this study was to assess the efficacy of relative cerebral blood volume (rCBV) map for RCVD in patients with DAVF. METHODS: Ten patients with angiographically proven DAVF with RCVD, 2 reference patients with DAVF without RCVD, and 10 control subjects underwent examinations with dynamic susceptibility contrast (DSC)-MR imaging. Four patients with DAVF with unilateral RCVD were evaluated, before and after tre

Ultrasonic blood flow volume assessment in the extracranial internal carotid artery in arteriovenous malformations.

Angle-independent ultrasound assessment of the volume flow in the extracranial internal carotid artery (ICA) is a new approach to evaluate the perfusion status of the human brain. Normal values using a new technical device (Quantix ND, Cardiosonix Ltd, Israel) and correlation analysis to quantitative CBF measurements were recently published. Aim of this study was to evaluate this device in arteriovenous malformations where cerebral blood flow is known to be increased. Five patients suffering from large supratentorial arteriovenous malformations (AVM) could be examined. All patients showed pre-

Noninvasive assessment of arterial compliance of human cerebral arteries with short inversion time arterial spin labeling.

A noninvasive method of assessing cerebral arterial compliance (AC) is introduced in which arterial spin labeling (ASL) is used to measure changes in arterial blood volume (aBV) occurring within the cardiac cycle. Short inversion time pulsed ASL (PASL) was performed in healthy volunteers with inversion times ranging from 250 to 850 ms. A model of the arterial input function was used to obtain the cerebral aBV. Results indicate that aBV depends on the cardiac phase of the arteries in the imaging volume. Cerebral AC, estimated from aBV and brachial blood pressure measured noninvasively in systol

The importance of monitoring during operations on conjoined twins.

Besides complete and careful preoperative investigation of thoracopagus conjoined twins, monitoring of the various parameters, such as arterial blood pressure, central venous pressure, blood pH, and temperature, are most essential for accurate blood replacement, maintenance of normal body chemistry and thermoregulation. Equally important is continued monitoring in the postoperative period when assisted ventilation may be required as well.Twin "A" returned from the operating room in excellent condition, breathing spontaneously through an endotracheal tube. Twin "B" returned on positive-pressure

Relative blood volume measurements by magnetic resonance imaging facilitate detection of testicular torsion.

RATIONALE AND OBJECTIVES: The authors determine the utility of relative blood volume measurements (rBV) using a blood pool marker for magnetic resonance imaging (MRI) in detection of early testicular torsion. METHODS: Testicular torsion was induced in rats by counterclockwise 720 degrees rotation and fixation of the testis in the scrotum. MPEG-PL-DTPA-Gd enhanced MRI (30 mumol Gd/kg bolus injection) was performed 1 hour after torsion at 1.5 T using fat-suppressed three-dimensional fast spoiled gradient-recalled sequence for relative blood volume measurement and three-dimensional time-of-fligh

Validation of fractional moving blood volume measurement with power Doppler ultrasound in an experimental sheep model.

OBJECTIVE: To compare fractional moving blood volume (FMBV) estimation using power Doppler ultrasound (PDU) with blood flow estimation using radioactive microspheres (RMS) for evaluation of fetal organ blood perfusion. METHODS: Blood flow was measured in the adrenal gland of nine exteriorized fetal lambs. Five fetal lambs underwent total umbilical cord occlusion in order to induce changes in the adrenal blood flow (asphyxia group). Four lambs were used as sham controls (control group). Three RMS injections, with coincident PDU recordings of the adrenal gland, were performed in each lamb. In t

Craniosynostosis: an assessment of blood loss and transfusion practices.

Assessment and accurate replacement of blood loss during primary craniosynostosis repair is difficult due to patient size and surgical technique. Eighty-five charts of all patients undergoing primary craniosynostosis repair over a 15-year period were reviewed to determine blood loss and to assess blood transfusion practices both intraoperatively and postoperatively. Blood loss was calculated on the basis of estimated red cell mass (ERCM). Blood transfusion management was considered appropriate if the postoperative or posttransfusion ERCM was within 15 per cent of the preoperative value. Isolat

Evaluation of the "tilt test" in children.

The "tilt test" (assessment of orthostatic pulse and blood pressure) is a non-invasive screening test for acute intravascular volume loss with well-accepted utility in the emergency evaluation of adults. Its value in children has been questioned because of reports of high false-positive rates. We report a prospective study to compare "tilt test" results between children with and without clinical volume depletion in a pediatric emergency setting. Children 4 to 15 years old seeking care at an urban pediatric emergency department were eligible. Children meeting the following criteria were classif

Orthostatic changes in normovolemic children: an analysis of the "tilt test".

Orthostatic changes in pulse rate and blood pressure were assessed on 455 normovolemic children between 4 and 17 years of age who visited emergency departments with a variety of complaints. Blood pressures and pulse rates were measured in the upper extremity with the patient supine, sitting, and standing. The postural changes in pulse rate and blood pressure for age groups 4 to 9 years, 10 to 13 years, and 14 to 18 years were computed, and statistical analyses were performed to identify factors predisposing to changes in pulse rate and blood pressure. In all categories of chief complaint, comp
