Evaluation of a weight management service in the Northern Territory, Australia.

BACKGROUND: There is a growing demand for specialist weight management services across Australia, yet publicly funded weight management services are lacking, with limited data on the effectiveness of these services. AIMS: To evaluate the effectiveness of a regional specialist weight management service on weight loss and weight loss maintenance, and to determine attrition rates of this service. METHODS: A prospective evaluation of adults who attended the publicly funded Top End Weight Management Service, with their first appointment between August 2021 and August 2023 and follow-up period of

European Guidelines for Obesity Management in Adults.

Obesity is a chronic metabolic disease characterised by an increase of body fat stores. It is a gateway to ill health, and it has become one of the leading causes of disability and death, affecting not only adults but also children and adolescents worldwide. In clinical practice, the body fatness is estimated by BMI, and the accumulation of intra-abdominal fat (marker for higher metabolic and cardiovascular disease risk) can be assessed by waist circumference. Complex interactions between biological, behavioural, social and environmental factors are involved in regulation of energy balance and

Rumination in anorexia nervosa: Cognitive-affective and neuroendocrinological aspects.

Rumination about body weight/figure as well as food is common in patients with Anorexia Nervosa (AN) and may be a maintenance factor of the disorder. While rumination can generally be considered as a cognitive-affective process, food-related rumination may be driven primarily by a physiological response to undernutrition. In the current longitudinal study, we integrate ecological momentary assessment of rumination and affect and, as a biological marker of undernutrition, plasma leptin levels collected from 33 AN patients. At the very acute stage and again after short-term weight-restoration pa

Energy intake, body composition and reproductive performance of the gilt.

The relationship between age, live weight, body composition and energy status on the onset and maintenance of reproductive activity in females is reviewed. When possible, swine studies have been employed, although, of necessity, much data are drawn from other species. The relationship between age, weight and puberty is controversial. However, from the available data we conclude that neither age nor weight are reliable indices of reproductive development, but that minimum threshold values for these characteristics must be achieved before puberty can occur. Human data provides a strong indicatio

Multicenter prospective trial evaluating the tolerability of imatinib for Japanese patients with chronic myelogenous leukemia in the chronic phase: does body weight matter?

Imatinib at a daily dose of 400 mg is the standard treatment for chronic myelogenous leukemia in the chronic phase. However, the feasibility of this dose for small Japanese adults has not been clarified. We prospectively investigated the toxicity and efficacy of this dose in adult Japanese patients. Among the 89 evaluable patients with a median body weight of 62.8 kg, imatinib therapy was held in 40 subjects (45%), due to Grade 3-4 toxicities in 30 patients (75%) and Grade 2 toxicities at the discretion of the attending physician in 10 patients (25%). However, treatment was resumed and the dos

Characterization of phenobarbital pharmacokinetics and dosing in adults receiving extracorporeal membrane oxygenation.

BACKGROUND: Achieving adequate sedation on extracorporeal membrane oxygenation (ECMO) is challenging due to altered pharmacokinetics. Limited data exists on phenobarbital in adult ECMO patients. The aim was to characterize the volume of distribution (Vd) and dosing. METHODS: This single-center retrospective study examined adult ECMO patients who received intravenous phenobarbital and corresponding serum concentrations between January 1, 2017 and March 1, 2024. Phenobarbital levels for analysis were obtained 0.5-4 h after phenobarbital loading dose (LD) and up to 4 h prior to a maintenance dos

Long-term efficacy of dietary weight loss in sleep apnoea/hypopnoea syndrome.

Weight loss is associated with clinical improvement in sleep apnoea/ hypopnoea syndrome (SAHS). The aim of this study was to ascertain whether the therapeutic efficacy of dietary weight loss is maintained in the long-term. From a total of 216 overweight SAHS patients treated by only a weight reduction programme, 24 cured by this method (apnoea/hypopnoea index (AHI) at diagnosis 443+/-27.8, after weight loss 3+/-3.1) were re-evaluated after a mean (+/-SD) period of 94.3+/-27.4 months post-cure. No correlation was found between changes in AHI and body mass index experienced by each patient in th

Weight Loss in Advanced Chronic Kidney Disease: Should We Consider Individualised, Qualitative, ad Libitum Diets? A Narrative Review and Case Study.

In advanced chronic kidney disease, obesity may bring a survival advantage, but many transplant centres demand weight loss before wait-listing for kidney graft. The case here described regards a 71-year-old man, with obesity-related glomerulopathy; referral data were: weight 110 kg, Body Mass Index (BMI) 37 kg/m², serum creatinine (sCr) 5 mg/dL, estimated glomerular filtration rate (eGFR) 23 mL/min, blood urea nitrogen (BUN) 75 mg/dL, proteinuria 2.3 g/day. A moderately restricted, low-protein diet allowed reduction in BUN (45-55 mg/dL) and good metabolic and kidney function stability, with a

Effects of an early, late, and self-selected time-restricted eating intervention on weight loss maintenance in adults with overweight or obesity: A 12-month follow-up of a randomized controlled trial.

BACKGROUND & AIMS: Time-restricted eating (TRE), regardless of the eating window timing, promotes modest weight loss and improves cardiometabolic health. However, the sustained maintenance of these benefits over the long term is unclear. This study aims to investigate the effects of TRE schedules-8-h windows in the early day, late day, and participant-selected times-on body weight loss maintenance compared to usual care (UC), 12 months after a 12-week intervention in adults with overweight or obesity. METHODS: This parallel-group, randomized controlled trial recruited 99 adults with overweigh

Anorexia nervosa and body fat distribution: a systematic review.

The aim of this paper was to conduct a systematic review of body fat distribution before and after partial and complete weight restoration in individuals with anorexia nervosa. Literature searches, study selection, method development and quality appraisal were performed independently by two authors, and data was synthesized using a narrative approach. Twenty studies met the inclusion criteria and were consequently analyzed. The review had five main findings. First, during anorexia nervosa adolescent females lose more central body fat, while adult females more peripheral fat. Second, partial we

Predicting allopurinol response in patients with gout.

AIMS: The primary aim of this research was to predict the allopurinol maintenance doses required to achieve the target plasma urate of ≤0.36 mmol l(-1) . METHODS: A population analysis was conducted in nonmem using oxypurinol and urate plasma concentrations from 133 gout patients. Maintenance dose predictions to achieve the recommended plasma urate target were generated. RESULTS: The urate response was best described by a direct effects model. Renal function, diuretic use and body size were found to be significant covariates. Dose requirements increased approximately 2-fold over a 3-fold ran

Causes and consequences of the reverse epidemiology of body mass index in dialysis patients.

A high body mass index (BMI) has been consistently shown to have a strong predictive correlation to decreased all-cause and cardiovascular mortality in maintenance hemodialysis (MHD) patients, ie, individuals with advanced chronic kidney disease undergoing maintenance dialysis. Indeed, according to some but not all, even morbid obesity (BMI > 35 kg/m 2 ) confers survival advantages. Among the possible causes of reverse epidemiology of BMI are the following: (1) stable hemodynamic status in obesity, (2) higher concentrations of receptors of tumor necrosis factor-alpha and neurohormonal alterati
