Lengthening of the forearm by callus distraction.

Ten patients aged 3 to 13 years (mean, 9 years and 7 months) underwent forearm lengthening by callotasis. The indications for lengthening were shortening and/or deformity of the forearm due to exostosis of the distal ulna in five cases, enchondroma of the distal ulna in one, growth disturbance after fracture of the distal radius in one, radial club hand in one, congenital amputation of the forearm in one and congenital dislocation of the radial head in one. Four had lengthening of the ulna, one of the radius and five of both the radius and the ulna. The average lengthening achieved was 30 mm.

Comparison of the effect of reamed and unreamed locked intramedullary nailing on blood flow in the callus and strength of union following fracture of the sheep tibia.

This study was performed to compare the effects of reamed and unreamed locked intramedullary nailing on blood flow in the callus and early strength of union in a fractured sheep tibia model. After the creation of a standardized short spiral fracture by three-point bending with torsion, each tibia was stabilized by the insertion of a locked intramedullary nail. Ten animals were allocated randomly into two groups: one that had reaming prior to nail insertion and one that did not. Blood flow was measured in real time with use of laser Doppler flowmetry. Endosteal perfusion was determined at the f

Elastic stable intramedullary pinning of long bone shaft fractures in children.

Stable intramedullary pinning is an excellent method for treating shaft fractures in children when there is an indication for operative management. It is a new biomechanical concept which aims at developing the bridging external callus. This closed pinning uses flexible rods which are introduced into a metaphysial area. Technique, results and indications are described particularly in regard to shaft fractures of the femur and the forearm. This closed surgical procedure can always be performed; complication rates are slight (no sepsis, no delayed union). Early mobilisation is possible, and use

Flexible intramedullary nailing of tibial-shaft fractures.

Closed flexible intramedullary nailing was used in 38 fractures of the tibia shaft. Simple transverse, oblique, or spiral fractures were shown to be stable after nailing. Fractures with a large unicortical butterfly fragment, involving more than one-third of the cortex, had a tendency to angulate. There were 23 fractures in the distal third of the tibial shaft; nine in the middle third; and four in the proximal third. The operations were simple and quick, lasting an average of 38 minutes. Sixteen patients used a short walking cast, usually for periods of four to six weeks. The cast was usually

Scanning electron microscopic studies of the staples in normal and in some pathological and experimental conditions.

By means of the electro-scanning microscope we were able to demonstrate the three-dimensional bony structure of the normal stapes, and the variations in structure of the different stages of otosclerosis. One case of osteopsatyrosis is also discussed. The callus formation in an experimentally fractured footplate is compared with otosclerosis.

Metformin regulates bone marrow stromal cells to accelerate bone healing in diabetic mice.

Diabetes mellitus is a group of chronic diseases characterized by high blood glucose levels. Diabetic patients have a higher risk of sustaining osteoporotic fractures than non-diabetic people. The fracture healing is usually impaired in diabetics, and our understanding of the detrimental effects of hyperglycemia on fracture healing is still inadequate. Metformin is the first-line medicine for type 2 diabetes (T2D). However, its effects on bone in T2D patients remain to be studied. To assess the impacts of metformin on fracture healing, we compared the healing process of closed-wound fixed frac

Direct electromagnetic coupling for non-invasive measurements of stability in simulated fracture healing.

Diagnostic monitoring and prediction of bone fracture healing is critical for the detection of delayed union or non-union and provides the requisite information as to whether therapeutic intervention or timely revision are warranted. A promising approach to monitor fracture healing is to measure the mechanical load-sharing between the healing callus and the implanted hardware used for internal fixation. The objectives of this study were to evaluate a non-invasive measurement system in which an antenna electromagnetically couples with the implanted hardware to sense deflections of the hardware

The detection of significant fractures in suspected infant abuse.

OBJECTIVE: Skeletal survey is a commonly used means of detecting fractures in infants, and is used as a screen in suspected cases of physical abuse. It is recognised that in live infants, a repeat survey some days after a suspected episode of injury will detect more fractures than one taken shortly after the suspected injury, indicating that the latter lacks sensitivity. In infants who die soon after a suspected episode of physical abuse, the managing clinicians do not have the option of a second survey; however there is the opportunity for the microscopic examination of bones removed at autop

Fusion of cervical vertebrae from a basal archosauromorph from the Middle Triassic Denwa Formation, Satpura Gondwana Basin, India.

This report describes two adjacent, longitudinally-fused anterior cervical vertebrae from a basal archosauromorph. The specimen was collected from the Denwa Formation, Satpura Gondwana Basin, India. The differential diagnosis of the fusion includes genetic or environmentally-mediated congenital malformations, nonspecific spondyloarthopathy, and various infectious agents. These observations represent the first published recognition of archosauromorph vertebral pathology from specimens that were discovered in India. The observations affirm that basal archosauromorphs suffered from disorders that

Low dose erythropoietin stimulates bone healing in mice.

Beyond its classical role in regulation of erythropoiesis, erythropoietin (EPO) has been shown to exert protective and regenerative actions in a variety of non-hematopoietic tissues. However, little is known about potential actions in bone regeneration. To analyze fracture healing in mice, a femoral 0.25 mm osteotomy gap was stabilized with a pin-clip technique. Animals were treated with 500 U EPO/kg bw per day or with vehicle only. After 2 and 5 weeks, fracture healing was analyzed biomechanically, radiologically and histologically. Expression of PCNA and NFκB was examined by Western blot ana

Delayed radial nerve palsy due to entrapment of the nerve in the callus of a distal third humerus fracture.

A 45-year-old male patient was referred for management of radial nerve palsy. His past medical history revealed that he had been injured in a car accident and broken his left humerus 4 months ago. Primary stabilization of the fracture has been achieved by the application of a long-arm plaster cast. His medical reports displayed that he had experienced no clinical signs of radial nerve palsy at that time. After the cast was removed, he noticed that he could not extend his wrist. Surgical exploration revealed that the radial nerve was encased inside the callus. He had probably not realized the w

Metatarsal lengthening in congenital brachymetatarsia: one-stage lengthening versus lengthening by callotasis.

We retrospectively reviewed and compared the outcomes of 15 one-stage metatarsal lengthenings with intercalary bone graftings in 10 patients with those of nine metatarsal lengthenings by callotasis in five patients younger than 15 years. In the one-stage lengthening group, the diaphyseal osteotomy site was gradually distracted intraoperatively for 20-30 min to relax the surrounding soft tissues. In the callotasis group, lengthening was achieved with mini-Orthofix M-100. There was little difference in the outcomes between the two groups in terms of length gain, percentage increase, and complica

Temporal variation of applied inter fragmentary displacement at a bone fracture in harmony with maturation of the fracture callus.

The amplitude of inter fragmentary displacement in long bone fractures greatly influences the pattern and speed of healing. Unfortunately, the amplitude of natural cyclical displacement arising from patient activity is random because of the inherent flexibility of fixation devices under natural loading. Although fixators may be designed to control the amplitude of this displacement, the amplitudes most beneficial to healing have not been determined. Furthermore, the appropriate amplitude must vary during healing as the reparative tissue (callus) progresses histologically and stiffens during ma

Phosphorylation of epidermal keratins.

When human and rat epidermis are exposed to 32P-orthophosphoric acid, labeled phosphate is incorporated into several proteins. The pattern of phosphorylation is identical whether the isotope is delivered in vivo or in vitro. The predominant phosphorylated proteins are insoluble in Tris-HCl buffer but soluble in SDS-beta-mercaptoethanol. They migrate in SDS-polyacrylamide gels with apparent molecular weights between 45,000 and 65,000. When analyzed by two-dimensional gel electrophoresis, the labeled phosphoproteins co-migrate with keratins isolated from human callus. Serine is the phosphate acc

[The effects of transection of inferior alveolar nerve on inflammatory factors during haematoma phases in mandibular defect healing of denervated rabbits].

OBJECTIVE: To study the effects of transection of inferior alveolar nerve (IAN) on the healing of mandibular fracture by investigating the changes of inflammatory factors expression, including tumor necrosis factor-alpha (TNF-alpha), interleukin-1 (IL-1) and interleukin-6 (IL-6) in the denervated rabbits' bony callus. METHODS: 50 Chinese rabbits were randomly divided into the experimental group and control group, the experimental group underwent mandibular fracture and transection of IAN and the control group underwent mandibular fracture only. The bony callus were collected at 1, 2, 3, 5 and…

The effect of reaming on the repair of a rabbit tibial osteotomy stabilised by an intramedullary locking nail.

AIM: The aim of this investigation is to study the effect of intramedullary reaming on the repair of a rabbit tibial osteotomy stabilised by a specially designed locking nail. MATERIALS AND METHODS: A tibial osteotomy was carried out in two similar groups of ten New Zealand White rabbits. In one group, reaming was carried out prior to nailing, while in the second group, the nail was inserted without reaming. At four weeks postoperatively, CT scans of operated and non-operated contralateral tibiae enabled the percentage increase in bone volume of the whole tibial shaft to be measured in both r

Molecular aspects of healing in stabilized and non-stabilized fractures.

Bone formation is a continuous process that is initiated during fetal development and persists in adults in the form of bone regeneration and remodeling. These latter two aspects of bone formation are clearly influenced by the mechanical environment. In this study we tested the hypothesis that alterations in the mechanical environment regulate the program of mesenchymal cell differentiation, and thus the formation of a cartilage or bony callus, at the site of injury. As a first step in testing this hypothesis we produced stabilized and non-stabilized tibial fractures in a mouse model, then use…
