Incidental diagnosis of two intracranial aneurysms following surgical evacuation of chronic subdural hematoma.

The development of subarachnoid hemorrhage (SAH) following evacuation of chronic subdural hematoma (CSDH) is a very rare phenomenon. SAH in this context occurring secondary to intracranial aneurysm rupture is still rare. We report a case of an elderly lady who presented with right hemiplegia and altered sensorium. Computed tomography (CT) scan of the brain revealed a left fronto-temporoparietal CSDH with midline shift, which was promptly evacuated surgically via a single burr-hole. Postoperatively, her level of consciousness deteriorated and there was increased the amount of drain. Emergency C

Penetration of intravenous hydroxyethyl starch into the cerebrospinal fluid in patients with impaired blood-brain barrier function.

UNLABELLED: Hypovolemic patients with impairment of the blood-brain barrier may receive IV hydroxyethyl starch (HES) to stabilize cardiovascular function and to increase cerebral perfusion pressure. It is not known whether HES can penetrate into the cerebrospinal fluid (CSF) under those conditions. We investigated plasma and CSF levels of HES after IV infusion in patients with suspected disturbance of the blood-brain barrier. Eight adult patients were studied who were being treated for head trauma or subarachnoid hemorrhage, with an external CSF drain in place. All patients exhibited radiograp

Intracranial Pressure Monitoring and Management in Aneurysmal Subarachnoid Hemorrhage.

Aneurysmal subarachnoid hemorrhage is a medical condition that can lead to intracranial hypertension, negatively impacting patients' outcomes. This review article explores the underlying pathophysiology that causes increased intracranial pressure (ICP) during hospitalization. Hydrocephalus, brain swelling, and intracranial hematoma could produce an ICP rise. Although cerebrospinal fluid withdrawal via an external ventricular drain is commonly used, ICP monitoring is not always consistently practiced. Indications for ICP monitoring include neurological deterioration, hydrocephalus, brain swelli

Intraparenchymal Occipital Cyst Formation after Contralateral Dural Arteriovenous Fistula Obliteration: A Case Report.

Postoperative intracranial cyst formation is a rare complication. We report a case of an air-containing cyst in the contralateral left occipital lobe parenchyma after ligation of a dural arteriovenous fistula of the right transverse sinus. We reviewed related literature and discussed the mechanisms of this rare complication. A 59-year-old man presented with a right transverse sinus dural arteriovenous fistula with cortical reflux discovered incidentally during a brain checkup. The patient underwent surgical ligation of the draining vein combined with spinal drainage for intraoperative brain re

Drainage of cells and soluble antigen from the CNS to regional lymph nodes.

Despite the absence of conventional lymphatics, there is efficient drainage of both cerebrospinal fluid (CSF) and interstitial fluid (ISF) from the CNS to regional lymph nodes. CSF drains from the subarachnoid space by channels that pass through the cribriform plate of the ethmoid bone to the nasal mucosa and cervical lymph nodes in animals and in humans; antigen presenting cells (APC) migrate along this pathway to lymph nodes. ISF and solutes drain from the brain parenchyma to cervical lymph nodes by a separate route along 100-150 nm wide basement membranes in the walls of cerebral capillarie

Intracranial pial single-channel arteriovenous fistula presenting with significant brain edema.

The authors report a rare case of pial single-channel arteriovenous fistula presenting with significant brain edema. A 51-year-old woman was admitted with a 5-day history of headache and nausea, followed by consciousness disturbance. Computed tomography showed cerebellar swelling with obstructive hydrocephalus. Magnetic resonance imaging revealed extensive vasogenic edema in the cerebellum bilaterally. Angiography demonstrated 2 different arteriovenous shunts (AVSs) at peripheral branches of the right anterior inferior cerebellar artery. One was located on the suboccipital surface. It drained

Paradoxical emboli from left superior vena cava causing recurrent brain abscess.

Persistent left superior vena cava is a rare but well-recognised condition. We describe a case of persistent left superior vena cava draining directly into the left atrium, with a fixed anatomical right-to-left shunt and paradoxical embolic events causing recurrent brain abscess. Surgical ligation was curative.

Voluntary locomotion induces an early and remote hemodynamic decrease in the large cerebral veins.

SIGNIFICANCE: Behavior regulates dural and cerebral vessels, with spontaneous locomotion inducing dural vessel constriction and increasing stimulus-evoked cerebral hemodynamic responses. It is vital to investigate the function of different vascular network components, surrounding and within the brain, to better understand the role of the neurovascular unit in health and neurodegeneration. AIM: We characterized locomotion-induced hemodynamic responses across vascular compartments of the whisker barrel cortex: artery, vein, parenchyma, draining, and meningeal vein. APPROACH: Using 2D-OIS, hemo

Postoperative drainage for 6, 12, or 24 h after burr-hole evacuation of chronic subdural hematoma in symptomatic patients (DRAIN-TIME 2): study protocol for a nationwide randomized controlled trial.

BACKGROUND: Chronic subdural hematoma (CSDH) is a common acute or subacute neurosurgical condition, typically treated by burr-hole evacuation and drainage. Recurrent CSDH occurs in 5-20% of cases and requires reoperation in symptomatic patients, sometimes repeatedly. Postoperative subdural drainage of maximal 48 h is effective in reducing recurrent hematomas. However, the shortest possible drainage time without increasing the recurrence rate is unknown. METHODS: DRAIN-TIME 2 is a Danish multi-center, randomized controlled trial of postoperative drainage time including all four neurosurgical d

CSF transport at the brain-meningeal border: effects on neurological health and disease.

The existence of specialised structures that allow a continuous exchange of CSF between different anatomical compartments at the brain-meningeal border is challenging conventional notions around molecular transport within the brain. Experimental findings highlight the conduits and cellular structures controlling the transport of CSF and immune cells between the brain parenchyma (via the glymphatic system), the subarachnoid space (enclosed by the meningeal pia and arachnoid layers), and the outmost meningeal dural layer and calvaria (via the so-called arachnoid cuff exit points). Full recovery after severe bacterial encephalitis and brain abscess following deep brain stimulation surgery for Parkinson's disease.

Encephalitis and cerebral abscesses are rare, serious complications of deep brain stimulation (DBS) surgery. We report on two patients with Parkinson's disease (PD) who developed a critical condition with focal neurological symptoms and decreased consciousness, after an initially uncomplicated subthalamic nucleus DBS implantation. Because of the clinical and radiological suspicion of encephalitis and brain abscess, DBS was explanted and the abscess drained. After prolonged antibiotic treatment they gradually fully recovered, with persistently improved PD symptoms in patient 1. Concluding, pati

Report of an immunocompetent case with disseminated infection due to Nocardia otitidiscaviarum: Identification by 16S rRNA gene sequencing.

Nocardia otitidiscaviarum belongs to the agents of opportunistic infections seen in immunocompromised patients, but may occur rarely in immunocompetent patients. In this report we described a case of a previously healthy 69-year-old woman with cerebral and retroperitoneal abscess due to Nocardia otitidiscaviarum. The patient was admitted to hospital because of loss of strength in her right arm and leg. Nocardia spp. was isolated from the abscess material. The intracranial lesions were drained by stereotactic craniotomy. The large abscess located around the left kidney was drained and microscop

Thrombolysis for non-traumatic intra-ventricular hemorrhage in adults: a critical reappraisal.

Intra-ventricular hemorrhage (IVH) is a critical condition with high rate of morbidity and mortality due to acute hydrocephalus and secondary brain injury. Mechanisms underlying the clinical deterioration are not only related to the appearance of an acute hydrocephalus but also to blood-clot mass effect and the inflammatory effects of blood break-down products which impede local blood flow and exert a direct toxic effect on the peri-ventricular structures leading to chronic hydrocephalus. An effective treatment strategy should aim at IVH fast removal and reduction of blood-clot mass effect. Al

Autologous and acrylic cranioplasty: a review of 10 years and 258 cases.

INTRODUCTION: Cranioplasty is a well-accepted neurosurgical procedure that has application to a wide range of pathologies. Given the varied need for both autologous and synthetic cranial grafts, it is important to establish rates of procedural complication. METHODS: A retrospective review identified 282 patients undergoing cranioplasty at our institution over a 10-year period, of which 249 patients underwent 258 cranioplasties with either autologous or acrylic flaps. A database including patient age, gender, presenting diagnosis, hospital of surgery, presence of a drain, and surgical complica

An Incidental Discovery of Supracardiac Left-sided Partial Anomalous Pulmonary Connection and Dilation of the Azygos Vein Detected in Adulthood.

INTRODUCTION: Anomalous pulmonary venous connection is a rare congenital variant of the pulmonary veins drained into the right atrium. The left-side partial anomalous pulmonary connections are usually detected in adulthood and occasionally drain into the left brachiocephalic vein. CASE DESCRIPTION: An asymptomatic 63-year-old woman with a known history of left carotid body paraganglioma was admitted to our institution to evaluate this tumor by computed tomography angiography of the neck and brain. As an incidental finding, CTA demonstrated a supracardiac partial anomalous pulmonary venous con

Natural history of intracranial vascular malformations: a review.

It is difficult to assess the natural history of intracranial vascular malformations because they are varied in nature, they are frequently silent clinically, they are often treated when they are discovered, and untreated lesions are not often followed in an organized way. Capillary telangiectasias are usually occult lesions of no clinical significance. Cavernous hemangiomas may cause seizures and may bleed, but the approximate yearly risks of bleeding and of death have not been determined. Venous angiomas seldom cause symptoms, with the exception that those in the cerebellum seem to have a pr
