Four cases of spontaneous haemorrhages to the brain stem were described. They concerned young patients (middle age was 34 years) without previous risk factors. The diagnosis was established on the ground of clinical examination, neuroimaging methods-CT, MR, cerebral angiography. We could not indicate the cause of haemorrhage (i.e. a blood vessel pathology, nor coagulation disturbances neither other pathological coincidence were found). Vascular malformations of the brain stem are a histologically heterogenous collection of lesions which most often present with sudden and progressive neurologic deficit related to haemorrhage. Since 1987 the authors have treated eleven cases of brain stem vascular malformation. Seven of the patients were treated with complete surgical extirpation of the haematoma and malformation because of progressive neurologic deficit. Two patients with acute brain stem infarctions were examined angiographically and with the transcranial Doppler. One patient had a patent basilar artery but worsened clinically. The second patient had a thrombosis of the left vertebral artery which was dislodged with the angiographic catheter. Thrombolytic therapy was initiated. Serial transcranial Doppler examinations showed persistent patency of the vertebrobasilar system although the patient failed to improve. The usefulness of the transcranial Doppler in the management of acute stroke is discussed in the context of these patients.

Six patients who manifested acquired pendular oscillation (APO) after brain-stem hemorrhage were examined with electro-oculograph. The characteristics of APO in these 6 cases were compatible with those reported in the literature as the syndrome of so-called 'rhythmic palato-ocular myoclonus'. The characteristics of APO of our cases did not differ by the presence or absence or rhythmic involuntary oscillation of the palate. We believe that our initial experience establishes the fact that ABRs can be routinely and reliably performed in an operating room environment. There was no added risk to the patient, and operative delays were minimal. We did note transient fluctuation in latency values up to 1.5 msec. A characteristic pattern of ABR was observed in hyper-and hypofunction of the thyroid gland. The most conspicuous change took place in the BSCT. Its value correlated well with the level of serum T4. Dynamic changes of ABR pattern were recorded during the development of myxedema and on return to an euthyroid state. This preliminary study suggests that examinations of ABR may serve as a tool in the overall assessment of thyroid status.

Fifty-eight patients underwent unilateral vertebral arterial reconstructions over a 16-year period. Thirty-four underwent carotid operations as well. The first 18 patients underwent vertebral arterial reconstructions in conjunction with carotid endarterectomy as mandated in the Joint Study of Extracranial Arterial Occlusion as a Cause of Stoke. The simultaneous occurrence of brain stem glioma and disseminated Strongyloides stercoralis infestation in an Ecuadorian male is reported. The unusual finding of Strongyloides larvae in the potassium hydroxide preparation for fungi and in the Gram stain for bacteria from a sputum during hemoptysis fortuitously established the diagnosis of disseminated strongyloidiasis. Of interest was the demonstration of various stages of Strongyloides in the sputum during treatment. Strongyloides infestation must be assiduously searched for in susceptible patients who are, or who are to be, immunosuppressed.

Case histories are reported of 18 patients in whom the diagnosis of primary brain stem haematoma and brain stem haemorrhage was made by computed tomography (CT). The possibility of an early diagnosis of brain stem haematoma, and the differential diagnosis of brain stem haemorrhage by a combination of clinical and tomodensitometric criteria is emphasized. It is concluded that CT is a highly reliable method for the diagnosis, location, and management of brain stem haematoma.

A patient with transient global amnesia also had transient bilateral gaze nystagmus which was detected both by conventional bedside examination and upon electronystagmographic recording. The nystagmus was absent one week later indicating recovery of the temporary brain stem deficit. The recording of objective evidence of brain stem dysfunction in the form of gaze nystagmus in a patient who had transient global amnesia, suggests that both were due to a transient ischemic attack involving the cerebral blood supply in the vertebrobasilar distribution.
