Candidemia among Iranian Patients with Severe COVID-19 Admitted to ICUs.

As a novel risk factor, COVID-19 has led to an increase in the incidence of candidemia and an elevated mortality rate. Despite being of clinical importance, there is a lack of data regarding COVID-19-associated candidemia (CAC) among Iranian patients. Therefore, in this retrospective study, we assessed CAC epidemiology in the intensive care units (ICUs) of two COVID-19 centers in Mashhad, Iran, from early November 2020 to late January 2021. Yeast isolates from patients' blood were identified by 21-plex polymerase chain reaction (PCR) and sequencing, then subjected to antifungal susceptibility …

Candidemia from an upper urinary tract infection complicated by candida endophthalmitis.

A 51-year-old Japanese woman developed candidemia as an outpatient secondary to a Candida albicans upper urinary tract infection complicated by previously undiagnosed type 2 diabetes mellitus with poor glycemic control and ureterolithiasis. The patient did not have any risk factors typically associated with candidemia, such as an indwelling vascular catheter, parenteral nutrition or broad-spectrum antibiotic use. During the clinical course, her condition was complicated by unilateral candida endophthalmitis, which progressed despite the administration of systemic antifungal agents and ultimate…

High rate of non-albicans candidemia in Brazilian tertiary care hospitals.

In order to evaluate the epidemiology of candidemia in Brazil, we performed a prospective multicenter study conducted in six general hospitals from São Paulo and Rio de Janeiro, We enrolled a total of 145 candidemic patients (85 males) with a median age of 32 years. Non-albicans species accounted for 63% of all episodes and the species most frequently causing candidemia were C. albicans (37%), C. parapsilosis (25%), C. tropicalis (24%), C. rugosa (5%), and C. glabrata (4%). Systemic azoles were used before the onset of candidemia in only six patients. There were no differences in the coexistin…

Risk factors for hospital-acquired candidemia. A matched case-control study.

Nosocomial candidemia has become an important infection not only because of an apparently increasing incidence but also because of its high fatality rate. We studied 28 risk factors through a matched case-control study that included 88 pairs of patients hospitalized between July 1983 and December 1986. The strongest single risk factor found in the univariate analysis was the number of prior antibiotics administered: the exposure odds ratio (OR) was 12.50 when patients who received three to five antibiotics were compared with those who received none to two antibiotics. A multiple logistic regre…

LAMP-Based Detection of Candida albicans: A Potential Tool for Candidemia Diagnosis.

Candidemia is a life-threatening bloodstream infection, most commonly caused by Candida albicans (C. albicans), and remains a significant concern in immunocompromised patients. Rapid and accurate species-level detection is essential for effective clinical management. Loop-mediated isothermal amplification (LAMP) is a robust nucleic acid amplification method that enables rapid pathogen detection under isothermal conditions. This study developed and evaluated an ITS2-targeted LAMP assay for the detection of C. albicans in human blood. Analytical sensitivity and specificity were assessed using te…

Development and validation of a modified SOFA score for mortality prediction in candidemia patients.

Candidemia is a life-threatening bloodstream infection associated with high mortality rates, particularly in critically ill patients. Accurate risk stratification is crucial for timely intervention and could improve patient outcomes. This study aimed to enhance the predictive performance of the sequential organ failure assessment (SOFA) score by developing a modified SOFA (mSOFA) score, which is specifically designed for candidemia patients. Using data from MIMIC-III, MIMIC-IV, and ICU-JN databases, we identified key prognostic variables through LASSO regression and integrated into the mSOFA_3…

Candidemia caused by Candida haemulonii: a case report and literature review in neonates.

Candidemia poses a significant challenge for hospitalized neonates with an increase in morbidity and mortality. However, candidemia caused by Candida haemulonii in newborns is rare but fatal. We report such a case in China and performed a literature review. A neonate with a gestational age of 31+6 weeks and a birth weight of 1,420g was diagnosed with C. haemulonii candidemia. The infectious agent was identified by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry and sequencing of the internal transcribed spacer region. The in vitro antifungal susceptibility testing …
