Cardiovascular function in Omani children with sickle cell anaemia.

Systolic murmurs were detected in 22 (61%) of the 36 children with sickle cell anaemia (SCA) who completed the study. Cardiomegaly was detected in 14 (39%). Mean values of left and right ventricular dimensions were higher in SCA than in controls (p < 0.05). Left atrial chambers and aortic root dimensions followed the same pattern. The dilated cardiac chambers in SCA were not associated with any abnormality in systolic or diastolic left ventricular function nor with significant pulmonary hypertension.

A successful case of percutaneous fistula closure in a patient with high output heart failure and extracardiac arteriovenous fistula.

Intra-cardiac shunt diseases may cause chronic ventricular volume overload, but extra-cardiac fistula could also cause high-output heart failure (HF). A patient presented with high-output HF and significant extra-cardiac shunt flow. Although the size and shape of the patient's left ventricle suggested dilated cardiomyopathy, considerable origins were not identified except for a high-flow fistula between the right subclavian artery and right internal jugular vein. Right heart catheter examination revealed inappropriately high cardiac output. Left-to-right shunt ratio was calculated at 40.3% fro

Effect of proximal artery restriction on flow reduction and cardiac function in hemodialysis patients with high-flow arteriovenous fistulas.

OBJECTIVE: Arteriovenous fistula is the preferred vascular access for hemodialysis patients. High-flow arteriovenous fistula may cause high-output heart failure. Various procedures are used to reduce high-flow arteriovenous fistula. This study aimed to assess the efficacy of proximal artery restriction combined with distal artery ligation on flow reduction for high-flow arteriovenous fistula and on cardiac function and echocardiographic changes in patients undergoing hemodialysis. METHODS: A retrospective analysis was performed on data collected from the medical records of patients undergoing

Atrial Flutter With Intraventricular Conduction Delay, Hypotension, and Bradycardia in the Setting of High-Output Ileostomy With Renal Failure, Hyperkalemia, and Metabolic Acidosis: A Case Report With Brief Literature Review.

After ostomy creation, some patients tend to develop high outputs from their ostomy site, which puts them at risk for volume depletion, renal failure, and electrolyte and acid-base abnormalities. This case emphasizes the need for a multidisciplinary team-based approach for the care of patients after ostomy creation, especially those with persistent high output and significant cardiac comorbidities, including cardiac arrhythmias. Patient education and subsequent follow-ups and assessments are important to prevent life-threatening complications in these patients.

Oxygen transport during extracorporeal oxygenation for the treatment of adult respiratory distress syndrome.

An inventory of hemodynamic and respiratory measurements was obtained in two patients with acute respiratory failure syndrome prior to and during venoarterial extracorporeal membrane oxygenation for study of oxygen transport. As the inspired oxygen concentration fraction was reduced from 1.0 to 0.5 and paO2 was increased from less than 50 to physiologic ranges during extracorporeal membrane oxygenation, tachycardia and pulmonary hypertension were reversed. Total oxygen transport was reduced rather than increased. Oxygen consumption and oxygen extraction were not altered during extracorporeal m

Recurrence of hypertension in primary aldosteronism after discontinuation of spironolactone. Time course of changes in cardiac output and body fluid volumes.

Hemodynamic mechanisms underlying the hypertensive action of long-standing sodium overload were studied in ten subjects with aldosterone-producing adenoma. Arterial pressure was 129+/-4/91+/-4 mm Hg (mean+/-SEM) during spironolactone treatment, 177+/-5/114+/-3 mm Hg six weeks after the drug had been stopped, and 136+/-4/94+/-3 mm Hg two months after operation. In the six weeks following cessation of spironolactone, cardiac output, stroke volume, blood volume and sodium space were determined at weekly intervals. In each subject the biochemical syndrome of aldosterone-excess had been fully devel

Effect of the mode of administration of inhaled anaesthetics on the interpretation of the F(A)/F(I) curve--a GasMan simulation.

The effects of blood solubility, cardiac output and ventilation on the rise of the alveolar towards the inspired concentration, the F(A)/F(I) curve, of an inhaled anaesthetic are often thought to reflect how these factors affect wash-in of the central nervous system compartment and, therefore, speed of induction because F(A) is the partial pressure ultimately attained in the central nervous system (F(VRG)). These classical F(A)/F(I) curves assumed a constant F(I). We used GasMan to examine whether changes in solubility, cardiac output and ventilation affect the relationship between the F(A)/F(

[Pediatric shock--pathophysiology, diagnosis and treatment].

Shock remains a significant cause of morbidity and potential mortality in the pediatric population. It is defined as a impaired perfusion with a too high oxygen demand in comparison to oxygen delivery. The cause of hypoxia may be found in a hypovolemic condition due to hemorrhage or loss of gastro-intestinal fluids, a disorder in volume distribution or a cardiac dysfunction. Less frequent are patients with an obstruction in the outflow tract of the heart or disorders in binding oxygen to hemoglobin. Hypoxia leads to lactat-acidosis with the clinical signs of tachypnoea, tachycardia and restles

Myocardial Strain and Cardiac Output are Preferable Measurements for Cardiac Dysfunction and Can Predict Mortality in Septic Mice.

Background Sepsis is the overwhelming host response to infection leading to shock and multiple organ dysfunction. Cardiovascular complications greatly increase sepsis-associated mortality. Although murine models are routinely used for preclinical studies, the benefit of using genetically engineered mice in sepsis is countered by discrepancies between human and mouse sepsis pathophysiology. Therefore, recent guidelines have called for standardization of preclinical methods to document organ dysfunction. We investigated the course of cardiac dysfunction and myocardial load in different mouse mod

[Modifications of the left ventricular function as the result of severe and chronic reduction of the preload].

In order to evaluate the effect which the cronic, severe, and constant decrease of the preload of the left ventricle has on the contractile function of this chamber, 15 cases, with the strict technical conditions and permitted the planned measurements, were taken from 86 studies of ventricular function in patients with tight mitral stenosis. The following parameters were calculated; the ejection fraction, the maximum DP/dt, the zero point diastolic pressure, the final diastolic pressure, the diastolic AP, the mid-diastolic pressure, the diastolic compliance index, and the passive elastic modul

Successful propranolol therapy for infantile hepatic hemangioma with cardiac failure.

Infantile hepatic hemangioma (IHH) can cause severe complications such as high-output cardiac failure and growth faltering, requiring prompt diagnosis and intervention. In the absence of standardized treatment guidelines, propranolol has emerged as an effective and well-tolerated therapy. We report a female infant with multifocal IHH who presented with high-output cardiac failure, hepatomegaly, and growth faltering. She experienced rapid and sustained clinical as well as radiologic improvement with propranolol therapy. This case reinforces propranolol's role as a safe, effective first-line the

Systemic hemodynamic and microvascular responses in spontaneously hypertensive rats during Escherichia coli bacteremia.

Renovascular hypertension profoundly alters skeletal muscle arteriolar responses to sepsis, yet systemic hemodynamics to sepsis are not affected by hypertension. In this study, we hypothesized that microvascular responses of skeletal muscle and systemic hemodynamics are changed during high- and low-cardiac-output Escherichia coli bacteremia in normotensive Wistar-Kyoto (WKY) and spontaneously hypertensive rats (SHR). During high-cardiac-output bacteremia, blood pressure and heart rate increased in WKY, but blood pressure decreased in SHR. During low-cardiac-output bacteremia, blood pressure in

Echocardiographic evaluation of fetal cardiac function: clinical and anatomical correlations in two cases of endocardial fibroelastosis.

BACKGROUND: Two fetuses with endocardial fibroelastosis, one with critical aortic stenosis and one with high-output cardiac failure due to chorioangiomatosis, are presented to evaluate the correlation between Doppler echocardiographic findings, the fetal clinical condition and the anatomical substrate found at postmortem. METHODS: Doppler measurements of cardiac function (systolic, diastolic and global) and a cardiovascular score incorporating five parameters of fetal well-being were recorded. RESULTS: In the fetus with critical aortic stenosis, the cardiovascular score was diminished, there

Hemodynamic and cardiac adaptation in essential hypertension. Consequences for therapy.

In so-called essential hypertension, at least three different subsets of pathophysiologic findings can be identified: mild hypertension in nonobese juvenile subjects characterized by elevated cardiac output, normal peripheral resistance, increased sympathetic activity as measured by norepinephrine and plasma renin activity; hypertension in the elderly with a low cardiac output, often with left ventricular hypertrophy, elevated total peripheral resistance, nephrosclerosis contracted intravascular volume, and low plasma renin activity; and obese hypertensive patients with a high cardiac output,

High-Output Heart Failure and Pulmonary Hypertension Caused by Arteriovenous Graft: Diagnosis and Treatment.

BACKGROUND: Surgical arteriovenous (AV) fistulas provide vascular access for hemodialysis in patients with end-stage renal disease, but this low-resistance pathway can lead to high-flow circulation and cardiac decompensation. CASE SUMMARY: A 42-year-old woman with end-stage renal disease from chronic glomerulonephritis and a failed renal transplant resumed hemodialysis via a 22-year-old left-arm brachiocephalic AV graft. She developed high-output heart failure. We ligated the AV graft and placed a peritoneal dialysis catheter. Hemodynamic parameters were normal 6 months postligation. DISCUSS

Hemodynamic response to positive end-expiratory pressure and prone position in COVID-19 ARDS.

BACKGROUND: Use of high positive end-expiratory pressure (PEEP) and prone positioning is common in patients with COVID-19-induced acute respiratory failure. Few data clarify the hemodynamic effects of these interventions in this specific condition. We performed a physiologic study to assess the hemodynamic effects of PEEP and prone position during COVID-19 respiratory failure. METHODS: Nine adult patients mechanically ventilated due to COVID-19 infection and fulfilling moderate-to-severe ARDS criteria were studied. Respiratory mechanics, gas exchange, cardiac output, oxygen consumption, syste

High-output heart failure caused by a large renal arterio-venous (AV) fistula.

A man in his early 70s presented with heart failure syndrome. The clinical diagnosis was possible constrictive pericarditis. However, at cardiac catheterisation, there was a large step-up in oxygen saturation in the inferior vena cava between the mid-abdomen and entry into the heart. Abdominal imaging showed a large renal arterio-venous fistula, which was subsequently embolised, resulting in the resolution of the patient's symptoms.

[An elderly woman with macrocytic anemia and acute high-output heart failure following acute bleeding due to a gastric ulcer].

Although macrocytic anemia can develop in patients with acute blood loss, such anemia in very old patients is uncommon. In this report, we describe the course of an 89-year-old woman who had a rapid recovery from macrocytic anemia by medication only after acute blood loss due to a gastric ulcer. She had been treated with antihypertensive drugs for the previous 28 years at our outpatient clinic, and was admitted because of acute anemia 6 days after she had experienced tarry stool. Her hemoglobin (Hb) count and mean corpuscular volume (MCV) were 8.4 g/dl, and 103 fl, respectively. A gastroscopic

High-output heart failure in mitochondrial myopathy: a fulminant form with severe lactic acidosis and rhabdomyolysis.

A 17-year-old male with mitochondrial myopathy was transferred to our hospital because of respiratory distress and a rapidly deteriorating level of consciousness. He had a markedly increased cardiac output with low systemic vascular resistance and decreased arteriovenous oxygen difference. Severe lactic acidosis, rhabdomyolysis and acute renal failure were also present. He was treated in the intensive care unit with continuous hemofiltration and hemodialysis. This case demonstrates that cardiac failure in mitochondrial encephalomyopathy may be caused by myocardial abnormalities as well as by r
