Ceftazidime versus imipenem-cilastatin as initial monotherapy for febrile neutropenic patients.

One hundred febrile episodes in 89 neutropenic patients after cytotoxic chemotherapy were randomized to be treated with either ceftazidime or imipenem as initial monotherapy. The clinical characteristics of the two groups of patients were comparable. The response of the fever in patients who received imipenem was significantly better than that in those who received ceftazidime (77 versus 56%, respectively; P = 0.04), especially in those with microbiologically documented infection (81 versus 33%, respectively; P = 0.02). The in vitro susceptibilities and the clinical responses suggested that, w…

Liposomal tobramycin and ceftazidime as advanced nanocarriers against Pseudomonas aeruginosa infections.

The worrisome decline of antibiotic efficacy against Pseudomonas aeruginosa infections emphasizes the urgent need for new antibiotic delivery techniques. Liposomal formulations of tobramycin and ceftazidime were developedand comprehensively studied utilizing an active loading approach. Both formulations showed nanoscale size (<120 nm), low polydispersity index (<0.3), and spherical morphology, as validated by TEM examination. Tobramycin and ceftazidime encapsulation efficiency was in the rank of 20 %. Drug entrapment exploits both temperature change and electrostatic interaction between the ch…

Klebsiella pneumoniae Carbapenemase Variants Resistant to Ceftazidime-Avibactam: an Evolutionary Overview.

First variants of the Klebsiella pneumoniae carbapenemase (KPC), KPC-2 and KPC-3, have encountered a worldwide success, particularly in K. pneumoniae isolates. These beta-lactamases conferred resistance to most beta-lactams including carbapenems but remained susceptible to new beta-lactam/beta-lactamase inhibitors, such as ceftazidime-avibactam. After the marketing of ceftazidime-avibactam, numerous variants of KPC resistant to this association have been described among isolates recovered from clinical samples or derived from experimental studies. In KPC variants resistant to ceftazidime-aviba…

Rapid Desensitization after a Type I Hypersensitivity Reaction to Ceftazidime/ Avibactam.

BACKGROUND: Frequent antibiotic use increases the risk of multidrug-resistant pathogen formation and hypersensitivity to antibiotics, including Type I hypersensitivity reactions. PRESENTATION: We present a case of the first successful induction of tolerance procedure for the antibiotic ceftazidime/ avibactam. The patient developed immediate generalized urticaria and flushing on his first dose of ceftazidime/avibactam. He was able to tolerate a 12-step desensitization procedure that resulted in the clearance of his infection. CONCLUSIONS: Drug desensitization procedures are useful for patient…

Burkholderia pseudomallei acquired ceftazidime resistance due to gene duplication and amplification.

Ceftazidime (CAZ) is the antibiotic of choice for the treatment of Burkholderia pseudomallei infection (melioidosis). The chromosomally-encoded PenA β-lactamase possesses weak cephalosporinase activity. The wild-type penA gene confers clinically significant CAZ resistance only when overexpressed due to a promoter mutation, transcriptional antitermination or by gene duplication and amplification (GDA). Here we characterise a reversible 33-kb GDA event involving wild-type penA in a CAZ-resistant B. pseudomallei clinical isolate from Thailand. We show that duplication arises from exchanges betwee…

Successful Treatment of Persistent Burkholderia cepacia Complex Bacteremia with Ceftazidime-Avibactam.

We report our clinical experience treating a 2-month-old infant with congenital diaphragmatic hernia who experienced prolonged bacteremia with Burkholderia cepacia complex (Bcc) despite conventional antibiotic therapy and appropriate source control measures. The infection resolved after initiation of ceftazidime-avibactam. Whole-genome sequencing revealed that the isolate most closely resembled B. contaminans and identified the mechanism of resistance that likely contributed to clinical cure with this agent. Ceftazidime-avibactam should be considered salvage therapy for Bcc infections if other…

The First Case of Ceftazidime-Induced Antibiomania: A Case Report and Literature Review.

Antibiomania is a rare type of secondary mania caused by antibiotics. Here, we describe the first case report of antibiomania in a peritoneal dialysis patient caused by ceftazidime, a cephalosporin antibiotic. Manic symptoms were noted in the patient described here a few days after starting the antibiotic and worsened over the course of a few weeks of antibiotic treatment. There is a close temporal relationship between the start of antibiotics and the onset of manic symptoms in this case. Antibiotics from various groups have been implicated as the causative agents in a few case reports of anti…

A Selective Medium for Screening Ceftazidime/Avibactam Resistance in Carbapenem-Resistant Enterobacterales.

Ceftazidime/avibactam (CZA) is an alternative antibiotic used for the treatment of infections caused by carbapenem-resistant Enterobacterales (CRE). However, the CZA-resistant CRE strains have been detected worldwide. Therefore, it is critical to screen CZA-resistant CRE strains in colonized patients or a specific population so as to rapidly implement infection control measures to limit their transmission. In this study, we developed a Salmonella-Shigella (SS) CZA-selective medium and assessed its performance to screen for clinical CZA-resistant CRE isolates in both pure-strain specimens and s…

Bifunctional nano-Ag3PO4 with capabilities of enhancing ceftazidime for sterilization and removing residues.

Since the efficacy of antibiotics towards bacteria is decreasing over time, the rising of antibiotic emission has become an increasingly grave issue. In this study, we proposed an integrated antibacterial nanotechnology without pollution residues, which synergistically enhances the antibacterial activity of ceftazidime by using the inorganic nano-Ag3PO4, and subsequently removes drug residues by photocatalysis. Ag3PO4 were synthesized using a simple ion-exchange method without any reducing agent or protectant. The combined antibacterial activity of Ag3PO4 and 22 kinds of antibiotics against Es…

Population pharmacokinetics of ceftazidime after a single intramuscular injection in wild turtles.

Ceftazidime, a third-generation cephalosporin, is important for treating opportunistic bacterial infections in turtles. Antibacterial dosage regimens are not well established for wild turtles and are often extrapolated from other reptiles or mammals. This investigation used a population pharmacokinetic approach to study ceftazidime in wild turtles presented for rehabilitation. Ceftazidime was administered to 24 wild turtles presented to the Turtle Rescue Team at North Carolina State University. A sparse blood sampling protocol was used to collect samples from 0 to 120 hr with three samples per…

In vitro selection of ceftazidime-avibactam resistance in Enterobacteriaceae with KPC-3 carbapenemase.

Ceftazidime-avibactam is active against most Enterobacteriaceae isolates with KPC carbapenemases. We investigated whether this activity could be compromised by mutation. Single-step and multistep selections were attempted using ceftazidime-avibactam (avibactam fixed at 1 or 4 μg/ml) versus two strains each of Enterobacter cloacae and Klebsiella pneumoniae, all with the KPC-3 enzyme. Mutant bla KPC alleles were sequenced, and their parentage was confirmed by typing. Ceftazidime-avibactam selected mutants at up to 16× MIC, with frequencies of ca. 10(-9). This contrasted with previous experience …
