Noninvasive biomarkers in heart transplant: 2020-2021 year in review.

PURPOSE OF REVIEW: Endomyocardial biopsy (EMB), the current gold standard for cardiac allograft monitoring is invasive, may have a low sensitivity and is associated with significant variability in histopathologic interpretation. Fortunately, on-going research is identifying noninvasive biomarkers that address some of these limitations. This review provides an update on noninvasive blood-based methods for rejection surveillance and diagnosis in heart transplantation. RECENT FINDINGS: Recent studies highlight good test performance to detect acute rejection for donor-derived cell-free DNA (dd-cf

Detection of Klebsiella pneumoniae cfDNA in pleural fluid and its clinical value.

BACKGROUND: Klebsiella pneumoniae (KP) is an important opportunistic pathogen that can easily cause pneumonia and pleural effusion when body resistance is reduced. However, the positive rate of KP detected from clinical pleural effusion by traditional methods, including bacterial culture, is meager. Therefore, new detection methods are urgently needed to improve the positive detection rate of KP and other bacteria in pleural effusion. METHODS: Simulated pleural fluid of KP infection was first set up. Then circulating cell-free DNA (cfDNA) was extracted from cultured hydrothorax and detected b

High levels of dd-cfDNA identify patients with TCMR 1A and borderline allograft rejection at elevated risk of graft injury.

The clinical importance of subclinical, early T cell-mediated rejection (Banff TCMR 1A and borderline lesions) remains unclear, due, in part to the fact that histologic lesions used to characterize early TCMR can be nonspecific. Donor-derived cell-free DNA (dd-cfDNA) is an important molecular marker of active graft injury. Over a study period from June 2017 to May 2019, we assessed clinical outcomes in 79 patients diagnosed with TCMR 1A/borderline rejection across 11 US centers with a simultaneous measurement of dd-cfDNA. Forty-two patients had elevated dd-cfDNA (≥0.5%) and 37 patients had low

Exosomes-Associated DNA-New Marker in Pregnancy Complications?

Despite a large number of studies, the etiology of pregnancy complications remains unknown. The involvement of cell-free DNA or fetal cell-free DNA in the pathogenesis of pregnancy complications is currently being hypothesized. Cell-free DNA occurs in different forms-free; part of neutrophil extracellular traps; or as recently discovered, carried by extracellular vesicles. Cell-free DNA is believed to activate an inflammatory pathway, which could possibly cause pregnancy complications. It could be hypothesized that DNA in its free form could be easily degraded by nucleases to prevent the infla

Promising Blood-Based Biomarkers for Melanoma: Recent Progress of Liquid Biopsy and Its Future Perspectives.

Because the recent success of novel therapeutic approaches has dramatically changed the clinical management of melanoma, less invasive and repeatable monitoring tools that can predict the disease status, drug resistance, and the development of side effects are increasingly needed. As liquid biopsy has enabled us to diagnose and monitor disease status less invasively, substantial attention has been directed toward this technique, which is gaining importance as a diagnostic and/or prognostic tool. It is evident that microRNA, cell-free DNA, and circulating tumor cells obtained via liquid biopsy

Liquid biopsy in lung cancer management.

Liquid biopsy is a promising tool for a better cancer management and currently opens perspectives for several clinical applications, such as detection of mutations when the analysis from tissue is not available, monitoring tumor mutational burden and prediction of targeted therapy response. These characteristics validate liquid biopsy analysis as a strong cancer biomarkers source with high potential for improving cancer patient's evolution. Compared to classical biopsy, liquid biopsy is a minimal invasive procedure, and it allows the real-time monitoring of treatment response. Considering that

Caffeine dissociates complexes between DNA and intercalating dyes: application for bleaching fluorochrome-stained cells for their subsequent restaining and analysis by laser scanning cytometry.

BACKGROUND: Removal of the nucleic acid-bound fluorochrome is desirable when stained cells have to be reanalyzed using other fluorochromes. It is also often desirable to remove DNA-bound antitumor drugs from drug-treated cells, to improve cell staining. We have previously observed that in aqueous solutions, the methylxanthine caffeine (CFN) decreases interactions between planar aromatic molecules such as intercalating dyes or antitumor drugs and nucleic acids. The aim of this study was to explore whether this property of CFN can be utilized to remove the DNA-bound intercalating dyes propidium

In Situ Reactivity of Electrochemically Generated Nitro Radical Anion on Tinidazole and Its Monomeric and Dimeric CuII Complexes on Model Biological Targets with Relative Manifestation of Preventing Bacterial Biofilm Formation.

Formation of nitro radical anion (-NO2 •-) and other reduction products of 5-nitroimidazoles, although important for antimicrobial activity, makes the drugs neurotoxic. Hence, an appropriate generation and their role in the free radical pathway needs proper realization. This was attempted by studying the action of tinidazole and its CuII complexes on model targets (nucleic acid bases and calf thymus DNA). Results obtained were correlated with studies on biological species where prevention of biofilm formation on Staphylococcus aureus and Pseudomonas aeruginosa was followed. Tinidazole and its

Precise Mutation Detection in Low-Abundance Tumor-Derived Extracellular Vesicles via Microfluidic Immunomagnetic Reversible Affinity Capture.

Molecular diagnostic technologies based on liquid biopsy are increasingly vital for clinical tumor molecular profiling. Compared to highly fragmented and easily degradable cell-free DNA (cfDNA), extracellular vesicles (EVs) offer distinct advantages by protecting nucleic acids from degradation. Enrichment of tumor-derived EVs using specific surface markers significantly enhances the relative abundance of mutant nucleic acids, establishing a foundation for highly sensitive mutation detection. To fully leverage this enrichment strategy, we developed the MIRACLE platform, an integrated system com

Air pollution-linked epigenetic modifications in placental DNA: Prognostic potential for identifying future foetal anomalies.

Prenatal exposure to air pollution is a significant risk factor for the mother and the developing foetus. The accumulation of pollutants in the placenta can cause a self-cascade loop of pro-inflammatory cytokine responses and DNA double-strand breaks. Previous research has shown that airborne particulate matter can damage the epigenome and disturb mitochondrial machinery, ultimately impairing placental function. Mitochondria are essential for preserving cellular homeostasis, energy metabolism, redox equilibrium, and epigenetic reprogramming. As these organelles are subtle targets of environmen

Sustainable polysaccharide-poly(2-oxazoline) gene-delivery platforms enhanced by free polyethyleneimine chains.

Developing safe and efficient non-viral vectors remains a major challenge in nucleic acid delivery, particularly for applications that demand minimal cytotoxicity, including ocular therapies. Here, we introduce a series of hybrid gene-delivery platforms based on permanently cationic polysaccharides (dextran and chitosan) grafted with poly(2-oxazoline) (POx) and supplemented by small amounts of free branched polyethyleneimine (BPEI). The introduction of permanent positive charges onto dextran and chitosan backbones enabled strong DNA binding and polymer-DNA complex formation, while grafted poly

Extracellular vesicles: A potential future strategy for dental and maxillofacial tissue repair and regeneration.

Extracellular vesicles (EVs), nano-sized bilayer membrane structures containing lipids, proteins and nucleic acids, play key roles in intercellular communication. Compared to stem cells, EVs have lower tumorigenicity and immunogenicity, are easier to manage and cause fewer ethic problems. In recent years, EVs have emerged as a potential solution for tissue regeneration in stomatology through cell-free therapies. The present review focuses on the role of EVs in dental and maxillofacial tissue repair and regeneration, including in dental and periodontal tissue, maxilla and mandible bone, temporo

Decoding the Follicular Fluid Microenvironment: Integrative Biomarkers and Multiomics Signatures to Predict IVF Outcomes.

BACKGROUND: With the increase in the number of patients with infertility, the number of patients requiring in vitro fertilization and embryo transfer (IVF-ET) is also increasing. No established method exists for predicting pregnancy rates under IVF-ET, and the urgent development of pregnancy predictive biomarkers is required. Follicular fluid (FF) can be collected noninvasively during oocyte retrieval and contains numerous substances indicative of follicular status, making it a strong candidate for such biomarkers. METHODS: This review summarizes the predictive potential of several biomarkers

Mesenchymal stem cell-based cell-free strategies: safe and effective treatments for liver injury.

Various hepatoxic factors, such as viruses, drugs, lipid deposition, and autoimmune responses, induce acute or chronic liver injury, and 3.5% of all worldwide deaths result from liver cirrhosis, liver failure, or hepatocellular carcinoma. Liver transplantation is currently limited by few liver donors, expensive surgical costs, and severe immune rejection. Cell therapy, including hepatocyte transplantation and stem cell transplantation, has recently become an attractive option to reduce the overall need for liver transplantation and reduce the wait time for patients. Recent studies showed that

Blood circulating cell-free mitochondrial DNA as a potential biomarker for major depressive disorder: a meta-analysis.

BACKGROUND: Mitochondrial dysfunction has been implicated in major depressive disorder (MDD), but reliable, measurable biomarkers remain elusive. As a minimally invasive and quantifiable biomarker, circulating cell-free mitochondrial DNA (ccf-mtDNA) in blood offers potential for objective assessment of mitochondrial stress in MDD. However, evidence linking regarding the association between ccf-mtDNA levels and MDD is limited and inconsistent. METHODS: We systematically searched eight databases, including PubMed, EMBASE, and major Chinese repositories. Thirteen studies with 1370 participants (

Cell-free DNA in female infertility: from pathological mechanisms to clinical biomarkers.

Female infertility, accounting for 40–50% of infertility cases globally, underscores the urgent need for non-invasive biomarkers to guide early intervention. Cell-free DNA (cfDNA)—released via apoptosis, necrosis, or active secretion—has emerged as a dynamic molecular reflector of tissue pathophysiology. This review synthesizes recent advances in cfDNA biology and their clinical applications in female infertility. Mechanistically, cfDNA drives inflammation, oxidative stress, and neutrophil extracellular traps (NETs), exacerbating dysfunction in polycystic ovary syndrome (PCOS), endometriosis (
