Epidemiology and management of atlas fractures.

PURPOSE: The purpose of this study was to gain new insights into the epidemiologic characteristics of patients with atlas fractures and to retrospectively evaluate complication rates after surgical and non-surgical treatment. METHODS: In a retrospective study, consecutive patients diagnosed with a fracture of the atlas between 01/2008 and 07/2018 were analyzed. Data on epidemiology, concomitant injuries, fracture patterns and complications were obtained by chart and imaging review. RESULTS: In total, 189 patients (mean age 72 years, SD 19; 57.1% male) were treated. The most frequent trauma m

Occipitalization of the atlas.

This paper discusses occipitalization of the atlas and its relationship to the chiropractic practitioner. Patients commonly consult a chiropractor with complaint of headache, suboccipital stiffness, restricted motion, dizziness and other symptoms related to the upper cervical region. Differential diagnosis of the exact etiological factor of these symptoms must be made via a thorough history, physical examination, and roentgenological examination. If occipitalization of the atlas is detected on the initial roentgenological examination, then follow-up magnetic resonance imaging, computerized tom

Spontaneous atlantoaxial subluxation as a presenting manifestation of juvenile ankylosing spondylitis. A case report.

Juvenile-onset ankylosing spondylitis is an unusual disorder which can present with either peripheral arthritis or with more classic hip girdle and back symptoms. Atlantoaxial instability has been recognized as a late complication in only one patient and has not been reported as an early manifestation. This paper presents a case in which atlantoaxial instability and neck pain without neurologic involvement was a presenting manifestation. The laboratory and clinical features of juvenile ankylosing spondylitis are discussed, along with the importance of qualitative sacroiliac joint scintigraphy

[Frequency of Ponticulus posticus and Ponticulus lateralis in patients admitted to hospital with traumatic brain injur].

OBJECTIVE: To determine the frequency of Ponticulus posticus and Ponticulus lateralis among patients admitted to a24-hour emergency hospital. MATERIAL AND METHODS: The study included 2887 patients with a traumatic history who were admitted to the hospital for emergency reasons. This principle of sampling allowed us to exclude a high concentration of studies of patients with chronic neurological diseases, the cause of which could be symptomatic Kimmerle's anomaly (KA). During the period 01.01.20 to 31.12.20, primary CT was performed on an emergency basis in 7348 patients. Of these, CT scans of

[Partial aplasia of the atlas in a child].

Congenital anomalies of the atlas are rare but well documented. They can range from total agenesis to partial clefts of the posterior and/or anterior arches and are mostly incidental findings after a sustained trauma. It is essential to differentiate these congenital variants from fractures of the atlas to prevent the patient from unnecessary (neuro-) surgical intervention. In this article the case of a 10-year-old girl is described with an unusual finding of partial aplasia of the posterior arch. The clinical features and diagnostic and therapeutic procedures are discussed.

Isolated avulsion fracture of the anterior tubercle of the atlas.

Avulsion fracture of the tubercle of the atlas is a rare injury which is probably frequently overlooked. The fracture results from hyperextension of the neck against contraction of the long flexors. We describe two different fracture patterns and discuss the anatomical implications thereof.

Jefferson fracture: presentation of a new diagnostic sign.

A case of a Jefferson fracture is presented. A new diagnostic sign, which is viewed on a lateral radiograph and consists of interposition of the basion between the anterior arch of C-1 and the odontoid process, is described. This sign may aid in the rapid identification of Jefferson fractures.

[Unusual trauma of atlas. (Jefferson's double fracture with a fracture of right lateral mass). Clinical note (author's transl)].

Reporting a study of theirs about a Jefferson bilateral fracture combined with a dislocated fracture of the atlas vertebra on its right side, the Authors inquire into the traumathology of the cervical spine giving account of the result study.

Horizontal fracture of the anterior arch of the atlas.

Horizontal fracture of the anterior arch of the atlas can be easily overlooked on lateral radiographs. The fracture is not associated with neurological deficit but may be the cause of severe pain. A review of the literature reveals only 3 previously reported cases. Seven new cases, including 2 without other cervical spine fractures, indicate a much higher incidence than was previously thought. Hypotheses about the mechanism of injury are discussed.
