Systemic anaphylaxis is an acute allergic emergency resulting from generalized mast cell degranulation. In the United States, it is estimated that anaphylaxis accounts for about 500 deaths each year. Hymenoptera-sting hypersensitivity is one of the most common causes of systemic anaphylaxis. Cytochemical examinations of the activity of the lymphocyte and neutrophil acid phosphatase (AP), a lysosomal marker enzyme, was carried out in patients with discoid lupus erythematosus to assess the function of the cellular lysosomal system. The lymphocytic and neutrophilic AP activities were found reduced both at the expense of reduced number of reacting cells and of the enzyme level in these formed elements.

The authors studied 74 pregnant women with preeclampsia presenting various degrees of gravity. The activity of alkaline phosphatase was determined cytochemically in the leucocytes. It was established that the activity of the enzyme was unchanged in mild preeclampsia and diminished progressively with advancement of the pathological process, compared with the respective values of normally progressing pregnancy at the same gestational period. A cytochemical criterium is proposed for differentiating the mild form of preeclampsia form the moderate form of preeclampsia.

The 25-g D-xylose absorption test was assessed in a total of 40 adult patients with coeliac disease before and after treatment and in 25 control patients. Because of a large overlap neither the urinary excretion rates nor the 1-h blood concentrations were of any use in the assessment of the individual patients. They sometimes overlap with other collagen vascular diseases, and not infrequently are associated with malignancy. Plasma cell acid phosphatase has been studied in 51 patients, 30 of whom were affected with multiple myeloma (MM) and 21 with nonmyelomatous monoclonal immunoglobulinemias (NMMI). Scoring of results displayed a highly significant difference between the median of the MM and NMMI group (343 vs. 204). The use of plasma cell acid phosphatase as a further criterion in the differential diagnosis of monoclonal immunoglobulinemiasis emphasized.

A distinctive pattern of alpha-naphthyl butyrate esterase positivity was observed in all of a series of 14 patients with HCL. This pattern was not observed in a range of other haematological malignancies including nine cases of CLL, two cases of Sézary's syndrome, six cases of null cell ALL, two cases of Schilling-type monocytic leukaemia and one case of lymphosarcoma cell leukaemia. The potential diagnostic value of this simple cytochemical test is discussed in relation to existing methods.

Malignant hyperpyrexia is a highly lethal complication of general anesthesia. It occurs with sufficient frequency, particularly in children and young adults, to justify use of continuous temperature monitoring during adminnstration of general anesthesia.
