A 39-year-old woman had been investigated elsewhere due to symptomatic hypokalaemia, renal potassium wasting and metabolic alkalosis. Vomiting was considered to be the underlying cause but the patient repeatedly denied this behaviour. After extensive investigations, Bartter's syndrome was finally diagnosed. Eighteen months later the patient was readmitted due to progressive renal insufficiency. On the basis of a very low urinary chloride excretion and the aforementioned laboratory results, it was concluded that the metabolic disturbances were due to vomiting and the diagnosis of Bartter's synd

INTRODUCTION: The Endocrinology Clinical Skills Assessment Program (ECSAP) was developed at Wake Forest University School of Medicine to enhance history-taking, diagnostic reasoning, and personalized feedback in endocrinology fellows. Recognizing a gap in formative clinical skill evaluation, ECSAP introduces standardized case-based assessments across key endocrine topics.
METHODS: ECSAP sessions occur twice yearly and include four 25-minute standardized clinical scenarios (e.g., adrenal incidentaloma, hypercalcemia). Each case is pre-assigned to faculty, who provide fellows with a one-line pr

PURPOSE: Changes in graduate-level clinical reasoning for clinical swallow evaluation were examined and compared between video simulation and standardized patient simulation training. The purpose of this study was to provide information for speech-language pathology programs to consider when integrating simulation for clinical reasoning for clinical swallow evaluation within the graduate-level curriculum to support intentional, evidence-based use of academic resources for teaching and learning.
METHOD: A randomized pretest/posttest mixed-methods design was implemented to compare a video simul…

INTRODUCTION: The purpose of this study was to identify clinical tests for scaphoid fractures and, using a higher order analysis, to determine their diagnostic accuracy.
METHODS: A literature review of the databases CINAHL, Embase, Medline and PUBMED from 1980 to September 30, 2011 was conducted to obtain applicable literature on clinical tests used in identifying scaphoid fractures. Methodological quality was determined using the criteria for validity suggested by Sackett et al (2000). Using a random effects model, pooled positive likelihood ratios (PPLR) were established for any test evalua

BACKGROUND: Sound clinical reasoning is crucial for synthesizing assessment and contextual information into evidence-based and patient-centred management decisions. Whilst clinical reasoning processes and development have been explored in broader health contexts, to date there has been limited evaluation of how clinical reasoning skills are employed by pre-professional speech-language therapists (SLTs) who conduct clinical swallowing examinations (CSEs).
AIMS: To evaluate how pre-professional SLTs (senior undergraduate students) apply reasoning and decision-making processes during simulated C…

Artificial intelligence (AI) is increasingly used to support diagnosis across medicine, yet extending its success to psychological assessment raises a distinct epistemological problem. Psychological diagnosis requires interpreting subjective experience, contextualizing clinical information, and developing explanatory hypotheses within the clinician-patient relationship. In this Perspective, we compare the inferential structure of medical and psychological diagnosis and argue that psychopathological reasoning relies especially on abduction, namely the generation and iterative revision of plausi

Clinical reasoning is a core competency in health professions education, yet evidence regarding effective teaching and assessment strategies remains fragmented across disciplines. This systematic review synthesized evidence on educational strategies and assessment approaches used to develop and evaluate clinical reasoning in medicine, dentistry, nursing, and physiotherapy. Studies published between January 2000 and November 2025 were identified through systematic searches of PubMed, Scopus, Google Scholar, and manual searches of relevant journals, and were narratively synthesized in accordance…

RATIONALE, AIMS AND OBJECTIVES: Clinical reasoning is often studied as if it occurred inside the clinician's head. Yet, in practice, clinicians think, decide and act through constant interaction with patients, colleagues, technologies, routines and local constraints. This article challenges predominantly individual and decontextualised accounts of clinical reasoning and reframes reasoning as a situated socio-cognitive practice. We aim to show how explicit and tacit knowledge interact with the social, material and organizational environments in which clinical practice unfolds.
METHODS: We deve…

Effective clinical reasoning is central to competent medical practice, yet cultivating this skill remains one of the most challenging goals in medical education. Although large language models (LLMs) create new opportunities, most follow a passive answering paradigm - where users pose queries and models passively respond - failing to support the step-by-step, heuristic instruction essential for clinical reasoning. To bridge this gap, we propose CReaTe-LM, the first language model that effectively provides heuristic and stepwise clinical reasoning instruction. We begin by introducing a unified …

BACKGROUND: Large language models (LLMs) demonstrate strong performance in knowledge-based medical tasks, yet their clinical reasoning capabilities in complex ophthalmic decision-making, particularly glaucoma management, remain insufficiently characterized. Retrieval-augmented generation (RAG) has been proposed as a strategy to improve factual grounding and safety, but its clinical value requires systematic evaluation.
METHODS: We conducted a retrospective, scenario-based comparative evaluation using 40 real-world glaucoma cases spanning primary, secondary, postoperative, and end-stage diseas…

Hairy cell leukemia (HCL) is a rare, indolent B-cell neoplasm that often presents with nonspecific laboratory findings, which can delay diagnosis. We report the case of a 75-year-old male with incidentally detected isolated thrombocytopenia and relative lymphocytosis. Peripheral blood smear showed lymphoid cells with cytoplasmic projections suggestive of hairy cells. Flow cytometry confirmed a dominant clonal population consistent with classical HCL, along with a small CD5+/CD23+ monoclonal B-cell population compatible with a chronic lymphocytic leukemia-like clone. Abdominal CT also revealed

INTRODUCTION: The Person-Centered Hypothesis (PCH) framework is a structured clinical reasoning model designed to enhance "in action" reasoning in person-centered care (PCC). The PCH integrates elements of the biopsychosocial model, pain phenotyping, and contextual factors, addressing gaps in real-time clinical decision-making.
PURPOSE: To describe the proposed PCH framework that synthesizes four domains: dominant pain phenotype(s), relevant clinical patterns, regional and co-existing contributors, and psychosocial and lifestyle influencing factors. This masterclass outlines the clinical use …

An 89-year-old man presented with a 3-year history of chronic daily headaches that significantly worsened over the past 4 months. Initially episodic, the left-sided headaches became continuous and were accompanied by systemic symptoms such as weight loss and elevated inflammatory markers, specifically C-reactive protein and erythrocyte sedimentation rate. Over subsequent weeks, the neurologic examination revealed new-onset left-sided ophthalmoplegia, ptosis, and marked visual acuity loss. This article explores the differential diagnosis for such a complex clinical presentation and highlights h

We present the case of an 82-year-old woman with subacute altered mental status, oculomotor disturbances, and ataxia. On examination, she exhibited bilateral ptosis, complete horizontal ophthalmoplegia, and limited vertical eye movements during upgaze associated with prominent truncal ataxia. Cerebral MRI showed a mild hyperintensity on T2 and fluid-attenuated inversion recovery sequences in the posterior brainstem extending to the upper cervical cord, without gadolinium enhancement. Clinical and radiologic features suggested an encephalomyelitis with prominent brainstem involvement. We summar

A 48-year-old man was referred to the movement disorders clinic for 10 years of progressive slurred speech, spasticity, limb incoordination, and wide-based gait. Extensive neurologic workup was inconclusive, including serum and CSF testing, neuroimaging, EMG/NCS, exome sequencing, and mitochondrial testing. An ataxia repeat expansion panel ultimately revealed the final diagnosis. In this report, we review the clinical characteristics of a rare, late-onset, autosomal recessive cerebellar ataxia and discuss the importance of pursuing targeted gene testing to avoid diagnostic delays, especially a

Cerebral venous sinus thrombosis (CVST) is uncommon. Risk factors include inherited and acquired factors. Rapid diagnosis and treatment is essential and can help prevent complications, which can include seizures and visual disturbance. A 25-year-old woman with a background history of CVST and intermittent warfarin use presented to the hospital in 2021 with a 3-month history of progressive eye swelling and headache. Her headache was located in the right frontal region and worsened with movement. Her workup was consistent with recurrent CVST and dural arteriovenous fistula. IR-guided embolizatio

The "epilepsy-dyskinesia" spectrum is increasingly recognized in neurogenetic and neurometabolic conditions. It can be challenging to diagnose because of clinical and genetic heterogeneity, atypical or nonspecific presentations, and the rarity of each diagnostic entity. This is further complicated by the lack of sensitive or specific biomarkers for most nonenzymatic neurometabolic conditions. Nevertheless, clinical awareness and timely diagnosis are paramount to facilitate appropriate prognostication, counseling, and management. This report describes a case of a teenage girl who had presented a
