Congenital dilatation of the common bile duct in children.--The etiologic significance of the narrow segment distal to the dilated common bile duct.

Radiographic studies of 26 choledochal cyst patients were evaluated. Abnormal junction of the pancreatico-biliary system (PBS) was demonstrated in all patients. The transverse diameter of dilated common bile ducts was closely related to the length of the narrow segment of the distal common bile duct. Common bile ducts had not dilated despite the anomalous junction of PBS during the follow-up period, ranging from 2 to 12 years after choledochocysto -jejunostomy in 7 patients. The present studies indicate that no abnormal junction of PBS but stenosis of the distal common bile duct may be etiolog…

Inspissated bile within the common bile duct simulating cholangiocarcinoma. Case report.

There are a variety of abnormalities that can lead to obstruction of the common bile duct, several of which also can cause dilatation and duct irregularity. The differential diagnosis includes such entities as cholangiocarcinoma and carcinoma of the pancreas invading the common bile duct. We present an unusual case in which inspissated bile caused irregularity and obstruction of the common bile duct that radiographically simulated a cholangiocarcinoma.

Idiopathic retroperitoneal fibrosis producing common bile duct and pancreatic duct obstruction.

Idiopathic retroperitoneal fibrosis is a rare finding usually confined to the retroperitoneal space. Symptoms caused by this fibrotic process are usually secondary to compression and constriction of local anatomic structures. The ureters are the most common site of involvement, although the biliary tree, duodenum, and vasculature may also be affected. This case report describes a patient with retroperitoneal fibrosis involving the pancreas and common bile duct, and whose clinical course and investigative results stimulated cancer of the pancreas.

Large common bile duct stone: a case report.

Choledocholithiasis, the presence of stones within the common bile duct (CBD), poses significant therapeutic challenges, especially when stones exceed 15 mm in diameter. Large CBD stones can lead to complications such as biliary obstruction, cholangitis, and pancreatitis. Endoscopic retrograde cholangiopancreatography (ERCP) is the first-line treatment; however, surgical intervention may be required for refractory cases. We present a case of a 50-year-old female with a giant CBD stone measuring 6 × 4 × 4 cm, managed successfully via open CBD exploration and T-tube placement after failed endosc…

Malignant triton tumor of the common bile duct.

Malignant triton tumor (MTT) is a rare subset (5%) of malignant peripheral nerve sheath tumors (MPNSTs), classified as soft-tissue sarcomas. … denum, and rectum. In this article, we present what we believe to be the first report of MTT in the common bile duct. A multidisciplinary approach was the key in establishing this particular diagnosis, and workup included endoscopic ultrasound, endoscopic retrograde cholangiopancreatography, pathological staining, and genetic testing. Literature focusing on MTT re …

[Primary neuroendocrine tumor of the common bile duct].

Primary neuroendocrine tumors (NETs) of the bile duct are extremely rare and represent only 0.2-2% of all gastrointestinal NETs. Within the biliary system, the main bile duct is the most affected site. A 28-year-old man with a 6-month history of intermittent jaundice, pruritus, and choluria. MRCP, PET-CT and endoscopic ultrasound were performed. A well-differentiated neuroendocrine neoplasia was diagnosed. Complete resection of the main bile duct was performed with lymphadenectomy of the hepatic pedicle with Roux-en-Y hepaticojejunostomy, without complications. The patient had an adequate evol

Carcinosarcoma of common bile duct: A case report.

BACKGROUND: Carcinosarcomas of the common bile duct (CBD) are an extremely rare finding in the clinical setting. Based on a review of 12 literatures, 3 cases had the imaging features of ossification. Carcinosarcomas are prone to distant metastasis, as they possess clinical features of both carcinoma and sarcoma, and generally have with a poor prognosis. Due to the small number of cases reported, clinical experience in the diagnosis and treatment of the disease is lacking. CASE SUMMARY: The patient was a 75-year-old woman who had experienced recurrent chills with nausea and vomiting for 3 mo. …

Impacted common bile duct stone managed by hepaticoduodenostomy.

A bypass procedure such as a hepaticoduodenostomy may be an alternative to the traditional choledochoduodenostomy in the management of the retained, impacted distal common bile duct stone especially in the presence of sepsis. In low-resource settings with lack of fluoroscopy, fibreoptic instruments (choledoscope) or radiologically guided wire baskets or balloons for stone retrieval, there are operative hazards in blindly exploring the common bile duct. We present herein a hepaticoduodenostomy performed for a retained, impacted distal common bile duct stone in a low-resource setting with a good…

Impacted Common Bile Duct Stone Managed by Hepaticoduodenostomy.

A bypass procedure such as a hepaticoduodenostomy may be an alternative to the traditional choledochoduodenostomy in the management of the retained, impacted distal common bile duct (CBD) stone, especially in the presence of sepsis. We present herein a hepaticoduodenotomy performed for a retained, impacted distal CBD stone in a low-resource setting with a good outcome. This impacted stone had complicated an open cholecystectomy for acute cholecystitis by causing the dehiscence of the cystic duct stump as a result of distal biliary obstruction.

Double common bile duct associated with pancreaticobiliary maljunction.

A 45-year-old female was admitted to the hospital with a diagnosis of acute pancreatitis. A computed tomography scan showed two extrahepatic bile ducts. Magnetic resonance cholangiopancreatography suggested a stone in one of the bile ducts. Endoscopic retrograde cholangiopancreatography revealed two extrahepatic bile ducts joining at the hilum of the liver accompanied with pancreaticobiliary maljunction. Sphincterotomy was performed and a protein plug was drained from the bile duct. Several treatment options were discussed, and the patient was treated with laparoscopic cholecystectomy without

Common bile duct duplication: the more the murkier.

Congenital duplication of the common bile duct is an extremely rare anomaly of the biliary tract, which putatively represents failure of regression of the embryological double biliary system. Depending on the morphology of the duplicated bile duct, the anomaly can be classified into five distinct subtypes as per the modified classification (proposed by Choi et al). Among the five subtypes of bile duct duplication, type V duplication is considered to be the least common with only two previous cases of type Va variant reported in medical literature prior to the current report.

Transcystic approach to laparoscopic common bile duct exploration.

BACKGROUND AND OBJECTIVES: One-stage laparoscopic management for common bile duct stones in patients with gallbladder stones has gained wide acceptance. We developed a novel technique using a transcystic approach for common bile duct exploration as an alternative to the existing procedures. METHODS: From April 2010 to June 2012, 9 consecutive patients diagnosed with cholelithiasis and common bile duct stones were enrolled in this study. The main inclusion criteria included no upper abdominal surgical history and the presence of a stone measuring <5 mm. After the gallbladder was dissected free…

Primary malt lymphoma of the common bile duct.

Primary mucosa-associated lymphoid tissue (MALT) lymphoma arising in the common bile duct (CBD) is extremely rare. In our case of MALT lymphoma, CT and MRI showed long, segmental, irregular wall thickening of the CBD and minimal dilatation of the upstream bile duct. A preoperative diagnosis of cholangiocarcinoma was made, but histologic evaluation confirmed MALT lymphoma of the CBD. We herein present a rare case of MALT lymphoma of the CBD with CT and MRI findings.

Mixed neuroendocrine tumor of the common bile duct.

CONTEXT: Mixed adenoendocrine tumors of the extrahepatic bile ducts are exceedingly rare and most of those who are rarely diagnosed are adenocarcinomas. Neuroendorine tumors accounts for only 0.2-2%. CASE REPORT: We report a case of mixed adenoneuroendo-carcinoma of the common bile duct in an 82-year-old male. CONCLUSION: Clinical experience suggests that the neuroendocrine component of mixed tumors behave more aggressively than the regular biliary adenocarcinoma component. This clinical behavior may have an important role in the management of this clinical entity.

Double common bile duct: report of a case.

We report a case of double common bile duct. A 51-year-old woman was examined for abdominal pain. Ultrasound revealed a stone of approximately 7 mm in the gallbladder, and abdominal computed tomography revealed air in the intrahepatic bile duct and gallbladder. The endoscopic retrograde cholangiography showed that the contrast medium flowed into the duodenal bulbus from the gallbladder. The diagnosis was inflammatory cholecystoduodenal fistula, and a cholecystectomy was performed. There were no inflammatory changes surrounding the gallbladder. The resected specimen showed that the fistula was …

Papillary adenoma of the distal common bile duct.

A 73-year-old man with a papillary adenoma located in the distal common bile duct is reported. He underwent pylorus-preserving pancreatoduodenectomy. The lesion in the common bile duct featured papillary proliferation of the epithelium and fibrous elements with diffuse infiltration by inflammatory cells. Positive staining for MIB-1 (Ki-67) and p53 was identified in the nuclei of the proliferative epithelium. These findings suggested the malignant potential of this lesion. Further progress in imaging diagnostic techniques should increase the frequency with which such lesions are discovered. Eve…
