Third-party governance and artificial intelligence for diabetes management in primary health care, China.

PROBLEM: Fragmented primary health care in China fails to tackle the growing burden of noncommunicable diseases. Despite substantial investment, fewer than half of patients with diabetes achieve glycaemic control. APPROACH: Tianjin's 2020-2023 reform established a public-private partnership model where WeDoctor managed community health centres under a capitation scheme. This strategy integrated: (i) monthly prepaid capitation covering all diabetes-related outpatient services; (ii) claims auditing and clinical decision support using artificial intelligence (AI); (iii) dedicated health managers

Mobile mammography: assessment of self-referral in reaching medically underserved women.

BACKGROUND: Mobile mammography can be useful in reaching medically underserved women. However, it is not known whether self-referral for mobile mammography is the best approach for reaching the most vulnerable populations. OBJECTIVES: 1) To describe the community outreach patterns of a county-sponsored mobile mammography unit, 2) To characterize the follow-up patterns for women with abnormal screening mammograms, and 3) to identify reasons why women screened on mobile units seek follow-up care outside of the safety-net system. METHODS: We prospectively followed women aged > or = 40 years who

Barriers to mental health care utilization among internally displaced persons in the republic of Georgia: a rapid appraisal study.

BACKGROUND: There is a paucity of evidence on access to services for mental health and psychosocial support for conflict-affected populations in low- and middle-income countries. In the Republic of Georgia, rates of utilization of mental health services among internally displaced people with mental disorders are low. We set out to identify the health system barriers leading to this treatment gap. METHODS: We used rapid appraisal methods (collection and triangulation of multiple data sources) to investigate barriers to accessing mental health care services among adult IDPs in Georgia. Data col

Feasibility of a Church-Based, CHW-Delivered Diabetes and SDOH Intervention in a Hispanic Community: A Quasi-Experimental Study.

BACKGROUND: Social determinants of health (SDOH) screenings have identified disproportionate inequities in Hispanic populations that increase diabetes risk, yet they rarely lead to interventions. Churches are trusted community hubs with the potential to address health needs; however, partnerships with Hispanic churches remain largely unexplored. OBJECTIVE: To assess the feasibility of a church-based, community health worker (CHW)-led program to identify and address SDOH inequities in a Hispanic community. DESIGN: Pilot quasi-experimental feasibility study PARTICIPANTS: Forty-seven Hispanic a

Mapping global oral health financing architectures: a scoping review protocol.

INTRODUCTION: Oral healthcare financing varies considerably across countries in revenue collection, pooling, purchasing arrangements and benefit design. However, the global landscape remains fragmented, with limited systematic comparison of how established health-financing principles apply specifically to oral healthcare. This scoping review aims to map and classify oral health financing architectures globally and describe their structural design, including revenue sources, prepayment and pooling arrangements, purchasing and provider payment approaches and benefit and cost-sharing design. MET

Addressing Social Determinants of Health in Primary Care Patients May Reduce Hospital Readmissions: A Quasi-Experimental Study.

BACKGROUND: Health care systems are increasingly partnering with community-based organizations to address social determinants of health (SDH). We established a program that educates and connects patients with SDH needs at a primary care clinic to community services and facilitated referrals. OBJECTIVE: To evaluate the effect of addressing SDH soon after discharge on hospital readmission in a clinic population. DESIGN: Pre/post, quasi-experimental design with longitudinal data analysis for quality improvement. PARTICIPANTS: Clinic patients (n = 754) having at least one hospital discharge bet

Knowledge Assessment and Screening Barriers for Breast Cancer in an Arab American Community in Dearborn, Michigan.

Breast cancer (BC) is the most common malignancy afflicting women during their lifetime. Screening for breast cancer can reduce mortality through early cancer detection. Lack of knowledge is an important barrier leading to low screening rates. Little research has evaluated breast cancer knowledge and barriers to receiving appropriate screening among Arab Americans. Given that the Arab American population has grown approximately 47% since 2000, this study was designed to assess breast cancer knowledge and screening barriers in an Arab American community. An anonymous survey addressing knowledge

Dementia care gaps in Sub-Saharan Africa: A scoping review of technology, environmental, community perspectives, and financial constraints.

Dementia is a growing public health challenge in Sub-Saharan Africa (SSA), where population aging is expected to substantially increase the regional burden of dementia in the coming decades. Rapidly changing demographics, underdeveloped health infrastructure, and limited awareness of dementia symptoms and care pathways contribute to delayed diagnosis, treatment, and care. This scoping review examined literature published from 2021 to 2025 to identify gaps in dementia care across four key domains: environmental and social factors, technological capacity, community-based caregiving, and financia

Rethinking sources of representative controls for the conduct of case-control studies in minority populations.

BACKGROUND: Recruitment of controls remains a challenge in case-control studies and particularly in studies involving minority populations. METHODS: We compared characteristics of controls recruited through random digit dialing (RDD) to those of community controls enrolled through churches, health events and other outreach sources among women of African ancestry (AA) participating in the Women's Circle of Health Study, a case-control study of breast cancer. Odds ratios and 95% confidence intervals were also computed using unconditional logistic regression to evaluate the impact of including t

Identifying Barriers and Strategies to Support a Community Navigator-Driven Approach for Lung Cancer Screening.

Background/Objectives: Although lung cancer is the leading cause of cancer-related deaths in the United States, rates of screening have remained persistently low nationwide. This study sought to identify barriers, facilitators, and support strategies necessary for implementing a novel community health navigator workflow to improve lung cancer screening uptake in both rural and urban settings. Methods: Semi-structured interviews were conducted with primary care providers (n = 5), community scientists (n = 7), community health navigators (n = 4), and radiology staff (n = 2). Socioeconomic determinants and mental health correlates of intimate partner violence-related emergency department visits among women in California.

BACKGROUND: Intimate partner violence (IPV) is a major public health concern affecting one in three US women. Many survivors rely on emergency departments (EDs), particularly when socioeconomic disadvantage and mental health vulnerabilities heighten risk. OBJECTIVE: This study identifies demographic, socioeconomic, and clinical factors associated with IPV-related ED visits among women aged 15-64 years in California. METHODS: We performed a cross-sectional analysis of the California State Emergency Department Database (SEDD-HCUP) records from 2018 to 2020. IPV was defined using validated ICD-

Community Testing and SARS-CoV-2 Rates for Latinxs in Baltimore.

INTRODUCTION: Latinxs have been disproportionately impacted by COVID-19. Latinx immigrants, in particular, face significant barriers to SARS-CoV-2 testing, including lack of insurance, language barriers, stigma, work conflicts, and limited transportation. METHODS: In response to a disproportionately high SARS-CoV-2 positivity rate among Latinxs at the Johns Hopkins Health System, investigators implemented free community-based testing by partnering with religious leaders and leveraging the skill of trusted community health workers. Data were extracted from the electronic health record and a Re

State Medicaid coverage for tobacco-dependence treatments--United States, 1994-2001.

Tobacco use is the leading preventable cause of death in the United States. One of the national health objectives for 2010 is to increase insurance coverage of evidence-based treatment for nicotine dependence (i.e., total coverage of behavioral therapies and Food and Drug Administration [FDA]--approved pharmacotherapies) in Medicaid programs from 36 states to all states and the District of Columbia (DC) (objective 27.8). To increase both the use of treatment by smokers attempting to quit and the number of smokers who quit successfully, the Guide to Community Preventive Services recommends redu

Motivating factors and possible barriers to participation in digital prevention courses of two statutory health insurance funds in Germany: a qualitative interview study.

BACKGROUND: Digital prevention courses offered by statutory health insurance funds in Germany represent a low-threshold opportunity to support health-promoting behaviors through pre-recorded, video-based programs focusing on exercise, nutrition, or stress management. However, little is known about insured individuals' motivations and barriers to participation. This study aimed to identify relevant motivating and obstructive factors for participation in digital prevention courses and to explore differences compared to on-site formats. METHODS: We conducted semi-structured telephone interviews

Navigating Disparities and Overcoming Barriers to Access Diabetic Kidney Disease Therapies: A Proposed Multifactorial Approach.

<p>Background: Diabetic kidney disease (DKD) is a serious complication arising from long-term diabetes, disproportionately affecting historically marginalized populations, such as racial and ethnic minority groups, populations with low socioeconomic status, or a lower level of education. This review explores the causes of these disparities and barriers to accessing DKD therapies and proposes solutions. Summary: Socioeconomic factors like lack of health insurance, low income, and limited health literacy significantly hinder access to DKD therapies for marginalized communities. Additionally, ina
