Venous air embolism, which may complicate medical and surgical procedures, activates complement and triggers thromboinflammation. In lepirudin-anticoagulated human whole blood, we examined the effect of air bubbles on complement and its role in thromboinflammation. Whole blood from 16 donors was incubated with air bubbles without or with inhibitors of C3, C5, C5aR1, or CD14. We report a case of C5 deficiency in combination with Sjögren's syndrome (SS). Our patient presented with polyarthritis and complaints of oral and ocular dryness. In the serum there was a very low titer of total hemolytic complement (CH50) due to a deficiency of the fifth complement component. C5 deficiency is often associated with recurrent life threatening infections, membranous glomerulonephritis, and discoid lupus erythematosus, but it has not been described in association with primary SS.

Serum C1q, C3, C4, and C5 components of complement levels were measured in 35 healthy subjects and 29 patients with acute conjunctivitis due to adenovirus type 2, 7, and 8 infections. Serum C1q and C3 levels were found to be significantly low, and there was no change in serum C4 and C5 levels. A progressive decline in the serum complement level with the severity of disease was observed. The change in the complement levels in serum showed a tendency to reach normal levels with clinical improvement.

Human sera were examined for evidence of complement activation by Fonsecaea pedrosoi. The serum that had been incubated with F. pedrosoi showed immunoelectrophoretic C3 conversion and generation of C5a, as measured by radioimmunoassay.
