[The birth of biological therapy in Hungary: the story of László Meduna's first patient receiving convulsive therapy].

Convulsive treatment was introduced to psychiatry by László Meduna, a Hungarian neuropsychiatrist. In his autobiography, Meduna gave a detailed description of his first patient who underwent convulsive therapy. According to Meduna's recollections, this patient was L. Zoltán, who after 4 years of fluctuating catatonic stupor received several sessions of camphor-induced convulsive therapy resulting in full remission and discharge from the institution. In this communication, the authors reconstruct L Zoltán's case history from the original case notes, which were recovered from the Archives of the…

Treatment of refractory convulsive status epilepticus in children: other therapies.

Refractory convulsive status epilepticus occurs when seizures are not controlled with initial benzodiazepine therapy or a subsequent anticonvulsant drug. Typically drug-induced anesthesia is then pursued with midazolam or a barbiturate. This results in prolonged, intensive care, which requires meticulous attention to medical management to minimize complications. When seizures persist other options must be considered. These include (1) other medications, (2) surgery, (3) the ketogenic diet, (4) hypothermia, (5) inhalational anesthetic agents, and (6) immune modulating therapy. This review addre

[Electroconvulsive therapy--sources of error and aids in insufficient duration of convulsions].

Modern electroconvulsive therapy (ECT) involves the administration of six to ten convulsions of sufficient duration (25 seconds) by unilateral electrical stimulation of the non-dominant hemisphere. Missed or abortive seizures are a common clinical problem but may often be accounted for by concurrent medication and technical problems with unilateral ECT seizure induction. In the absence of such factors, controlled hyperventilation and intravenous caffeine are adjunctive means to achieve an adequate unilateral ECT treatment course.

Indications for electric convulsion therapy and its use by senior psychiatrists.

A survey by questionnaire of all senior psychiatrists in the Wessex Region showed that they considered depressive psychosis to be the major indication for electric convulsion therapy (ECT). A good clinical response was thought to be predicted by the presence of psychomotor retardation, depressive delusions, depressed mood, early morning wakening, diurnal variation, loss of appetite, and agitation. ECT was judged to be extremely useful for treating mania and acute undifferentiated, catatonic, and paranoid schizophrenia; of some use in hypochondriasis; but of little value or contraindicated when

An up-to-date review of theories of shock therapies: are convulsive shock therapies "moral treatments"?

About four decades ago, empirically developed shock therapies were introduced as treatments for mental disorders. However, there still remains today a wide disagreement as to why these procedures are effective. It has long been known that the behavior in lower animals can be altered by the use of planned psychologcal methods. Likewise psychological procedures (employing such factors as suggestion and fear) had been used by preliterate man all through the ages in the treatment of mental illness. During the past century various types of pre-modern shock therapies using suggestion, fear or excita

Clinical effect of non-convulsive electroconvulsive therapy combined with olanzapine in patients with schizophrenia.

OBJECTIVE: To explore the clinical effect of non-convulsive electroconvulsive therapy combined with olanzapine in schizophrenia patients. METHODS: From September 2021 to August 2023, a total of 112 patients with schizophrenia treated in our hospital were selected as the study participants. These patients were randomly divided into a control group and a research group, with 56 cases in each group. Patients in the control group were administered olanzapine tablets. Based on olanzapine tablets, patients in the research group received non-convulsive electroconvulsive therapy. The psychotic sympto…

Stigmatization towards electro convulsive therapy: Impact of practical teaching on medical and nursing students.

BACKGROUND: This study explored the impact of theoretical and practical teaching on electro convulsive therapy (ECT) on medical and nursing students' stigmatizing attitudes towards ECT and representations of it. METHOD: Fourth-year medical students and nursing students answered questions from the Questionnaire on Attitudes and Knowledge of ECT (QuAKE) and from the Mental Illness: Clinicians' Attitudes version 2 (MICA v2) scale. The questionnaires were completed before and after observing a 3-hour practical training session in the ECT unit. The endpoint was the impact of practical training as …

Carbon Monoxide Poisoning Presenting as Non-Convulsive Status Epilepticus Treated with Hyperbaric Oxygen Therapy.

Carbon monoxide (CO) poisoning is one of the most serious medical emergencies causing life-threatening conditions, including cardiovascular and neurological sequelae. Acute CO poisoning can lead to myocardial ischemia, ventricular arrhythmia, syncope, seizures, and coma. Seizures and other neurological complications in the early stages of presentation are related to severe intoxication in CO poisoning. In such situations, aggressive hyperbaric oxygen therapy is recommended. In CO poisoning, non-convulsive status epilepticus has rarely been observed following hyperbaric oxygen therapy (HBO2). W

A randomized clinical trial of adjunctive ketamine anesthesia in electro-convulsive therapy for depression.

BACKGROUND: Electroconvulsive therapy (ECT) is a rapid acting and effective treatment for both major depressive disorder (MDD) and bipolar disorder (BP). Both propofol and ketamine are commonly used anesthetic agents but recent clinical studies suggest that ketamine has rapid-acting antidepressant properties, itself, at sub-anesthetic doses. METHODS: A total of 77 inpatients (41 MDD and 36 BP) were randomly assigned to receive ECT with propofol (1mg/kg) anesthesia or with ketamine (0.5mg/kg) plus propofol (0.5mg/kg). Depressive symptoms were assessed with the 24-item Hamilton Depression Ratin…
