Cough is one of the most common reasons for health care visits. This article seeks to illustrate the classification, appropriate workup, and management of acute and subacute cough. It highlights etiologies such as bronchitis, pneumonia, and exacerbation of heart failure and asthma. There is an emphasis placed on potentially life-threatening pathologies associated with cough and how to appropriate triage cough severity in an emergency setting. The pathophysiology of cough and how it may lead to severe barotraumatic complications such as pneumothorax and Mallory Weiss tears is discussed.

Upper airway cough syndrome (UACS), formerly known as postnasal drip syndrome, is one of the most common causes of chronic cough. It encompasses a spectrum of upper airway disorders, including allergic rhinitis, nonallergic rhinitis, and chronic rhinosinusitis which stimulate cough through mucosal inflammation and upper airway hypersensitivity. This article reviews the pathophysiology, diagnostic evaluation, and management of UACS from an otolaryngologic perspective. The lack of a clear diagnostic test requires clinical differentiation from other causes of chronic cough. Evidence-based treatme…

Upper airway cough syndrome (UACS), formerly known as postnasal drip syndrome, is one of the most common causes of chronic cough. UACS, asthma, and gastroesophageal reflux make up 90% of the cause of chronic cough. UACS is a clinical diagnosis of exclusion with no diagnostic testing or objective findings. UACS can be present with or without associated rhinitis and chronic rhinosinusitis. Treatment includes dual therapy with H1 receptor antihistamines and decongestants. Diagnosis is confirmed when therapeutic intervention results in symptom resolution.

Cough variant asthma (CVA) was first described by W. Corrao. CVA was described as the isolated chronic cough as the only presenting symptom responsive to bronchodilator therapy. This phenotype of asthma is present with airway hyperresponsiveness, eosinophilic inflammation airways and bronchodilator responsive coughing without typical manifestation of asthma such as wheezing or dyspnea. CVA shares common features with classic asthma such as eosinophilic inflammation and airway remodeling. Because of that, CVA is clinically considered as a variant type of asthma.

Chronic cough, that 10-38% outpatients have, is an important cause of morbidity and mortality. This symptom can be seen 3-40% of adult patients and reduces quality of life. 95% patients that cough chronically have one of these three diseases: upper airway cough syndrome, gastroesophageal reflux or asthma. In this review these three diseases and rare causes of chronic cough will be discussed and diagnostic steps will be explained.

BACKGROUND: Occasionally, patients who complain of chest pain after the onset of coughing are diagnosed with rib fractures. We investigated the characteristics of cough-induced rib fractures.
METHODS: Between April 2008 and December 2013, 17 patients were referred to our hospital with chest pain after the onset of coughing. Rib radiography was performed, focusing on the location of the chest pain. When the patient had other signs and symptoms such as fever or persistent cough, computed tomography of the chest was carried out. We analyzed the data retrospectively.
RESULTS: Rib fractures were …

Despite the high prevalence of cough in children, the topic has been poorly researched. Although pediatricians recognize that chronic cough in children is different from that in adults, this difference seems less recognizable to other health professionals. During childhood, the respiratory tract and nervous system undergo a series of anatomical and physiological maturation processes that influence the cough reflex. Additionally, immunological responses undergo developmental and memorial processes that make infection and congenital abnormalities the overwhelming cause of cough in children. The …

The management of chronic cough, a common complaint in children, is challenging for most health care professionals. Millions of dollars are spent every year on unnecessary testing and treatment. A rational approach based on a detailed interview and a thorough physical examination guides further intervention and management. Inexpensive and simple homemade syrups based on dark honey have proved to be an effective measure when dealing with cough in children.

Chronic cough is a common symptom in childhood and very often the expression of a cold like respiratory tract infection. Chronic cough may follow an acute cough lasting more than 4-8 weeks or may be the primary symptom of an underlying severe respiratory disease. It is important to find out the exact origin of chronic cough to allow accurate investigation steps and treatment. In childhood, adults like diagnostic approach is not appropriate and differential diagnosis must encompass children specific age dependent diagnosis. This article aims to stress a specific paediatric approach in prolonged…

The evidence for supramedullary influences on cough is largely indirect. Cough can be voluntarily induced or inhibited, functions usually thought to reside in the cerebral cortex. A sensation of 'urge-to-cough' usually precedes cough due to an airway irritant stimulus, and this may well involve the cerebral cortex. In conditions with interruption of the pathways between the cortex and the brainstem, such as strokes and Parkinson's disease, voluntary cough may be inhibited without disruption of reflex cough from the larynx or lower airways. 'Habit cough', like Tourette's syndrome, is assumed to…

Epidemiological studies have shown that exposure to air pollution is associated with respiratory symptoms and decreases in lung function. This paper reviews recent literature showing that exposure to particulate matter, irritant gases, environmental tobacco smoke (ETS), mixed pollutants, and molds is associated with an increase in cough and wheeze. Some pollutants, like particulate matter and mixed pollutants, appear to increase cough at least as much as wheeze. Others, like irritant gases, appear to increase wheeze more than cough. For ETS, exposure during childhood is associated with cough a…

Cough is an important defensive reflex of the airway and also a common symptom of respiratory disease. Cough after common respiratory virus infection is transient but is more persistent when associated with conditions such as asthma, rhinosinusitis, gastro-oesophageal reflux, chronic obstructive pulmonary disease and lung cancer. Persistent cough may be due to peripheral and/or central sensitisation of cough reflexes initiated by cough receptors, rapidly adapting receptors or nociceptors. Treatment directed at associated conditions such as asthma (with anti-inflammatories) and gastro-oesophage…

OBJECTIVE: To report a case of chronic, persistent cough induced by omeprazole therapy.
CASE SUMMARY: A 42-year-old white woman presented with chronic, persistent cough after omeprazole initiation for treatment of postoperative heartburn. The cough was permanent, dry, and exhausting and worsened at night. Omeprazole therapy was continued for 4 months because the persistent cough was thought to be related to gastroesophageal reflux disease (GERD). However, no cause of persistent, chronic cough was identified. After omeprazole discontinuation, the cough resolved.
DISCUSSION: The most common ca…

We report on a 29-year-old female patient who developed a dolent nodular exanthema on both lower legs after a prolonged respiratory infection of about 14 days. The chest X-ray showed a distinct enlargement of the left hilus and reduced transparency in the left upper pulmonary lobe. These findings were interpreted as pneumonia with para-infectious erythema nodosum. The serology showed IgM antibodies against chlamydia pneumoniae. Therapy with a macrolide antibiotic led to a rapidly improved general condition and also to the disappearance of the exanthema. There are some reports on single cases w
