Erythropoietic protoporphyrias: updates and advances.

Protoporphyrias are caused by pathogenic variants in genes encoding enzymes involved in heme biosynthesis. They induce the accumulation of a hydrophobic phototoxic compound, protoporphyrin (PPIX), in red blood cells (RBCs). PPIX is responsible for painful cutaneous photosensitivity, which severely impairs quality of life. Hepatic elimination of PPIX increases the risk of cholestatic liver disease, requiring lifelong monitoring. Treatment options are scarce and mainly limited to supportive care such as protection from visible light. Here, we review the pathophysiology of protoporphyrias, their

[Surgical options in the treatment of chronic venous ulcers].

Surgery offers several options in prevention of chronic venous insufficiency and its sequelae. Both the operation on veins with valve dysfunction to reduce reflux and the elimination of obstruction in thrombosed veins aim for the reduction of venous hypertension. Elevated venous pressure, impairment of cutaneous capillaries and a chronic inflammatory process result in sclerosis of skin and subcutaneous tissue and might proceed to the fascia resulting in a chronic compartment syndrome. Non- healing chronic venous ulcers under conservative therapy for more than three months may be treated by vei

[Miltefosine: a new remedy for leishmaniasis].

There is a need for a safe and effective oral treatment for cutaneous and visceral leishmaniasis. Miltefosine is the first oral drug that is efficacious against different forms ofleishmaniasis, however it is not equally effective against all Leishmania species. Miltefosine is an alkylphosphocholine, originally developed for the treatment of cancer. The mechanism of action is probably based on interference with the synthesis and degradation of parasitic membrane lipids. Little is known about the pharmacokinetics ofmiltefosine; an important characteristic is its long elimination half-life of sev

[Cutaneous leishmaniasis: incidental outbreak in a new endemic area?].

In the autumn of '94 we saw 32 patients with cutaneous leishmaniasis from a newly populated areas south of Yerucham. Yerucham had never previously been reported as a focus of leishmaniasis. Ongoing construction in the town and the health hazards that resulted may play a part in this new situation. There are a number of ways of controlling and preventing outbreaks leishmaniasis. They include elimination of the vector, Phlebotomus and its host, the gerbils as well as their food supply, "maloach" bushes, other health hazards and vaccination of the population. Patients were treated according to th

Self-Applied Daylight Photodynamic Therapy: A Paradigm Shift?

Photodynamic therapy (PDT) involves the topical application of a photosensitizer and its activation by visible light, leading to the generation of protoporphyrin IX (PpIX) and reactive oxygen species. Daylight photodynamic therapy (dPDT), a variant utilizing natural sunlight as the energy source, enhances procedural flexibility by eliminating the need for specialized equipment. dPDT has been effectively used in dermatology to treat various cutaneous disorders, including neoplastic and infectious diseases. Traditionally, skin preparation and photosensitizer application are performed by trained

Cutaneous approaches to the lower lid and orbit.

In any discussion of surgical exposure, it is important to remember that of chief importance is what is done in the operative field, rather than how the site is exposed. Selection of approach is based on tradeoffs that must be weighed by the surgeon and tailored to the deformity for a given patient. Despite a recent surge in the popularity of the transconjunctival technique, periorbital surgery by a cutaneous approach is still a valid means of access for a variety of esthetic and particularly trauma procedures. There are few solid data comparing the two approaches, and what does exist shows li

Surgical planing of the skin; dichlorotetrafluoro-ethane as a freezing agent.

Surgical skin planing is, in the hands of an experienced operator, a safe and highly effective procedure for treating a number of cutaneous defects, most notably pitted acne scars. The operation is facilitated by the use of a new instrument (jet-spray handpiece) which allows the operator to freeze the skin and plane it almost simultaneously, and by a new freezing agent, dichlorotetrafluoro-ethane, which adds to the safety by eliminating the old hazards of inflammability, explosion, and the toxic inhalation of ethyl chloride. The ability to sharply differentiate between keloid and hypertrophic

Hypothesis: patient with possible disturbance in programmed cell death.

Programmed cell death is a physiological process in mammalian development by which specific types of cells are eliminated, and, hence, is of fundamental importance in normal human embryogenesis. A patient is described with multiple congenital anomalies that may be explained by a disturbance of programmed cell death. Anomalies included macrocephaly, hypoplastic lacrimal ducts, narrow external ear canals, pharyngeal mucous membrane fold, unilateral cryptorchidism, cord-like vasa deferentia, and complete cutaneous syndactyly of the hands and feet.

Suprainguinal ligament approach for surgical treatment of meralgia paresthetica. Technical note.

For relief of meralgia paresthetica the authors have developed a suprainguinal ligament approach for decompression of the lateral femoral cutaneous nerve of the thigh. This proximal approach offers an alternative to the standard infraligament methods. Its chief advantage is that identification of the nerve trunk is easy and accurate. This avoids the tedious dissection involved in looking for small distal branches with their variable location. Lysis is easily performed, and angulation of the nerve in the iliac fossa is avoided. With this method, poor operative results can usually be eliminated.

[Variants of plastic surgery using trapezoid adipose-cutaneous flaps in post-burn marginal deformities and contractures].

Most optimal methods of plasty with the maximum use of intact skin of a healthy leaf and adjacent area as a trapezoid cellulo-cutaneous flap are proposed. Different variants of the operation are proposed as one-, two-, three- and many-flaps plasty with local tissues and one- and two-flaps combined plasty. The choice is dependent on the localization and degree of contracture. Plasty by the methods proposed allows to adequately eliminate marginal contractures of large vessels, syndactylia, microstomia and narrowing of the lid slit.

Characteristic infectivity of Sporothrix schenckii to mice depending on routes of infection and inherent fungal pathogenicity.

Isolates of Sporothrix schenckii were examined for their infectivity in BALB/c mice. The mice were injected with yeast forms of S. schenckii isolates differing in clinical source (human cutaneous lesions and pulmonary lesions), and fungal growth was determined at intervals in the footpad and visceral organs. After subcutaneous injection of approximately 10 colony forming units (cfu) of S. schenckii into the footpad, locally restricted fungal infection developed gradually. At the peak of the infection (3-4 weeks post-inoculation), viable fungal counts reached 102-106 cfu/footpad. Dissemination

Antileishmanial Effects of Synthetic EhPIb Analogs Derived from the Entamoeba histolytica Lipopeptidephosphoglycan.

With an estimated number of new cases annually of approximately 1.4 million, leishmaniasis belongs to the most important parasitic diseases in the world. Nevertheless, existing drugs against leishmaniasis in general have several drawbacks that urgently necessitate new drug development. A glycolipid molecule of the intestinal protozoan parasite Entamoeba histolytica and its synthetic analogs previously showed considerable immunotherapeutic effects against Leishmania major infection. Here, we designed and synthesized a series of new immunostimulatory compounds derived from the phosphatidylinosit

Monocyte chemotactic protein-1 stimulates the killing of leishmania major by human monocytes, acts synergistically with IFN-gamma and is antagonized by IL-4.

We recently demonstrated that monocyte chemotactic protein-1 (MCP-1) is strongly expressed in lesions of patients with self-healing localized cutaneous leishmaniasis (LCL) whereas it is scarce in those of chronic diffuse cutaneous leishmaniasis (DCL). This finding indicated that MCP-1 may contribute to the healing process. In the present study, we analyzed the capacity of MCP-1 to trigger leishmanicidal activities. The results show that MCP-1 directly stimulates the elimination of intracellular Leishmania parasites by human monocytes, a potential that correlates with the induction of reactive

Changes in fibre populations of the rat hairy skin following selective chemodenervation by capsaicin.

Perineural application of capsaicin results in a selective and permanent reduction in the sensitivity to noxious chemical and heat stimuli and elimination of the neurogenic inflammatory response. The present quantitative immunohistochemical study has been undertaken to reveal the populations of cutaneous afferent nerves that are affected by perineural capsaicin treatment. Areas of intact and chemodenervated skin were determined with the aid of the vascular labelling technique. In sections taken from intact skin areas, staining with antibodies against protein gene product 9.5 revealed a rich ep

Skin lesions in histoplasmosis.

Histoplasmosis is a granulomatous infection caused by Histoplasma capsulatum, a dimorphic fungus. It is distributed worldwide and prevalent in certain regions of North and Central America. Pulmonary involvement is the most common clinical presentation. Cutaneous manifestations are reported to occur in 10% to 25% of AIDS patients with disseminated histoplasmosis. The skin lesions are polymorphic papules, plaques with or without crusts, pustules, nodules, mucosal ulcers, erosions, punched out ulcers, lesions resembling molluscum contagiosum, acneiform eruptions, erythematosus papules and keratot

[Gynaecological and obstetrical aspects of recurrent urinary tract infections].

The microbial colonization of vulva, vagina and cervix uteri represents the reservoir for recurrent urinary tract infection. All bacterial species of normal cutaneous or gastrointestinal flora can be found in the external genital tract even under physiological conditions. The higher concentration of microbes adds to the predisposition for urinary tract infection in cases of dysbiosis or inflammation, apart from specific infection by Trichomonas vaginalis, Neisseria gonorrhoeae or Chlamydia trachomatis. The specific immunological interaction between bacteria and host, i.e. between virulence fac

[Total parenteral nutrition as primary therapy in Crohn's disease (author's transl)].

Total parenteral nutrition was given to 10 patients with acute regional enteritis for 3--12 weeks. The rationale of this therapy is elimination of potential food allergens and decrease of intestinal irritation. Complete remission of the disease could be achieved in 8 patients, thus surgical removal of the intestine involved could be avoided. One entero-cutaneous and two perinal fistulas healed spontaneously; two other perianal fistulas improved so much, that local surgical treatment was sufficient. If well planned, total parenteral nutrition can be performed without disturbance of metabolism a

Immune serum confers protection against syphilitic infection on hamsters.

Pooled serum from hamsters immune to syphilitic infection conferred complete protection on recipient hamsters challenged with Treponema pallidum subsp. endemicum. Cutaneous lesions did not develop, and the recipients' lymph nodes weighed less than those of controls and contained no treponemes. Treponemicidal activity in the pooled immune serum was relatively high. When treponemes were incubated in immune serum and complement and the suspension was then inoculated into hamsters, recipients developed neither lesions nor enlarged lymph nodes teeming with treponemes. With hamsters already infected

Cutaneous Hemophagocytosis in a Sweet Syndrome-An Unusual Phenomenon in a Rare Guise.

Cutaneous hemophagocytosis is typically known as skin manifestation of syndromal hemophagocytic lymphohistiocytosis, which presents with fever, splenomegaly, cytopenia, hyperferritinemia, and hypertriglyceridemia. Pathophysiologically, an ineffective pathogen elimination has been postulated, which is compensated by excessive macrophage activation. In this study, we present an unusual case of skin limited cutaneous hemophagocytosis within a rare manifestation of a Sweet syndrome triggered by an upper respiratory infection and drug cofactors. A 38-year-old female patient presented with a painful

The evolution of the lesion in cutaneous leishmaniasis.

In order to elucidate the mechanisms for the elimination of Leishmania, the histological evolution of the lesions of cutaneous leishmaniasis was studied in 118 biopsies from four geographical areas known to be associated with different species or sub-species of Leishmania. Basically there were 3 types of response: A, parasites were eliminated within intact macrophages which later evolved as epithelioid cells; B, they were eliminated as a result of the lysis of the macrophages either individually or in small clusters, but the process was incomplete; C, there was necrosis proceeding to completio
