Surgical decompression: a life-saving procedure for an extensive spinal epidural abscess.

Extensive spinal epidural abscesses (SEAs) carry a high mortality rate. Traditionally they are treated non-operatively with long-term antibiotics and/or surgical decompression, but there is a continuing debate as to whether they should be managed by emergency surgical decompression. However, such decisions are made in the light of the clinical setting. We report the successful management of a female patient who presented with features of upper cervical cord compression and later developed septic shock and multisystem failure. Surgical decompression of the cervical spine and irrigation of the e

Current status of surgical decompression and drainage procedures upon the endolymphatic system.

At the present time, neither the etiologic factors nor the pathophysiologic mechanisms that lead to the development of endolymphatic hydrops or Ménière's disease are known; however, the anatomic-pathologic alterations in the labyrinth that are produced by the disease are well-documented. Undoubtedly, what is not known is responsible for the controversial status and questionable benefit that surgical decompression and drainage procedures have upon the endolymphatic system.

Near-holocord spontaneous spinal epidural haematoma: successful outcome following targeted surgical decompression.

Spontaneous spinal epidural haematoma (SSEH) is a rare neurosurgical emergency characterised by the accumulation of epidural blood in the absence of trauma. Near-holocord haematoma extension is exceptionally uncommon and poses significant challenges to surgical management.A man in the 40s presented with the sudden onset of low back pain, flaccid paraplegia, urinary retention and hypertension. MRI revealed an extensive posterior epidural haematoma extending from the cervicothoracic junction to the sacrum, with maximal compression at the T11-L2 levels.Within 4 hours, the patient underwent a focu

Surgical Management after Chiari Decompression Failure: Craniovertebral Junction Revision versus Shunting Strategies.

Revision surgery after posterior fossa decompression for Chiari malformation is not uncommon and poses both strategic and technical challenges. We conducted a single-center retrospective cohort study including all adult patients who underwent revision surgery after posterior fossa decompression for Chiari type I malformation between 2010 and 2019. Among 311 consecutive patients operated on for Chiari malformation at our institution, 35 patients had a least one revision surgery with a mean follow-up of 70.2 months. Mean delay for revision was 28.8 months. First revision surgery was performed at

Surgical decompression among Paget-Schroetter patients with subacute and chronic venous occlusion.

OBJECTIVE: Management of Paget-Schroetter syndrome (PSS) with first rib resection (FRR) and venoplasty is successful in re-establishing subclavian vein (SCV) patency in most cases. However, in cases with subacute or chronic venous occlusion, SCV patency may not be achieved. Thus, the role for FRR remains controversial in cases of subacute or chronic SCV occlusion. Our goal is to determine whether FRR is beneficial in PSS patients with subacute or chronic SCV occlusion. METHODS: A prospectively maintained thoracic outlet syndrome (TOS) database was searched for patients undergoing FRR who were

Influential factors of surgical decompression for ulnar nerve neuropathy in Guyon's canal.

BACKGROUND: Guyon's canal syndrome is nerve compressive pathology which can lead to sensory and/or motor function deficits. This problem is usually difficult to distinguish from cubital tunnel syndrome and relatively less common than cubital tunnel syndrome. This study evaluated the functional results and patient-reported outcomes following decompression of the ulnar nerve in Guyon's canal. METHODS: Patients who were diagnosed with Guyon's canal syndrome confirmed by electrodiagnostic studies and underwent nerve decompression surgery were included in this study. The functional improvement by

Surgical Nuances to Reduce and Manage Cerebrospinal Fluid Leaks after Microvascular Decompression.

BACKGROUND: No dural substitute has proven to be complication-free in a large clinical trial, even suggesting some benefit during watertight closure. However, primary dural closure is not always possible due to dural shrinkage from electrocautery for dural bleeding. OBJECTIVE: This study is performed to analyze the clinical outcomes related to cerebrospinal fluid (CSF) leakage after microvascular decompression (MVD) using a simple surgical technique. METHODS: Three hundred and sixty consecutive cases were treated with MVD and followed up for more than one month after surgery. Bleeding from t

Microvascular Decompression of the Trigeminal Nerve with Petrous Sling Technique: Surgical Video.

The retrosigmoid approach for microvascular decompression of the trigeminal nerve (TN) is an established and highly effective technique for the treatment of trigeminal neuralgia due to vascular compression. It is common to place a pledget or other cushion material between the source of vascular compression, typically the superior cerebellar artery (SCA), and the TN after vessel mobilization and decompression. A previous study demonstrated the use of a tentorial sling on the SCA to maintain decompression of the TN, with encouraging results.1 In this video, we demonstrate a novel technique using

[Microvascular decompression surgery for trigeminal neuralgia: technical review based on surgical anatomy].

To perform secure microvascular decompression for trigeminal neuralgia, it is important to perform each surgical procedure based on detailed anatomical knowledge. As for microsurgical procedure, there are two major approaches; the supracerebellar approach and the trans-horizontal fissure approach. This article reviews the advantages of each surgical approach and describes points to pay attention to, with figures which will help the surgeon to perform this surgery. The importance of real decompression or transposition of the offending vessels is stressed. The significance of preoperative imagin

[Surgical decompression in space-occupying cerebral infarct; notification of a randomized trial].

Patients with a hemispheric infarct and massive space-occupying brain oedema have a poor prognosis. Despite intensive conservative treatment, the case fatality rate may be as high as 80%, and most survivors are left severely disabled. Non-randomised studies suggest that decompressive surgery substantially reduces mortality and improves the functional outcome of survivors. The 'Hemicraniectomy after middle cerebral artery infarction with life-threatening edema trial' (HAMLET) is a newly-conceived randomised multi-centre clinical trial that compares the efficacy of decompressive surgery to impro

A case of Guyon syndrome with neuroapraxic block resolved after surgical decompression.

Guyon syndrome is the well-known ulnar entrapment at the wrist; usually surgical decompression improves the symptoms. Neurophysiological studies are essential to demonstrate the abnormality of ulnar nerve conduction at the wrist. We report a case of Guyon syndrome onset after 6 h of cycling. Neurophysiological study revealed ulnar neuroapraxic block at the wrist, and axonal impairment. Post-operative clinical and neurophysiological follow-up showed marked clinical improvement and neurophysiological resolution of abnormalities. To our knowledge, no cases of Guyon syndrome with neuroapraxic bloc
