Severe hypocalcemia following denosumab injection in a hemodialysis patient.

Denosumab is a human monoclonal antibody directed against RANKL (receptor activator of NF-κB ligand) and is a novel treatment for postmenopausal osteoporosis, although its safety and efficacy in end-stage renal disease is unclear. We report the case of a 61-year-old female hemodialysis patient who developed severe hypocalcemia (total serum calcium, 5.37 mg/dL [1.34 mmol/L]) after receiving a single subcutaneous injection of denosumab. We review this medication's mechanism of action and the very limited data regarding its use in stage 5 chronic kidney disease. We advise against the use of denos…

Denosumab: benefits of RANK ligand inhibition in cancer patients.

PURPOSE OF REVIEW: RANK ligand (RANKL) plays a central role in the cancer-induced bone destruction that results from bone metastasis. The RANKL inhibitor denosumab was recently approved for the prevention of skeletal-related events (SREs) in patients with bone metastases from solid tumors. This review summarizes recent findings of the effects of RANKL inhibition and denosumab in the cancer setting. RECENT FINDINGS: Preclinical data show that RANKL inhibition in combination with antitumor therapies has additive effects on preventing skeletal tumor progression. Preclinical breast cancer models …

Impact of clinical pharmacist-led interventions on denosumab adherence.

UNLABELLED: Pharmacist-led interventions significantly improved denosumab adherence in patients with osteoporosis at an academic medical center, reducing non-adherence from 20.8 to 4.9%. This quality improvement initiative included pharmacist-led medication counseling and laboratory monitoring, highlighting the effectiveness of pharmacist-physician collaboration in preventing rebound-associated vertebral fractures and enhancing patient outcomes. BACKGROUND: Interrupting or delaying denosumab beyond 7 months (210 days) from the last injection leads to transient increase in bone turnover known …

Denosumab discontinuation and fracture risk: getting the balance right.

Denosumab is a highly effective therapy for osteoporosis. Since its approval by the Therapeutic Goods Administration (TGA) in 2010, use of denosumab has surged across Australia, delivering substantial fracture-prevention benefits to tens of thousands of patients. However, stopping denosumab may lead to acute loss of bone with an increased risk of fractures. Increasing awareness of this so-called 'rebound phenomenon' has created fear and concern among consumers and prescribers. A recent TGA safety update in November 2025 has raised further concern regarding the risk of spontaneous vertebral fra…

Denosumab and invasive cervical root resorption: a case report.

This case report describes an adverse side effect from long-term denosumab therapy in a patient with metastatic breast cancer. The patient presented with extensive invasive cervical root resorption affecting most of her dentition. There were no other identified risk factors. As the treatment of breast cancer evolves leading to improved survival, and the number of cancer survivors increases, it is expected that dental practitioners will see a growing number of patients who are on long-term denosumab. Comprehensive dental examinations including radiographs are warranted to identify unusual or un…

Denosumab After Teriparatide in Premenopausal Women With Idiopathic Osteoporosis.

CONTEXT: We have previously reported that teriparatide is associated with substantial increases in bone mineral density (BMD) at the lumbar spine (LS), total hip (TH), and femoral neck (FN) and small declines at the distal radius in 41 premenopausal women with idiopathic osteoporosis (IOP), all severely affected with low trauma fractures and/or very low BMD. Effects of teriparatide dissipate if not followed by antiresorptives. OBJECTIVE: To assess the effects of 12 and 24 months of denosumab in premenopausal women with IOP completing 24 months of teriparatide. METHODS: This was a preplanned …

Update on denosumab in postmenopausal osteoporosis--recent clinical data.

Denosumab, a fully human monoclonal antibody against the key osteoclastogenic factor RANK ligand, is currently approved for the treatment of postmenopausal osteoporosis. Denosumab differs from bisphosphonates in many aspects, for example, its ability to act in the extracellular compartment and its likelihood to be distributed throughout the skeleton. In contrast, bisphosphonates have to be internalized by osteoclasts and are mainly located across bone surfaces. This could explain why patients with osteoporosis, who are already treated with bisphosphonates, might experience further benefit when…

New therapies for osteoporosis: zoledronic acid, bazedoxifene, and denosumab.

Intravenous (IV) zoledronic acid, a new once-yearly bisphosphonate therapy, is approved by the US Food and Drug Administration for treatment of postmenopausal osteoporosis, glucocorticoid-induced osteoporosis, and osteoporosis in men. IV zoledronic acid significantly reduced the risk of vertebral, nonvertebral, and hip fractures in postmenopausal women and decreased risk of clinical fracture and clinical vertebral fracture in men and women with hip fracture. Two promising new therapies are in late clinical development. Denosumab is a monoclonal receptor activator of nuclear factor- kappaB liga…

Denosumab: A Useful Addition to the Armamentarium for the Management of Male Osteoporosis.

Bone is a dynamic tissue. It remodels, preserving serum calcium, repairing microdamage, and maintaining strength. Osteoporosis is caused by a decrease in bone strength, which manifests clinically as low-energy vertebral and non-vertebral fractures. Osteoporosis poses a significant public health challenge. While it's often portrayed as primarily impacting postmenopausal women, there's been growing recognition among researchers and clinicians regarding its prevalence in men. Major fracture in men has higher mortality rates than in women. Denosumab is a fully human monoclonal immunoglobulin G2 (I…
