PURPOSE: Both heart failure and its treatment with diuretics or SGLT2 inhibitors increase fall risk in older adults. Therefore, decisions to continue or deprescribe diuretics or SGLT2 inhibitors in older heart failure patients who have fallen are generally highly complex and challenging for clinicians. However, a comprehensive overview of information required for rationale and safe decision-making is lacking. The aim of this clinical review was to assist clinicians in safe (de)prescribing of these drug classes in older heart failure patients.
METHODS: We comprehensively searched and summarize

Benzodiazepines (BZDs; including the related Z-drugs) are frequently targets for deprescribing; long-term use in older people is harmful and often not beneficial. BZDs can result in significant harms, including falls, fractures, cognitive impairment, car crashes and a significant financial and legal burden to society. Deprescribing BZDs is problematic due to a complex interaction of drug, patient, physician and systematic barriers, including concern about a potentially distressing but rarely fatal withdrawal syndrome. Multiple studies have trialled interventions to deprescribe BZDs in older pe

Long-term antidepressant pharmacotherapy is frequently continued beyond guideline-recommended durations. This occurs particularly among women, despite limited evidence of ongoing benefit for many people treated for mild-to-moderate depression and an increasing burden of adverse effects over time. In randomised trials, average symptom improvement over placebo is small (approximately 2 points on the 52-point Hamilton Depression Rating Scale), with an estimated number needed to treat of about 7-10 for a clinically meaningful benefit, underscoring the importance of planning for discontinuation onc

BACKGROUND: Patients with palliative needs often use potentially inappropriate medications, leading to adverse health outcomes. Deprescribing remains poorly enacted in primary care. Understanding the factors influencing deprescribing is essential for successful implementation into clinical practice.
AIM: To explore factors influencing deprescribing in community-dwelling patients with palliative needs in primary care from the perspectives of patients, their caregivers and primary healthcare practitioners.
DESIGN: Qualitative systematic review using thematic synthesis, registered in PROSPERO (

In this case report, a 60-year-old patient was referred to a multidisciplinary pain clinic to discontinue methadone. Before referral, a self-managed dose reduction from 40 to 15 mg had led to loss of function due to withdrawal symptoms. After referral, methadone was successfully deprescribed over three months. Withdrawal symptoms were managed through an individually tailored plan, supportive consultations with a nurse, and the use of clonidine. The patient regained function after deprescribing methadone. Deprescribing opioids can be managed through a patient-centered approach and close monitor

INTRODUCTION: Hypertension is highly prevalent among older people, and the balance of benefit and harm of antihypertensive therapy may shift with age. In certain cases, reducing or discontinuing antihypertensive treatment (deprescribing) may be appropriate. This systematic review and meta-analysis aimed to summarize available evidence on deprescribing antihypertensive medications in older adults aged 65 years and older.
METHODS: We searched MEDLINE, Embase, CINAHL, the Cochrane Library, the Web of Science Core Collection, ClinicalTrials.gov, ICTRP and Epistemonikos from inception to July 2024

IMPORTANCE: Older patients using many prescription drugs (hyperpolypharmacy) may be at increased risk of adverse drug effects.
OBJECTIVE: To test the effectiveness and safety of a quality intervention intended to reduce hyperpolypharmacy.
DESIGN, SETTING, AND PARTICIPANTS: This randomized clinical trial allocated patients 76 years or older who used 10 or more prescription medications to a deprescribing intervention or to usual care (1:1 ratio) at an integrated health system with multiple preexisting deprescribing workflows. Data were collected from October 15, 2020, to July 29, 2022.

IMPORTANCE: Deprescribing of antihypertensive medications is recommended for some older patients with polypharmacy and multimorbidity when the benefits of continued treatment may not outweigh the harms.
OBJECTIVE: This study aimed to establish whether antihypertensive medication reduction is possible without significant changes in systolic blood pressure control or adverse events during 12-week follow-up.

To investigate the effects of short-term discontinuation of methotrexate (MTX) on disease activity in patients with rheumatoid arthritis (RA) taking a stable dose of MTX. A post-hoc analysis of two randomized controlled trials was used to investigate the effects of temporary MTX discontinuation (for 2 weeks or 4 weeks) on responses to seasonal influenza vaccination. The impact of MTX discontinuation on the RA disease activity score (DAS28) and RA flare rate during discontinuation and after reintroduction was examined. The DAS28 increased during the 4-week MTX discontinuation period, before ret

Polypharmacy is common and associated with negative health outcomes in the elderly. We report the case of an 82-year-old man with a history of polypharmacy due to coronary artery disease, myocardial infarction, ischaemic cardiomyopathy, hyperlipidaemia, hypertension and persistent atrial fibrillation who presented with memory loss, cognitive impairment, fatigue and weakness. His treatment plan included an exclusively plant, whole-food diet and moderate physical activity which resulted in a rapid reduction of hyperlipidaemia and high blood pressure and the discontinuation of statin, antihyperte

WHAT IS KNOWN AND OBJECTIVE: In older patients, multiple chronic conditions lead to the intake of multiple medications and a higher risk of adverse drug events. The exposure to inappropriate medications in older patients with bleeding disorders is poorly explored. The aim of this study was to describe the exposure to potentially inappropriate medications (PIMs) and medications with anticholinergic and sedative properties in older community-dwelling patients with haemophilia or von Willebrand Disease (VWD).

Contemporary pharmacotherapy has improved disease-specific outcomes, but prescribing for multimorbidity remains largely guideline-driven, endpoint-focused, and insufficiently sensitive to cumulative treatment burden in patients with limited physiological reserve. Mitochondrial dysfunction and redox imbalance are shared biological processes that shape disease vulnerability, adaptive capacity, and susceptibility to drug-related toxicity; however, they are rarely operationalized in medication review or deprescribing. This mechanism-informed narrative review integrates mechanistic, translational,

Recent literature indicates that the benefits of aspirin use for primary prevention of atherosclerotic cardiovascular disease (ASCVD) may be equivocal to the risk. This has resulted in several organizations making updates to their clinical practice guidance on appropriate use. Despite this narrower scope, specific guidance on how to deprescribe aspirin in patients already on therapy for primary prevention is lacking. The purpose of this paper is two-fold: to provide a systematic review of the literature on aspirin deprescribing strategies and propose a practical aspirin deprescribing algorithm

INTRODUCTION: As people get older, they tend to take more preventive medication such as statins, beta-blockers and anti-coagulants to help prolong their lives. The risks of taking medication can start to outweigh the benefits in older people, and whether those with comorbidities want to extend these years of poor health is another consideration. One-third of older people will develop dementia, and they may not have the mental capacity to decide whether to continue or withdraw preventive medication. In these cases, deprescribing is left to advocates, such as healthcare professionals and family

BACKGROUND AND OBJECTIVES: Effective deprescribing requires shared decision making between a patient and their clinician, and should be used when implementing evidence-based deprescribing conversations. As part of the Shed-MEDS clinical trial, this study assessed barriers and enablers that influence patient decision making in deprescribing to inform future implementation efforts and adaptations.

OBJECTIVE: To assess how age-friendly deprescribing trials are regarding intervention design and outcome assessment. Reduced use of potentially inappropriate medications (PIMs) can be addressed by deprescribing-a systematic process of discontinuing and/or reducing the use of PIMs. The 4Ms-"Medication", "Mentation", "Mobility", and "What Matters Most" to the person-can be used to guide assessment of age-friendliness of deprescribing trials.
