Considerations in using psychotropic medication with dual diagnosis patients in recovery.

This article addresses some of the issues that arise when chemically dependent patients with coexisting psychiatric disorders require medication. A history of chemical dependence may alter prescribing practices, as certain medications may have a higher abuse potential or may enhance the danger of relapse to the primary drug of abuse. Recovering patients have complex attitudes and feelings toward medications that need to be explored, particularly as they affect compliance with prescribed regimens. Issues arising in the treatment of anxiety disorders, depression, and attention deficit disorders

ETHNOPHARMACOLOGICAL RELEVANCE: The classical prescription Kai Xin San (KXS) has a long history of clinical use and proven efficacy in treating depression. However, its active components and mechanisms of action require further elucidation. AIM OF THE STUDY: In this study, the pharmacological substance and mechanism of action of the anti-depressant effect of KXS will be investigated. MATERIALS AND METHODS: Wistar-Kyoto (WKY) rat served as a disease model. In this study, the depression-like degree was assessed by behavioral testing, and the efficacy of KXS in reducing disease was assessed usi

[Effects of chronic pancreatitis on salivary secretion].

In chronic pancreatitis, increased concentrations of Na+ and Cl- in sweat have been attributed to increased parasympathetic drive. It was postulated that similar changes might occur in saliva. To avoid masking increased parasympathetic drive, saliva was collected without stimulation. In patients with chronic pancreatitis, there were significant increases of concentrations of Na+ and Cl- in basal salivary concentrations. These differences disappeared when salivary secretion was stimulated with citric acid. Anatomic or neurologic modification of the salivary glands seemed unlikely as stimulated

Emetic activity of N-substituted norapomorphines.

Norapomorphine and ten of its N-substituted derivatives were prepared by modifications of procedures described earlier. In a dog emesis test the N-ethyl and N-n-propyl compounds had minimum effective doses of 0.00025 and 0.0005 mg/kg, respectively, when administered iv, sc, or im. In a modified Irwin mouse profile screen the minimum effective iv dose was 0.013 mg/kg for the N-ethyl and 0.0024 mg/kg for the N-n-propyl compound; percutaneous absorption was also observed in mice. All compounds examined caused the stereotyped apomorphine behavior syndrome but hypotensive effects were not serious.

The critical role of intracellular calcium in the mechanisms of plasticity of common snail defensive behavior command neurons LPl1 and RPl1 in nociceptive sensitization.

Studies on semi-intact common snail preparations addressed the involvement of intracellular calcium in changes in the excitability and responses to sensory stimuli of defensive behavior command neurons LPl1 and RPl1 during the acquisition of nociceptive sensitization. Application of sensitizing stimuli to the heads of control snails led to depolarization of neuron membranes, increases in neuron excitability, and depression of the responses of neurons to sensory stimuli during the short-term stage, and marked facilitation of responses in the long-term stage of sensitization. Acquisition of sens

State-specific GluCEST alterations in insular subregions are associated with depression and plasma inflammatory biomarker levels in patients with inflammatory bowel disease.

BACKGROUND: Depression commonly co-occurs with inflammatory bowel disease (IBD). Abnormal glutamate levels in the insula and altered plasma inflammatory biomarkers are observed in IBD and depression. However, the changes in glutamate concentrations in insular subregions in IBD and their relationship with depression and inflammatory markers remain unclear. This study aimed to investigate differences in glutamate concentrations in insular subregions between IBD patients and healthy controls (HCs) and their correlation with depression scores and inflammatory markers. METHODS: Forty-two IBD patie

The conservative management of self-inflicted chemical burns: Case report and literature review.

INTRODUCTION: Chemical burns, particularly injuries related to psychiatric illnesses, are underreported in the literature. We present two cases of self-inflicted alkali chemical burns managed conservatively. Frequent clinical review of the burn was aimed to ensure appropriate healing and compliance with the treatment plan based on regular dressing changes. CASE 1: A 24-year-old woman presented during the first day of the initial injury with a minor self-inflicted chemical burn to the right forearm. The causative agent was an oven cleaner containing sodium hydroxide triggering an alkali-based

Mechanistic insights into the role of plant polyphenols and their nano-formulations in the management of depression.

Depression is a condition characterized by low mood and an aversion to activity, that causes behavioral problems, poor quality of life and limits daily life activities. It is considered as the fourth leading cause of disability worldwide. Selective Serotonin Reuptake Inhibitors (SSRIs) Monoamine Oxidase (MAO) inhibitors, Tricyclic Antidepressants (TCAs), and atypical antidepressants are some of the conventional medications used to treat depression. However, only about half of patients with major depressive disorder (MDD) respond effectively to first-line antidepressant therapy. Additionally, t

Matured Hop Bitter Acids in Beer Improve Lipopolysaccharide-Induced Depression-Like Behavior.

Recent studies have demonstrated a close association between neural inflammation and development of mental illnesses, such as depression. Clinical trials have reported that treatment with non-steroidal anti-inflammatory drugs is associated with reduced risk of depression. Moreover, nutritional approaches for the prevention and management of depression have garnered significant attention in recent years. We have previously demonstrated that iso-α-acids (IAAs)-the bitter components in beer-suppress hippocampal microglial inflammation, thereby improving cognitive decline. However, effects of hop-

Thrombinogenesis and its pharmacological modulation in atherosclerosis.

Appearance of thrombin in circulating blood can be monitored in the clinical setting by measuring specific thrombin markers, such as fibrinopeptide A, thrombin-antithrombin III or prothrombin fragment 1 + 2. In myocardial infarction monitoring of thrombin activity is of growing clinical interest. High levels of thrombin markers indicate an increased risk to a patient with myocardial infarction. Persistent, high thrombin marker levels, despite heparin anticoagulation, point to ongoing thrombin generation that may necessitate more anticoagulation, increased antiplatelet treatment, angioplasty or

Pharmacological control of gastric acid secretion.

Gastric acid produced by the parietal cells has a significant role in a variety of gastrointestinal diseases, such as duodenal ulcer, gastric ulcer, and gastroesophageal reflux disease. We address acid production by the parietal cell and the pharmacological means to control gastric acid secretion. The physiology of parietal cell acid secretion is reviewed. The role of acid-suppressive therapy with histamine H2 antagonists and omeprazole in duodenal ulcer, gastric ulcer, and gastroesophageal reflux disease is discussed. Pharmacology, side-effect profile, and dosage requirements of the histamine

Mucus glycoprotein biosynthesis in the human gall bladder: inhibition by aspirin.

Aspirin, which inhibits mucin secretion in the gastrointestinal tract prevents gall stone formation in animals and may reduce gall stone recurrence in man. This study examines the effect of aspirin on mucin synthesis in human gall bladder explants. Two hundred explants were cultured with 3H-glucosamine (74 kBq/ml) for 24 hours at 37 degrees C. Mucin and other glycoproteins were isolated by papain digestion (72 hours) and exhaustive dialysis (144 hours) to remove non-incorporated radioactivity and digested protein. 3H-glucosamine was readily incorporated into glycoprotein. Pooled gall bladder e

[Test of pituitary adrenal suppressibility with I.V. dexamethsaone in the diagnosis of Cushing's syndrome (author's transl)].

The authors report a pituitary adrenal suppressibility test, using a 4 h IV infusion of dexamethasone phosphate (4 mg). Plasma cortisol is measured before and at the end of the infusion. The results allow a good discrimination between Cushing's disease (bilateral adrenal hyperplasia: 9 cases) and Cushing's syndrome of tumorous origin (adrenal tumors 7 cases, ectopic ACTH syndrome 3 cases, hypophyseal adenomas 2 cases), only bilateral adrenal hyperplasia cases are lowering significantly.

Effects of neostigmine and pyridostigmine at the neuromuscular junction.

The effect of subcutaneous administration of pyridostigmine or neostigmine for 7 to 15 days on neuromuscular transmission has been studied in the rat phrenic nerve-hemidiaphragm preparation. The quantal contents of end-plate potentials at different stimulus rates and the amplitude and frequency of miniature end-plate potentials were compared with those of untreated controls. The rate of release of acetylcholine quanta at high stimulus rates, and the frequency of miniature end-plate potentials, were reduced by pretreatment with both pyridostigmine and neostigmine. Presynaptic effects differed i
