Vilazodone for the treatment of major depressive disorder.

Major depressive disorder (MDD) affects 121 million people globally and is one of the leading causes of functional disability worldwide. As a recurrent disorder, MDD is associated with significant morbidity and functional disability as well as high direct and indirect costs to the health care system. Although several drug therapies are available for treating MDD, many patients do not achieve a sustained remission. Vilazodone was approved by the United States Food and Drug Administration in 2011 and has a distinctive pharmacology profile, as the drug is a selective serotonin reuptake inhibitor …

A brain-based endophenotype for major depressive disorder.

We have identified a brain-based endophenotype for major depressive disorder (MDD) that includes thinning of the cortex of the lateral aspect of the right hemisphere and the medial aspect of the left, as well as bilateral hypoplasia of frontal and parietal white matter. The endophenotype status of these abnormalities is supported by their presence in a multigenerational cohort of persons who themselves do not have MDD but who are at increased familial risk for developing the illness. Those who have the endophenotype but who are not ill nevertheless still suffer from inattention and poor visual…

Fronto-limbic volumetric changes in major depressive disorder.

BACKGROUND: Fronto-limbic dysregulation in major depressive disorder (MDD) may be influenced by early life stress and antidepressant treatment. The present structural MRI study aimed to determine the relationship between amygdala, cingulate and subgenual prefrontal cortex volumes in MDD and their associations with child abuse and antidepressants. METHODS: Right-handed subjects (21-50 years), meeting DSM-IV criteria for MDD, either with (n=19) or without (n=20) childhood sexual or physical abuse. Healthy controls (n=34) were matched for age, sex, education and smoking. 3D-MPRAGE images with a …

Impaired flexible decision-making in Major Depressive Disorder.

BACKGROUND: Depression is associated with dysfunctional affective states, neuropsychological impairment and altered sensitivity to reward and punishment. These impairments can influence complex decision-making in changing environments. METHODS: The contingency shifting variant Iowa Gambling Task (IGT) was used to assess flexible decision-making performance in a group of medicated unipolar Major Depressive Disorder (MDD) patients (n=19) and a group of healthy control volunteers (n=20). The task comprised the standard IGT followed by a contingency-shift phase where decks progressively changed r…

Enhanced visual motion perception in major depressive disorder.

Major depressive disorder (MDD) is a mood disorder that is not traditionally considered to affect the visual system. However, recent findings have reported decreased cortical levels of the inhibitory neurotransmitter GABA in occipital cortex. To explore possible functional consequences of MDD on visual processing, we applied a psychophysical visual motion processing task in which healthy young adults typically exhibit impaired perceptual discrimination of large high-contrast stimuli. It has been suggested that this phenomenon, spatial suppression, is mediated by GABAergic center-surround antag…

Duloxetine in the treatment of major depressive disorder.

Since depression impacts all body systems, antidepressant treatments should relieve both the emotional and physical symptoms of depression. … tly improved remission rates compared to placebo. Approximately 10% of the 1139 patients with major depressive disorder in placebo-controlled trials discontinued treatment due to an adverse event, compared to 4% of the 777 patients receiving placebo. In addition to nausea (1.4% incidence), which was the most common reason for discontinuation, dizziness, somnolence, a …

Depressive disorder as possible risk factor of osteoporosis.

Hypothalamo-pituitary-adrenal (HPA) axis is a very complicated control system playing an important role in stress reaction, where glucocorticoids suppress the autonomic (vegetative), endocrine, immunologic and psychic responses to stressful stimuli. We described the marked clinical, physiological, and biochemical connection between osteoporosis and major depressive disorder (MDD). Both conditions are associated with a hyperactive HPA axis and LC/NE system, and hence with increased CRH, cortisol, and catecholamine secretion. There are numerous states or diseases associated with osteoporosis and…

Minor depressive disorder in the context of miscarriage.

BACKGROUND: Although minor depressive disorder is of considerable clinical and public health importance, it has received limited research attention relative to major depressive disorder. This study examines the incidence rate and relative risk for minor depressive disorder following miscarriage. METHODS: Using a cohort design we tested whether miscarrying women are at increased risk for an episode of minor depression (diagnosed based on research criteria proposed in Appendix B of DSM-IV) in the 6 months following loss. The miscarriage cohort consisted of women attending a medical center for s…

Diagnostic discriminability of dysthymia and depressive personality disorder.

The Depressive Personality Disorder (DPD) is currently being considered for inclusion in future editions of DSM. However, there is little empirical research on DPD as currently defined in DSM-IV, including whether this disorder is sufficiently distinct from Dysthymic Disorder (Dysthymia). The goal of this study was to explore the relationship between DPD and Dysthymia with a sample of depressed outpatients. Scales were developed for the diagnostic criteria for both DPD and Dysthymia. These scales were then analyzed using cluster and factor analysis. Retroactive diagnostic assignment for these

Compensatory pituitary-thyroid mechanisms in major depressive disorder.

The thyrotropin-releasing hormone (TRH) stimulation test was administered to 47 patients meeting DSM-III criteria for major depressive disorder (with melancholia) and to 19 nondepressed patients. The wide variability of pituitary responses to TRH stimulation noted in the depressed patients provides evidence for the dysregulation of compensatory hypothalamic-pituitary-thyroid function in acute depression. Blunted thyroid-stimulating hormone (TSH) responses to TRH injection were found in 16 depressed (34%) and no nondepressed patients (p less than 0.01). Depressed patients who revealed blunted T…

Vascular correlates of circular type manic-depressive disorder.

An ambulant patient with a regular series of mood changes constantly varying between retarded depression and hypomania on a 32- to 36-day cycle was studied for 39 weeks. The patient completed a daily self-assessment of the 11-point Dorland mood scale each evening. This scale encompasses a range of moods varying from depression through euthymia to mania. Weekly recordings each lasting 65 min were made of resting heart rate, 14 parameters derived from electrical impedance plethysmography of the head together with arterial blood pressures before and after each recording. Results were correlated w

Sex effects on differentiating patients with major depressive disorder from bipolar disorder in depressive state: A fMRI study with follow-up.

BACKGROUND: Bipolar disorder (BD) is difficult to discriminate from major depressive disorder (MDD) before the appearance of mania or hypomania. This study was designed to identify whether patients with MDD and those who converted to BD are distinguishable using dynamic amplitude low-frequency fluctuations (dALFF) and describe the sex effects on the identification of the two disorders. METHODS: We compared the dALFF values of 35 BD patients who converted from MDD during the 2-year follow-up, 99 MDD patients, and 130 healthy controls (HCs) using two-way ANOVA. Pearson's correlation was used to…

Dissecting the shared genetic architecture of bipolar disorder, major depressive disorder, and attention-deficit hyperactivity disorder.

Major depressive disorder (MDD), bipolar disorder (BPD), and attention-deficit hyperactivity disorder (ADHD) are prevalent, highly heritable psychiatric disorders with significant degrees of genetic overlap. Using open-sourced summary statistics from the Psychiatric Genomics Consortium and 1000 Genomes European reference panel, we fit a latent factor model (F1) capturing the shared genetic liability across MDD, BPD, and ADHD. Multivariate GWAS identified 350 linkage disequilibrium-independent loci, 105 of which have not been previously reported by the contributing univariate GWASs. Univariate,…

Neural indicators of error processing in generalized anxiety disorder, obsessive-compulsive disorder, and major depressive disorder.

The ability to detect and respond to errors is critical to successful adaptation to a changing environment, and variation in error-related brain activity has been linked to psychopathology. The error-related negativity (ERN), an event-related potential component, represents a unique neural response to errors and is generated in the anterior cingulate cortex (ACC). In the present study, we measured the ERN in a sample of individuals with Generalized Anxiety Disorder (GAD), Obsessive Compulsive Disorder (OCD), Major Depressive Disorder (MDD), or some combination of the 3. Also included were 56 h…

Convergent and Distinct Transcriptional Signatures in Suicide, Bipolar Disorder, and Major Depressive Disorder.

Globally, a person dies by suicide every 40 seconds, underscoring a major global public health concern. In the United States, suicide is the second leading cause of death among individuals aged 10-34 years. Suicide is a complex, multifactorial phenomenon influenced by genetic and epigenetic factors and is strongly associated with mood disorders, particularly major depressive disorder (MDD) and bipolar disorder (BD). However, the extent to which their molecular signatures converge or diverge across disorders and brain regions remains incompletely understood. To address this gap, we conducted a …

Gut single-microbe landscape in patients with major depressive disorder and bipolar disorder.

Microbial communities in the human gut are highly diverse and complex, and many play critical roles in health and disease. Their functioning depends not only on species composition and diversity but also on intra- and intercellular transcriptional dynamics. Robust technologies capable of capturing single-microbe RNA sequencing information are urgently needed to understand microbial heterogeneity and host interactions. In this exploratory study, we applied droplet-based single-microbe RNA sequencing (smRNA-seq2) to analyze gut microbiomes from five patients with major depressive disorder (MDD),…

Differentiation between bipolar disorder and major depressive disorder based on AMPA receptor distribution.

An accurate diagnostic method using biological indicators is critically needed for bipolar disorder (BD) and major depressive disorder (MDD). The excitatory glutamate α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor (AMPAR) is a crucial regulator of synaptic function, and its dysregulation may play a central role in the pathophysiology of psychiatric disorders. Our recently developed positron emission tomography (PET) tracer, [11C]K-2, enables the quantitative visualization of AMPAR distribution and is considered useful for characterizing synaptic phenotypes in patients with psych…

Differentiation Across Bipolar Disorder and Major Depressive Disorder by Whole-Night Polysomnographic Findings.

Abstract. Background: There is growing evidence that understanding the role of sleep disturbance in bipolar disorder (BD) and major depressive disorder (MDD) is helpful when studying the high heterogeneity of patients across psychiatric disorders. Objective: The present study was designed to investigate the transdiagnostic role of sleep disturbance measured by polysomnography (PSG) in differentiating from MDD with BD. Methods: A total of 256 patients with MDD and 107 first-episode and never medicated patients with BD using the Diagnostic and Statistical Manual of Mental Disorders, Fourth Editi…
