Towards a patient-centred definition for atopic dermatitis flare: a qualitative study of adults with atopic dermatitis.

BACKGROUND: The term 'flare' is used across multiple diseases, including atopic dermatitis (AD), to describe increased disease activity. While several definitions of an AD flare have been proposed, no single definition of AD flare is widely accepted and it is unclear what the term 'AD flare' means from the patient perspective. OBJECTIVES: To understand AD flares from the adult patient perspective and to explore how adults with AD define an AD flare. METHODS: Participants were adults with AD recruited from the National Eczema Association Ambassadors programme, a volunteer patient-engagement p

Phenotypic and Endotypic Determinants of Atopic Dermatitis Severity From the Atopic Dermatitis Research Network (ADRN) Registry.

BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin condition with a highly variable clinical phenotype. OBJECTIVE: This study aimed to identify historical and clinical features and biomarkers associated with AD severity. METHODS: A US registry of extensively phenotyped AD participants (aged 0.73-80 years) were enrolled at 9 academic centers. Information on family and personal medical history, examination, skin swabs (culture), and serum biomarkers was collected to evaluate their association with AD severity. RESULTS: Participants with AD (N = 2862) whose disease was categoriz

Allergic contact dermatitis in children with and without atopic dermatitis.

BACKGROUND: Prevalence and causes of allergic contact dermatitis (ACD) in children vary with time and geographical area. OBJECTIVE: This study aimed to determine the relevant allergens causing ACD in children and the relation between ACD and atopic dermatitis (AD). METHODS: A cohort study on 349 children (0-15 years old) patch tested over a 7-year period was conducted. RESULTS: Patch test results were positive for at least 1 allergen in 69.3% of patients and were relevant in 69.8%. The highest sensitization rate (76.7%) was observed in children who are 0 to 5 years old (n = 86, 64% females)

[Malassezia species in patients with seborrheic dermatitis and atopic dermatitis].

Malassezia species, organisms normally colonizing the skin surface, are thought to play a role as either the cause or an exacerbating factor in a number of skin conditions, including pityriasis versicolor, Malassezia folliculitis, seborrheic dermatitis (SD) and atopic dermatitis (AD). Using a non-cultural PCR method, we analyzed Malassezia spp. extracted from the skin surface of SD and AD patients. The species most commonly detected in both patient groups were M. globosa and M. restricta, and the number of Malassezia spp. In these patients was higher than in healthy subjects. After a topical a

The Atopic Dermatitis Control Tool: Adaptation and Content Validation for Children and Caregivers of Children with Atopic Dermatitis.

INTRODUCTION: The Atopic Dermatitis Control Tool (ADCT) assesses six concepts regarding patient-perceived control of atopic dermatitis (AD) in adults and adolescents with AD. This study aimed to develop two modified ADCT versions, one for children with AD aged 8-11 years and another for caregivers of children with AD aged 6 months to 11 years. METHODS: Following the US Food and Drug Administration patient-reported outcomes guidance, the ADCT was modified to produce draft Child and Caregiver ADCT versions, maintaining the original six concepts. The instruments were refined and finalized throug

Phenotypes and Natural Course of Atopic Dermatitis: A Pathway to the Atopic March.

INTRODUCTION: Atopic dermatitis (AD) is a chronic inflammatory skin disorder that may progress to asthma and/or allergic rhinitis (AR) in children, a progression known as the "atopic march." METHODS: This study aimed to evaluate the natural history of AD, its clinical phenotypes, and the risk of progression to atopic march over a 10-year period. A retrospective review of medical records was performed for children diagnosed with AD and those with respiratory allergies who had prior AD. Patients were categorized into early transient, early persistent, and late-onset phenotypes based on eczema o

New Landscape for Atopic Dermatitis Therapies.

Targeted therapies have revolutionized the treatment of atopic dermatitis (AD) by offering patients highly effective disease control with complete or near-complete resolution. The American Academy of Dermatology updated their 2014 AD treatment guidelines in 2023. Since 2014, the treatment of AD has evolved immensely with the advent of biologics and small molecule inhibitors. While the foundation of AD treatment has stayed relatively consistent (ie, gentle skin care, emollients, and topicals), new therapies have supplanted previous systemic agents given their superior efficacy and safety profil

T-cell mechanisms in atopic dermatitis.

This review provides an overview of recent advances in understanding T-cell inflammation in atopic dermatitis (AD), a common chronic inflammatory skin disease. After recognizing their cognate antigen in the acute phase of the disease, the homing of T cells from the circulation into the skin via cutaneous lymphocyte antigen (CLA) is the prerequisite to skin inflammation and subsequent systemic and local, tissue resident, memory (TRM) formation. Initial observations suggest that antigen presentation occurs in structures such as induced skin-associated lymphoid tissue (iSALT), in addition to lymp

The primary mechanisms underlying atopic dermatitis.

INTRODUCTION: Atopic dermatitis (AD) is a complex skin disease frequently linked with other atopic symptoms such allergic rhinitis and asthma. The disease's history consists of persistent relapses with extreme pruritus, which lowers quality of life. AD has become a global health concern as its incidence has increased over the last few decades. It ranks as the third most common dermatologic disorder. AIM: There are several open questions about the mechanisms underlying atopic dermatitis (AD), This review aims to emphasize the recent advances in scientific research regarding the pathophysiologi

Infective Endocarditis Associated with Atopic Dermatitis.

We report a case of aortic valve infective endocarditis (IE) in a 24-year-old man with atopic dermatitis (AD). He had a history of balloon valvuloplasty for a stenotic bicuspid aortic valve, and had dental caries but no invasive dental procedure before the onset of IE. On admission, skin lesions of AD with itching and scratches were found on the neck, trunk, and extremities. Echocardiography showed a vegetation on the aortic valve with mild steno-regurgitation, but extension of IE to the annulus was not detected. Magnetic resonance imaging identified fresh cerebral infarction without neurologi

Systemic Treatment for Severe Atopic Dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory, relapsing disease of the skin, characterized by intense pruritus, maculopapular or vesicular erythematous lesions and scaling, sometimes accompanied by oozing, crusts and/or lichenification that has a negative impact on patients' quality of life. Prevalence is higher in children, around 15%, and approximately 5% in adults. Before introducing systemic therapy, it is mandatory to review patients' adherence to the correct use of topical treatments (corticosteroids, calcineurin inhibitors or cresoborole) and/or phototherapy. Ensure that environment
