Besides the common distal symmetrical sensory-motor polyneuropathy (DSP) that is often associated with autonomic dysfunction, diabetic patients may develop multifocal sensory-motor deficits (MDN) secondary to roots, plexus and nerve trunk involvement. Nerve ischaemia has been suggested as a common mechanism for the different patterns of diabetic neuropathies, yet the important clinical differences that exist between DSP and MDN suggest concurrent factors. In order to learn more on the subject, we prospectively studied 22 consecutive diabetic patients with MDN, for which other causes of neuropa

OBJECTIVE: Diabetic patients with incapacitating orthostatic hypotension can have either a "hyperadrenergic" or "hypoadrenergic" presentation. Although the latter is related to overt autonomic neuropathy, the former is proposed to be explained by appropriate autonomic responses. We hypothesize, however, that both conditions are part of a spectrum of autonomic dysfunction.
RESEARCH DESIGN AND METHODS: We studied 16 consecutive diabetic patients with preserved renal function referred for incapacitating orthostatic hypotension and characterized their autonomic and neurohumoral cardiovascular reg

Autonomic and sensory nerve dysfunction are progressive forms of diabetic neuropathies, often accompanied by other microvascular complications which are usually characterized by gradual onset of symptoms. Mononeuropathies, radiculopathies, motor palsies and acute painful neuropathies are more reversible forms of neuropathies. Their evolution is often unrelated to glycaemic control, vascular factors are more important in the pathogenesis and these complications are more common in middle aged and elderly men with type 2. diabetes mellitus. The diagnosis of neuropathies is based on following crit

Diabetes mellitus (DM) has a severe influence on the nervous system and it is more likely to occur on the nerves of the upper and lower extremities than on the cranial nerves. According to the statistics, the incidence of cranial nerve involvement ranges anywhere from 3% to 14%. The aim of this study is to perform facial nerve conduction studies in diabetic patients with peripheral neuropathy, confirmed by electrophysiological methods, to determine the frequency of affection of a cranial nerve conduction in a neuropathy which mainly occurs in a distal, symmetric fashion. The study was conducte

Diabetic autonomic neuropathy (DAN) is associated with a markedly reduced quality of life and poor prognosis. The manifestations of DAN cause multiple symptoms and involve the 1) cardiovascular system: resting tachycardia, reduced heart rate variability and circadian rhythm of heart rate and blood pressure, painless myocardial ischemia/infarction, orthostatic hypotension, exercise intolerance, perioperative instability, sudden death; 2) respiratory system: reduced ventilatory drive to hypercapnia/hypoxemia, sleep apnea; 3) gastrointestinal tract: esophageal motor dysfunction, diabetic gastropa

A 60-year-old woman, diagnosed as having a diabetic symmetrical proximal motor neuropathy, is presented. In March 1995, she was referred to us because of untreated diabetes mellitus since 1990. Insulin treatment during one month decreased her postprandial plasma glucose level from more than 400mg/dl to about 200mg/dl. Soon after the treatment, she noticed lower proximal limb weakness bilaterally. In several months, her weakness progressed as the fasting plasma glucose level was increased. Her muscle power gradually recovered when the plasma glucose level was normalized. We therefore suggest th

The utility of standard cardiovascular tests for diagnosis of cardiac autonomic neuropathy in diabetes has been well documented. Attention must be paid to standardizing the procedure with regard to time of day, metabolic status, distance from meal and insulin, coffee and smoking avoidance, and patient's collaboration. In the presence of cardiovascular disease or drugs affecting the cardiovascular or autonomic nervous system, some caution is needed in interpreting the results. More recent reflex tests, which evaluate mainly sympathetic or baroreflex activity, despite their ability to detect ear

The sympathetic skin response (SSR) was studied in 47 diabetic patients selected for the presence of symptoms and clinical signs of peripheral neuropathy and in 24 normal control subjects. The SSR was present in all controls but was absent at the foot in 66% and at the hand in 27.7% of the diabetic patients. Absence of the SSR failed to correlate with other electrophysiologic parameters on routine nerve conduction and electromyographic studies. Although absent SSR was more often found in patients with symptoms of autonomic dysfunction (P less than 0.05), there was no correlation with any speci

The importance of cardiac autonomic neuropathy (CAN) derives from its remarkable frequency and its clinical impact. The clinical features are postural hypotension and resting tachycardia, these abnormalities may be overlooked in a high number of patients asymptomatic. Although rarely life threatening, CAN causes considerable morbidity, which can be ameliorated by its identification and appropriate treatment. Circulatory reflexes were studied in 48 diabetic patients and 14 normal control subjects. Twenty-six of the diabetic patients had normal response. The remaining 22 had evidence of neuropat

This study was designed to correlate functional and ultrastructural examination of the innervation of the atria and vas deferens of rats with long-term alloxan-induced diabetes mellitus. After 7-8 months of diabetes the responses of preparations in vitro to nerve stimulation and to exogenous autonomic transmitter were compared with those from age-matched controls. Right atria from controls and diabetics were equally responsive to noradrenergic nerve stimulation and to exogenous noradrenaline. Left atria from diabetic rats were supersensitive to both noradrenaline and acetylcholine. The left at

Although nerve conduction slowing is a well-accepted abnormality in rats with acute experimental diabetes, reports of neuropathological changes in diabetic rat nerves have been inconsistent. To examine this further, we studied electrophysiological and morphological features of posterior tibial nerves and their distal branches from four-week streptozotocin-induced diabetic rats and matched controls. Diabetic rat posterior tibial motor conduction was slowed (mean +/- 1 SD, 34.8 +/- 3.1 m/sec; controls, 41.2 +/- 2.5 m/sec), and evoked muscle response amplitudes were only half of control values. U

Changes of heart frequency have been studied in diabetic neuropathy by blocking autonomic innervation of the heart with atropine and propranolol (intrinsic rate). It was found that in diabetic neuropathy the initial heart rate was higher than in the controls and in non-neutropathic diabetics, but this higher frequency could not be increased any further by the administration of atropine and propranolol compared to the values registered in the other two groups.

INTRODUCTION: Diabetic peripheral neuropathy (DPN) is one of the most common and debilitating complications of diabetes mellitus, affecting approximately 50% of patients with long-standing disease. While hyperglycemia is the primary driver of nerve damage, emerging evidence suggests that dyslipidemia may also contribute significantly to the development and progression of neuropathy. This study aimed to evaluate the clinical profile, lipid abnormalities, and nerve conduction patterns in patients with DPN and to explore the potential correlations between these parameters.

AIMS: Abnormalities in the oculomotor system may represent an early sign of diabetic neuropathy and are currently poorly studied. We designed an eye-tracking-based test to evaluate oculomotor function in patients with type 1 diabetes.
METHODS: We used the SRLab-Tobii TX300 Eye tracker®, an eye-tracking device, coupled with software that we developed to test abnormalities in the oculomotor system. The software consists of a series of eye-tracking tasks divided into 4 classes of parameters (Resistance, Wideness, Pursuit and Velocity) to evaluate both smooth and saccadic movement in different di

Nerve microvasculitis and ischemic injury appear to be the primary and important pathogenic alterations in lumbosacral radiculoplexus neuropathy of patients with (DLRPN) and without (LRPN) diabetes mellitus (DM). Here, we examine the involvement of inflammatory mediators in DLRPN and LRPN. Paraffin sections of sural nerves from 19 patients with DLRPN, 13 patients with LRPN, and 20 disease control patients were immunostained for intercellular adhesion molecule-1 (ICAM-1), tumor necrosis factor-alpha (TNF-alpha), interleukin-6 (IL-6), and nuclear factor kappaB (NF-kappaB). The findings were corr
