Aggregative Escherichia coli, Salmonella, and Shigella are associated with increasing duration of diarrhea.

In order to relate etiology of diarrhea to the duration of the episode, a cohort of 452 children up to 36 months of age was visited once weekly at their households for 18 consecutive months to record diarrheal morbidity. Fecal specimens were obtained in 453 diarrheal episodes occurring in 354 children during this period. The common putative agents as single isolations associated with diarrhea were EA-AggEC (17.2%), ETEC (14.1%), EPEC (6.0%) and rotavirus (4.0%). The pathogens with higher median duration of diarrhea were Shigella (13.5; mean +/- SD 13.7 +/- 2.7), Salmonella (8.8: 15.3 +/- 4.5)

Aetiology of bacterial diarrhoea in north Indian children with special reference to Campylobacter jejuni.

A total of 184 cases of acute diarrhoea, in patients aged 1-36 months, were studied clinicobacteriologically. Enteropathogens were isolated in 22.28% cases. Campylobacter jejuni was isolated from 2.17% cases only. Salmonellae were isolated from 7.6% cases and 64.28% of them were S typhimurium. More than 75% of all isolates of salmonellae, shigellae, Esch coli and Campylobacter jejuni were sensitive to gentamicin and cephaloridine. Secondary lactose intolerance was noted in 6.52% cases only.

Role of breast-feeding in the prevention and treatment of diarrhoea.

Recent studies have again shown the beneficial effects of breast-feeding in preventing morbidity and mortality from diarrhoea in infants. A case-control study in Brazil has shown that young infants who are not breast-fed have a 25-time greater risk of dying of diarrhoea than those who are exclusively breast-fed. A longitudinal study in the urban slums of Lima, Peru found that exclusively breast-fed infants have a reduced risk of diarrhoeal morbidity when compared with infants receiving only water in addition to breast-milk. Both these studies, along with numerous others in developing countries

Cryptosporidiosis: report of a case with life-threatening diarrhea.

A case of life-threatening chronic diarrhea, caused by Cryptosporidium spp., was reported. The patient was a seven month old boy from an orphanage suffering from voluminous watery diarrhea, fever and vomiting three days before admission. He presented with severe dehydration and severe hypokalemia. Numerous cryptosporidial oocysts were detected in the stools without other enteropathogens. Although co-trimoxazole and gentamicin were administered in the first week of illness, the diarrheal symptom was still persistent. At the end of the third week, spontaneous remission was observed. The overall

Pattern of protracted diarrhoea among children in Kuwait.

A total of 92 children with protracted diarrhoea were studied over a 3-year period (October 1983 to September 1986). The diagnosis was postgastro-enteritis syndrome in 61 (66.2%), coeliac disease in 17 (18.5%), Giardia lamblia infestation in 5 (5.4%), immune deficiency in 4 (4.4%), congenital chloride diarrhoea in 2 (2.2%), and glucose-galactose malabsorption, Shwachmann syndrome and intestinal lymphangiectasia in one each. Children with the postgastro-enteritis syndrome were young, malnourished and bottle-fed, and suffered a high incidence of antecedent bacterial infections. Six children died

Acquired chloride diarrhea.

Seven patients with intractable diarrhea were found to be excreting large quantities of chloride in the stool, associated with metabolic alkalosis, hyponatremia, hypokalemia, hypochloremia, and the excretion of chloride-free urine. They appear to have had an acquired, transient form of chloride diarrhea. We speculate that the diarrhea may have been perpetuated, in part, by fecal loss of chloride.

Prostaglandin synthetase inhibitor in an infant with congenital chloride diarrhoea.

Hyper-reninaemia, hypokaluria, and hypokalaemia in an infant with congenital chloride diarrhoea improved during treatment with a prostaglandin synthetase inhibitor, ketoprofen. There was evidence of increased activity of therenin-aldosterone system when ketoprofen was stopped. It is suggested that prostaglandins may be involved in stimulating the renin-aldosterone system in congenital chloride diarrhoea.
