Endovascular management of lap belt-related abdominal aortic injury in a 9-year-old child.

Blunt abdominal aortic trauma is a rare occurrence in children with only a few patients having been reported in the literature. Most such cases have been described in the context of lap belt injuries. We report a 9-year-old boy who suffered lap belt trauma to the abdomen during a high-speed road traffic accident resulting to the well-recognized pattern of blunt abdominal injury, that is, the triad of intestinal perforation, fractures of the lumbar spine, and abdominal aortic injury. The latter presented with lower limb ischemia due to dissection of the infrarenal aorta and right common iliac a

Aortic remodeling after thoracic endovascular aortic repair and Provisional ExTension To Induce COmplete ATtachment to treat complicated type B aortic dissection.

OBJECTIVE: Aortic remodeling after thoracic endovascular aortic repair (TEVAR) for complicated type B aortic dissection remains insufficiently characterized, particularly when the Provisional ExTension To Induce COmplete ATtachment (PETTICOAT) technique is applied. This study evaluated early geometric remodeling of the true and false lumens and the whole aorta following TEVAR with PETTICOAT using patient-specific computational modeling. METHODS: A single-center retrospective review of 20 patients (16 men; age, 65 ± 11 years; nine acute, seven subacute, and four chronic) with type B aortic dis

Short segment linear "flaps" of the abdominal aorta: proposed mechanism of origin and evaluation of natural history via retrospective imaging review.

PURPOSE: Luminal linear findings (LLF) of the abdominal aorta are often called aortic "flaps," triggering concern for acute aortic syndrome. However, these "flaps" are unlikely to represent intimomedial dissection given isolation to the abdominal aorta, short length, and incidental discovery. We aim to characterize the etiology and stability of LLF. METHODS: CT reports finalized January 2021-December 2022 were queried for terms "focal dissection," "dissection flap," "linear filling defect," and "linear flap." Patients were excluded for thoracoabdominal dissection, LLF in non-aortic vessel, no

Late Aortic Remodeling after Endovascular Repair of Complicated Type B Aortic Dissection-TEVAR Protects Only the Covered Segment of Thoracic Aorta.

BACKGROUND: TEVAR is the preferred way of treatment of complicated type B aortic dissection. The purpose of the study was to assess the impact of TEVAR on aortic remodeling in the thoracic and abdominal segment in long-term follow-up. METHODS: Twenty-three patients with complicated type B aortic dissection were treated by TEVAR in years 2004-2012 in our department. Aortic remodeling was rated based on preoperative and final follow-up computed tomography angiography. Aorta was measured at the level of left bronchus, the diaphragm, celiac trunk, and the aorta below the renal arteries. Diameter

Endovascular repair of abdominal aortic aneurysm associated with residual type B aortic dissection utilizing iliac branch endoprosthesis: a case report.

BACKGROUND: Iliac branch endoprosthesis (IBE) can preserve the internal iliac artery. Endovascular aortic repair (EVAR) for abdominal aortic aneurysms associated with residual type B aortic dissection has been rarely reported, and IBE has not been utilized in any of the reported cases. Herein, we describe a rare case of EVAR using IBE for an abdominal aortic aneurysm and a common iliac artery aneurysm associated with residual type B aortic dissection after thoracoabdominal aortic replacement. CASE PRESENTATION: A 70-year-old man underwent conservative treatment for acute type B aortic dissect

Successful short interval staged surgery in synchronous type A aortic dissection and giant abdominal aortic aneurysm patient.

The surgical therapy of synchronous type A aortic dissection and abdominal aortic aneurysm is complex and rarely reported, especially, when the abdominal aortic aneurysm is unsuitable for intervention. Recently, we have successfully performed sequential two staged surgeries on a 46-year-old woman. The first stage surgeries consisted of the Bentall procedure, total aortic arch replacement, and frozen elephant trunk implantation. The second stage surgeries included replacement from the descending aorta to the sub-renal abdominal aorta, reconstruction of the blood flow to the spinal, the celiac t

Acute non-traumatic disease of the abdominal aorta.

Acute pathology in the abdominal aorta is associated with significant morbidity and mortality. The most feared complication of abdominal aortic disease is acute rupture in the setting of atherosclerotic abdominal aortic aneurysm. Although frank rupture often is easily diagnosed on CT, other findings such as a hyperattenuating crescent, discontinuous intimal calcium, and draping of the aorta are subtle signs of aneurysm instability. A true aneurysm should be distinguished from a rapidly growing, saccular pseudoaneurysm in the setting of infectious aortitis, as treatment strategy differs. Acute

Magnetic Resonance Elastography of Regional Aortic Mechanics and True- and False-Lumen Stiffness in Aortic Dissection: A Feasibility Study.

OBJECTIVE: Regional aortic mechanics and the mechanical environments of the true and false lumens are not represented by maximal diameter alone. We evaluated whether magnetic resonance elastography (MRE) could provide paired descending thoracic and abdominal aorta stiffness estimates and separate true- and false-lumen estimates in thoracic aortic dissection. METHODS: We acquired 60-Hz descending thoracic and abdominal aortic MRE data in 10 healthy subjects and in 2 patients with chronic type B aortic dissections. Three motion-encoding directions and four temporal offsets were reduced to first

[Blunt trauma to the infrarenal abdominal aorta with neurologic lesions. Case report and review of the literature].

We report the case of a 31-year-old female involved in a severe motor vehicle accident. The diagnosis of blunt trauma to the abdominal aorta was not retained initially. The patient was referred to our institution when she developed a delayed paralysis of the lower limbs associated with the disappearance of both femoral pulses. Computed tomography evidenced dissection of the infrarenal aorta and NMR ruled out injury to the spinal cord. An aortoiliac endarteriectomy was then performed. Neurological recovery was partial at 3 months. We reviewed the frequency, the mechanisms and the management of

[Digital subtraction angiography of the aorta].

The results of 300 digital subtraction angiographies of the thoracic and abdominal aorta are analysed. Following intraarterial injection of contrast, all examinations were of good quality. Following I-V DSA, two thoracic (1.8%) and two abdominal (1.2%) aortograms were non-diagnostic. Anomalies in the position of the aorta, dilatation (aneurysm) or stenoses (coarctations, arteriosclerosis, occlusions) could be diagnosed by I-V DSA. One I-A DSA was performed for aneurysm of the ascending aorta in order to evaluate function of the aortic valve and for demonstrating the coronary artery origins, an

Volume analysis of true and false lumens in acute complicated type B aortic dissections after thoracic endovascular aortic repair with stent grafts alone or with a composite device design.

OBJECTIVE: This study compared the 12-month evolution of the aortic volume in patients who underwent thoracic endovascular aortic repair (TEVAR) with stent grafts alone or a composite device (Zenith TX2 stent graft and Zenith Dissection stent; Cook Medical, Bloomington, Ind) for acute (≤2 weeks from onset) complicated type B aortic dissection. METHODS: This was a retrospective analysis comparing prospectively collected data from 45 patients who underwent standard TEVAR repair at three European institutions (TEVAR group) and 39 patients in the Study of Thoracic Aortic type B Dissection using E

Type B aortic dissection after standard endovascular repair of abdominal aortic aneurysm.

Dissection of the aorta is a rare yet potentially serious complication following endovascular abdominal aortic aneurysm (EVAR). These can lead to visceral branch hypoperfusion, compromise of aneurysm exclusion, arterial dilation or rupture. Intimal injury and dissection in the context of EVAR may be associated with a number of risk factors that include adverse infrarenal neck morphology, device oversizing, barbed fixation and wire manipulation in the proximal aorta. Herein, we describe three cases of type B aortic dissection following EVAR and discuss possible causes. As the applicability of e

[Sudden/unexpected death due to spontaneous aortic rupture: two unusual manifestation forms].

Sudden unexpected death from a natural (pathological) cause often occurs under suspicious circumstances suggesting foreign intervention, so that a forensic autopsy is deemed necessary. In adults, a significant share of these deaths is due to spontaneous ruptures of the aorta. In figures, dissections of the ascending aorta and the aortic arch (mostly associated with secondary perforation into the pericardium) account for a large percentage followed by arteriosclerotic aneurysms of the abdominal aorta (with retroperitoneal and sometimes intraperitoneal hemorrhage). The authors report on two fata

A Novel Reverse Branch Technique for Reconstruction of a Renal Artery Perfused by the False Lumen After Thoracic Endovascular Aortic Repair.

PURPOSE: To describe an innovative endovascular technique that successfully reconstructs a renal artery completely perfused by the false lumen after thoracic endovascular aortic repair (TEVAR). CASE REPORT: A 65-year-old patient diagnosed with acute Stanford type B aortic dissection underwent successful TEVAR 4 years ago. Regular follow-up found that the thoracic aorta was well repaired, but the false lumen in the abdominal aorta had enlarged year by year. The left renal artery was supplied entirely by the false lumen, which caused kidney hypoperfusion. The abdominal aorta was successfully re

Aortic Dissection Associated with Autosomal Dominant Polycystic Kidney Disease.

A 78-year-old man who had been diagnosed with autosomal dominant polycystic kidney disease (ADPKD) and hypertension presented with chest pain. His family history was positive for ADPKD. Chest computed tomography (CT) revealed a type A aortic dissection with thrombotic occlusion of a false lumen and an ulcer-like projection in the ascending aorta, an aneurysm of the ascending aorta, and pericardial effusion. Abdominal CT showed multiple renal and hepatic cysts. At surgery, aortic dissection with thrombotic occlusion of the false lumen and an intimal tear in the distal ascending aorta were obser

Treatment of acute aortic type B dissection with stent-grafts.

PURPOSE: To evaluate the feasibility of endoluminal stent-grafts in the treatment of acute type B aortic dissections. METHODS: In five patients with acute aortic type B dissections, sealing of the primary intimal tear with an endoluminal stent-graft was attempted. Indication for treatment was aneurysm formation in two patients and persistent pain in three patients. One of the latter also had an unstable dissection flap compromising the ostium of the superior mesenteric artery. The distance from the intimal tear to the left subclavian artery was <0.5 cm in four patients, who had typical type B

An optimized retroperitoneal approach for open aortic repair by partially removing the tenth rib without incising the pleura and diaphragm.

The ninth intercostal thoracoretroperitoneal incision is the most common approach for open repair of suprarenal abdominal aortic aneurysm or abdominal aortic dissection, which necessitates incision of the pleura and diaphragm and may lead to cardiorespiratory distress postoperatively. We optimized this approach by partially removing the tenth rib, bluntly dissecting the diaphragm from its costal edge, and retracting the diaphragm superiorly. We successfully performed this approach in three patients with pararenal abdominal aortic aneurysm, abdominal aortic dissection, or renal artery aneurysm.

Primary aortoenteric fistula with a chronic isolated abdominal aortic dissection: report of a case.

A 47-year-old woman on long-term hemodialysis due to a chronic isolated abdominal aortic dissection was admitted to our department with severe abdominal pain. She had not suffered any hematemesis or melena. An emergency laparotomy revealed an abdominal aortic aneurysm with a diameter of 60mm, densely adhered to the ileum. An aortoenteric fistula manifesting as intramural rupture into the ileum was found after infrarenal abdominal aortic and bilateral common iliac cross-clamping. The fistula on the ileac side was nontransmural, but that on the aortic side communicated with the pseudolumen of th

Aortic Dissection: Spectacular Survival or Nightmare?

We describe the case of a male patient who remarkably survived two severe cardiovascular events: ascending aortic dissection and descending aortic dissection two years later. Later, the third occurrence of aortic dissection, this time involving the abdominal aorta, became an absolute nightmare for the patient, progressively being complicated by periaortic hematoma and severe anemia - events that ultimately led to his death.
