We present the case of a 73-year-old man affected by progressive and painful abdominal distension and paresthesia/hypoesthesia at the left leg. US and CT-scan revealed the presence in the left retroperitoneum of a large cystic mass without parietal thickening or enhancement after contrast injection. BACKGROUND: Gastric diverticula are infrequent anatomic abnormalities that are usually asymptomatic.
AIM: We report a case of large gastric diverticulum and discuss diagnostic and therapeutic challenges.
CASE REPORT: A 63-year-old man was admitted with vague epigastric discomfort. Physical examination and laboratory investigations were normal. Endoscopy revealed an erosive gastropathy and a pouch arising from the posterior aspect of the greater curve, 8 cm from the hiatus. Barium meal confirmed the presence of a 5 cm diameter diverticulum arising from the gastric fundus.

Nine cases of Meckel diverticulum seen over a period of four years are presented. Six presented with gangrene, perforation, intestinal obstruction, intraabdominal abscess and pain related to Meckel diverticulum. The remaining three were incidental findings. Seven of the patients were males and two females. Average age of the patients was 25.5 years. The paper reaffirms the well-known practice that Meckel diverticulum should be looked for at laparotomies and resected when seen.

To determine management guidelines for symptomatic duodenal diverticulum, we reviewed medical records of 26 patients. Complicated duodenal diverticulum was the only possible cause of symptoms-abdominal pain, fever and chills, melena, vomiting-in 18 patients. A case of incomplete intestinal obstruction caused by a giant Meckel's diverticulum in a 13-year-old girl is reported. Short resection of the ileum with the diverticulum and end-to-end anastomosis was carried out. The possibilities of diagnosis and treatment as well as the controversial classification of this rare congenital anomaly are discussed.
