The author suggested chromofibroduodenoscopy for the recognition of cystic duct patency in patients with acute cholecystitis. The method has certain advantages over the accelerated chromatic duodenal catheterization. Approbation of the method in the clinic showed its high informativeness on the condition of the cystic duct, which is very important in choosing the surgical tactics in acute cholecystitis, in determining the indications for the operation in particular. Complications associated with the use of the method were not encountered.

Whipple's disease is a rare, generalized inflammatory disorder due to the recently described bacterium Tropheryma whippelii. We report an unusual, successfully treated case of a 32-year-old woman, who presented with a 25 month history of large abdominal lymphomas, polyserositis and cachexia. The diagnosis of Whipple's disease was confirmed by duodenoscopy, lymph node and duodenal histology and polymerase chain reaction analysis of biopsy material and cerebrospinal fluid. A prolonged convulsive seizure with a subsequent 5 day period of coma were interpreted as signs of cerebral involvement. Und…

The significance of duodenal diverticula is studied, in particular with respect to the supposed role of juxtapapillary diverticular in the development of biliary and/or pancreatic diseases. Duodenal diverticula were found in 72 cases with the help of duodenoscopy and ERCP, i.e. in altogether 1% of the patients investigated by upper GI endoscopy. On the basis of their observations and the data reported in the literature, the authors conclude that the pathological role played by juxtapapillary diverticula in the development of biliary and/or pancreatic diseases is by obstructing the pancreatic j…

In the majority of instances, periampullary tumors include adenocarcinomas of the pancreatic head, duodenum, ampulla of Vater, and lower bile duct. Diagnosis is based mainly on a history of jaundice or is made by endoscopic duodenoscopy with retrograde pancreatography or by cholangiography or both. The best treatment for these tumors is pancreatoduodenectomy or palliative bypass if the tumor has spread beyond the region encompassed by resection. In experienced hands, resection can be accomplished with a mortality rate of less than 10 per cent and is followed by a 5-year survival rate of 30 to …

Hereditary diffuse gastric cancer (HDGC) is a rare and aggressive cancer that accounts for 1% to 3% of gastric cancer cases, often diagnosed late with poor outcomes, and it is due to a mutation in the Cadherin 1 (CDH1) gene. We present 2 sisters, ages 62 and 70, both carrying the CDH1 mutation, which is associated with significantly increased risks of HDGC and breast cancer. Both were diagnosed with invasive lobular carcinoma of the breast, a known manifestation of CDH1 mutations, emphasizing the need for genetic screening in patients with strong family cancer histories. Despite their elevated

Patients with neurofibromatosis type 1 are at increased risk of developing carcinoid tumours of the duodenum, particularly in the ampulla of Vater. Aggressive surgery with pancreatoduodenectomy is recommended for all ampullary carcinoid tumours because of their propensity to metastasise. We report a case of a 43-year-old woman with neurofibromatosis type 1, who presented with recurrent epigastric pain for 15 months. Evaluation using magnetic resonance cholangiopancreatography and side-viewing duodenoscopy revealed a submucosal tumour at the ampulla of Vater causing pancreatobiliary ductal obst…

A 24-year-old female patient with a 4-year history of recurrent abdominal pain, diarrhea, periodic fever and elevated C-reactive protein was referred to our institution. Colonoscopy, esophago-gastro-duodenoscopy, capsule endoscopy and contrast-enhanced CT showed no significant findings. Because she and her mother had sensorineural hearing loss, a diagnosis of cryopyrin-associated periodic syndrome was suspected. Genetic testing of the patient and her mother revealed a candidate germline variant of NLRP3. The patient was commenced on subcutaneous 150 mg canakinumab, a fully human monoclonal ant…

Signet ring cell carcinomas (SRCC) are aggressive neoplasms with poor prognoses, commonly found in the stomach, colon, and small intestine. However, their occurrence in the Ampulla of Vater is rare and their origin remains unclear. Among 163 cases of surgically treated ampullary carcinomas, we observed 4 cases (2.4%) of SRCC with jaundice being the primary clinical manifestation, followed by weight loss and abdominal pain. Duodenoscopy played a pivotal role in the diagnosis. Anatomopathological findings are detailed, emphasizing immuno-histological lineage and lymph node involvement. Two cases…

Sixteen patients with the congenital anomaly of pancreas divisum have been treated by enlarging the orifice of the accessory papilla. Attempts to do this at duodenoscopy succeeded in only 5 of 12 patients, and only one had a good clinical result. Sphincteroplasty was performed surgically in seven patients; five had a good or fair result. Most patients with poor results have undergone pancreatic resection. One patient died post-operatively; the remaining 15 divide equally as good (5), fair (5) and poor (5) results at follow-up after periods ranging from 7 months to 5 years. Results appear to be…

An 82-year-old man was admitted for acute cholangitis. He presented with jaundice and elevated hepatic cholestasis enzymes and C-reactive protein. Abdominal ultrasound and CT scan showed no evidence of gallstones and the main bile duct had 6 mm of diameter. Broad-spectrum antibiotic therapy was initiated and the patient was referred for Endoscopic Retrograde Cholangiopancreatography. During duodenoscopy, a pedunculated subepithelial lesion, measuring approximately 20-30 mm, and suggestive of being a lipoma was found, obstructing the papillary orifice and preventing its access. It was then remo…

A woman in her 70s with Churg-Strauss syndrome presented with epigastric pain. She was being treated with steroids at the time of admission. Computed tomography showed swelling of the gallbladder, and percutaneous transhepatic cholangiography revealed bloody secretion. On duodenoscopy, bleeding was observed from the orifice of the major duodenal papilla. Emergency cholecystectomy was performed under a diagnosis of hemorrhagic cholecystitis;intraoperatively, extensive hematoma was detected in the thickened wall of the gallbladder. Subsequent histopathological examination revealed mucosal ulcera…

Achalasia is usually a primary disorder of esophageal motility, but has been described in association with other pathological processes, suc … puterized tomographic scanning may be helpful, the most reliable diagnostic tool is esophago-gastro-duodenoscopy. This is a terminal disease with short life expectancy. Yet making the correct diagnosis can save the patient from futile treatment with muscle relaxants and endoscopic balloon dilatation, the accepted therapeutic measures in primary achalasia.

We report a case of von-Recklinghausen's disease presenting with obstructive jaundice and found to have a somatostatin-containing carcinoid tumor in the papilla of Vater and a small neurofibroma in the duodenum. A 42-year-old woman with von-Recklinghausen's disease presented with intermittent jaundice, pruritus, and mild steatorrhea of a two-year duration. Abdominal ultrasonography and computed tomography showed dilated intrahepatic ducts, common bile duct and pancreatic ducts. Duodenoscopy showed a tumor at the papilla of Vater, but a preoperative biopsy failed to provide a definite diagnosis…

Aorto-enteric fistulae are either primary or spontaneous, resulting from aneurysms of the aorta, or secondary to aortic surgery. Four symptoms are common to these two types, but every patient known to have an aortic aneurysm or a history of aortic graft surgery who presents with gastrointestinal bleeding or unexplained infection must be regarded as having an aorto-enteric fistula until proved otherwise. The fistula may be detected by duodenoscopy, but it is most often diagnosed at laparotomy, which is indicated when all paraclinical investigations are negative. While primary aorto-enteric fist…

Hemobilia is a syndrome caused by hemorrhage into the biliary ducts. It is characterized by colicky pains, jaundice, and intestinal bleeding; a correct diagnosis of this syndrome is made rather rarely. The most common cause is trauma; inflammatory or tumorous disease of the liver and bile duct system, bile stones and aneurysms of the hepatic artery are rarer causes. The diagnosis can be established and the arterio-biliary fistula localized by selective arteriography. A case is described, which shows, that, in addition, duodenoscopy with retrograde cholangiography is of diagnostic value. Other …

BACKGROUND: Helicobacter pylori is spreading worldwide with a high prevalence rate in the developing countries. Our primary goal was to measure the histology-based prevalence of Helicobacter pylori infection in children and to quantify its impact on the gastric inflammation and anemia. Our secondary goal was to study possible predictors for the presence of Helicobacter pylori in this cohort.
METHODS: A retrospective chart review was performed for children who underwent Esophago-gastro-duodenoscopy at Jordan university hospital in Jordan from 2008 to 2016. Data collected included epidemiolog…

Immediate post-operative complications such as obstructive jaundice, biliary fistulae or gastrointestinal hemorrhage, occurring after biliary tract surgery can be diagnosed endoscopically. The causes of bile flow obstruction and leakage from the common bile duct can be identified using ERC. In many cases the diagnosis can be followed by endoscopic treatment, therefore obviating re-laparotomy. Between 1972 and 1979 we carried out early post-operative ERC or esophago-gastro-duodenoscopy in a total of 94 patients after biliary tract surgery. In 75 patients with residual gallstones and 4 cases wit…
